331/1051. Pseudothrombophlebitis in neuropathic arthropathy.

    The pseudothrombophlebitis syndrome is a complication of large, ruptured, or dissecting popliteal cysts. We report a patient with neuropathic arthropathy of the knee and the pseudothrombophlebitis syndrome secondary to a dissecting popliteal cyst. This represents another form of arthritis associated with this syndrome. ( info)

332/1051. Radionuclide venography of lower limbs by subcutaneous injection: comparison with venography by intravenous injection.

    We have proved that subcutaneous injection (SC) of a small dose of Tc-99m pertechnetate (1 to 2 mCi: 37 to 74 MBq) at acupuncture points (K-3 and B-60) may offer an alternative method of radionuclide venography (RNV) of the lower limbs. In this study, we compared intravenous (IV) RNV and SC-RNV in 22 consecutive cases with typical signs and symptoms suggesting venous abnormality of the lower limb(s) from March to May 1988. They are 11 male and 11 female, aged 47.7 /- 15.7 years. Among the 44 limbs of the 22 cases, 4 were normal, 12 (27.3%) were found to have varicose veins in the legs only, 18 (40.9%) had partial stenosis of the deep veins (14 poplito-tibial and 4 superficial femoral), and 13 (29.6%) had complete stenosis of the deep veins (4 poplitotibial, 1 superficial femoral and 8 ilio-femoral. SC-RNV showed almost the same results as IV-RNV in 21 (47.7%), superior to IV-RNV in 22 (50%) (including 4.6% failure of IV-RNV), and inferior to IV-RNV in 1 (2.3%). We conclude that SC-RNV is definitely an alternative method of lower-limb venography. Since it is in most cases superior to IV-RNV, we suggest that it can take the place of IV-RNV in routine work. 333/1051. Metastatic adenocarcinoma of the cervix presenting as an inflammatory breast lesion.

    The occurrence of an inflammatory breast lesion and wide-spread venous thrombosis in a woman with metastatic adenocarcinoma of the endocervix is reported. Examination of biopsy specimens showed the inflammatory breast mass to be histologically consistent with metastasis from the endocervix. Recurrent venous thrombosis requiring large doses of heparin for control of the phlebitis was also a feature of this patient's illness. The disease was associated with elevation of serum CA 125 levels that did not parallel the patient's clinical course. ( info)

334/1051. Thromboembolic manifestations and congenital factor v deficiency: a family study.

    A case of congenital factor v deficiency is reported. Despite this defect in blood coagulation, the patient had experienced recurrent thrombophlebitis and was referred to us because of deep venous thrombosis of the lower limbs associated with pulmonary embolism. Both functional and immunological assays documented a deficiency of factor V (12 and less than 10%, respectively). The available family members were investigated and the same defect was found in 2 brothers of the propositus, who also suffered from thrombotic diseases (recurrent thrombophlebitis and myocardial infarction). The propositus has been treated with long-term oral anticoagulant therapy, no hemorrhagic complications or thrombotic recurrences being recorded in 2 years' time. ( info)

335/1051. Idiopathic extensive spontaneous venous thrombosis (a case report).

    Two patients with extensive spontaneous venous thrombosis are reported. Both had documented evidence of polyserositis, transient abnormalities of liver function tests along with normal coagulograms. Although one patient had a short, self-limiting illness, the other required treatment with coumarin derivatives. The relevant literature is discussed. ( info)

336/1051. Epithelioid hemangioendothelioma of the liver following contact with vinyl chloride. recurrence after orthotopic liver transplantation.

    Malignant epithelioid hemangioendothelioma of the liver is a recently recognized and uncommon neoplasm of vascular origin. Few cases have been reported and the treatment remains therefore difficult to define. A relationship to oral contraceptive use has been suggested but no other toxic agents have been incriminated. We report here the first case occurring after a close contact with vinyl chloride. Because of wide hepatic destruction and serious portal hypertension with bleeding varices, the patient underwent orthotopic liver transplantation. After a disease-free interval of 20 months, he died from variceal hemorrhage and encephalopathy due to local tumor recurrence with portal thrombosis. Nevertheless, orthotopic liver transplantation remains the only hope of salvage when extensive liver destruction and life-threatening complications are present. ( info)

337/1051. Complications of caval interruption by Greenfield filter in quadriplegics.

    patients with acutely injured spinal cords are thought to be at increased risk for thromboembolic disease and often have contraindications to anticoagulation therapy. From 1981 to 1986, 13 patients with quadriplegia at the new england Regional Model spinal cord Injury Center had caval interruption with a Greenfield filter. Twelve patients had deep venous thrombosis documented by venogram results and one had pulmonary embolism documented by arteriogram results. "Quad cough" chest physical therapy was required for mobilization of pulmonary secretions in nine patients. Follow-up abdominal x-ray results revealed significant abnormalities referrable to the filter in five patients having undergone "quad cough" therapy. Four patients had distal migration of the filter; three of the four had deformation of the filter. laparotomy for bowel perforation was required in two of these patients. quadriplegia requiring vigorous chest physical therapy ("quad cough") for pulmonary toilet may be a contraindication to caval interruption by Greenfield filter. Alternative techniques in the management of patients with quadriplegia and pulmonary compromise must be considered. ( info)

338/1051. mycoplasma hominis septic thrombophlebitis in a patient with multiple trauma: a case report and literature review.

    mycoplasma hominis was recovered from the site of a septic thrombophlebitis on the left cephalic veins of a patient with pelvic and other multiple trauma. The organisms were initially isolated from routine cultures in conventional blood agar media incubated anaerobically. The absence of other demonstrable pathogens and the patient's serologic response to the isolate support the role of the organism as the cause of this previously unreported mycoplasmal infection. M. hominis should be considered a possible cause of sepsis in selected cases of infections following pelvic trauma or manipulations of the genitourinary tract. ( info)

339/1051. Venous sonography for pregnancy-related thrombosis: case report.

    Venous sonography offers a reliable, noninvasive way of diagnosing deep venous thrombosis in the lower extremity, a condition with a potentially fatal consequence. It is possible that sonography may underestimate the degree of involvement of the veins. However this is not usually of clinical concern since systemic heparin therapy will be given regardless of the extent of the clot. ( info)

340/1051. Suppurative pylethrombosis: a changing clinical picture.

    In 2 patients presenting with fever of undeterminated origin, cryptogenic suppurative pylethrombosis caused by bacteroides fragilis was diagnosed. Combined antibiotic and anticoagulant therapy resulted in recanalization of the thrombosed vessels. The role of ultrasonography and computed tomography scan in the diagnosis and follow-up of this rare condition is discussed. ( info)
