1/14. Microcystic adnexal carcinoma: an unusual cause of swelling and paraesthesia of the lower lip.

    Microcystic adnexal carcinoma (MAC) is an uncommon, recently described, cutaneous adnexal malignant neoplasm, associated with significant morbidity as a consequence of its propensity for perineural invasion. The present report details the clinical and histological features of MAC in a young female presenting with lower labial swelling and paraesthesia.

2/14. Postlaparoscopic vulvar edema, a rare complication.

    Two cases of unilateral labial edema occurred after laparoscopic presacral neurectomy and were associated with massive chylous ascites. One woman was cured by a second laparoscopy to repair the chylous leakage. In the other, vulvar edema subsided in 2 days and chyloperitoneum subsided spontaneously in 3 weeks. The mechanism of postlaparoscopic vulvar edema is believed to be similar to that of Conn's postparacentesis labial edema, in which the unhealed puncture tract permits ascites to travel through and accumulate in the labia majora.

3/14. Severe granulomatous allergic tissue reaction after hyaluronic acid injection in the treatment of facial lines and its surgical correction.

    hyaluronic acid is considered to be nonimmunogenic. Frequently, it is used for the correction of facial lines. It is believed that hyaluronic acid injection fillers are safe and have no occurrence of serious adverse reactions or allergic reactions. Nevertheless, recent publications have documented the rate of intermittent swelling and severe granulomatous allergic reactions that evolved into abscesses. A clinical case of a 54-year-old patient is presented. After injection of hyaluronic acid in the treatment of nasolabial folds elsewhere, she developed palpable painful erythematous nodules evolving into abscesses several month after injection. Surgical treatment and correction of these lesions after hyaluronic acid injection of the nasolabial folds and the histological findings of these erythematous nodules are described. Histological and clinical examination documented intermittent swelling and severe granulomatous allergic reactions that may render the use of hyaluronic acid unacceptable. patients should be informed of the potential complications when treating facial lines with hyaluronic acid gel.

4/14. The use of water immersion for treatment of massive labial edema during pregnancy.

    The purpose of this article is to present a case of a pregnant woman with nephrotic syndrome who had unusually painful edema, and to share an intervention used to relieve her discomfort. The labial edema experienced by this patient was particularly problematic and interfered with normal activity; the hydrotherapy used in this case for treatment was successful in reducing the edema and the pain associated with it. When addressing atypical clinical problems such as this one, nurses can use evidence-based, inexpensive interventions to help their patients.

5/14. melkersson-rosenthal syndrome caused by saprodontia: a case report.

    A 24-year-old female patient with melkersson-rosenthal syndrome (MRS) in association with saprodontia is reported. She presented with lower labial swelling and left facial edema. Histological examination of the involved oral mucosa showed a noncaseating epithelioid granuloma. Results from the laboratory and imaging examinations were normal or negative. Her orofacial swelling disappeared after treatment of the saprodontia of the left first molar.

6/14. Adverse reactions during imatinib and lansoprazole treatment in gastrointestinal stromal tumors.

    OBJECTIVE: To report the case of a patient affected by gastrointestinal stromal tumors (GIST) who developed cutaneous adverse drug reactions during treatment with imatinib and lansoprazole. CASE SUMMARY: After 2 months of treatment with imatinib 400 mg/day, a 60-year-old white female affected by GIST developed bilateral palpebral edema with hyperemic conjunctivae and labial edema when lansoprazole 15 mg/day was introduced to treat dyspeptic symptomatology. Treatment was discontinued, and on reintroduction of both drugs, the patient developed stevens-johnson syndrome. Two months later, generalized cutaneous reactions appeared immediately following reintroduction of low-dose imatinib with corticosteroid plus lansoprazole treatment. After discontinuation of all drugs, with the exception of the corticosteroid, the progression of cutaneous lesions stopped. DISCUSSION: The use of imatinib is commonly associated with a high dose-dependent rate of rash and edema. Several cases of stevens-johnson syndrome have also been described, although not in patients affected by GIST. Severe skin reactions have been reported for lansoprazole including erythema multiforme, stevens-johnson syndrome, and toxic epidermal necrolysis. Applying Naranjo's algorithm, the adverse events were considered possible due to imatinib and probable due to lansoprazole. CONCLUSIONS: On the basis of the data reported, we conclude that the adverse reactions described may be attributed to either drug alone. However, combined use of drugs may increase the risk of onset of these adverse reactions due to a potential drug interaction involving CYP3A4.

7/14. Labial edema following treatment of condyloma acuminata with CO(2) laser in an adolescent: a case report and literature review.

    PURPOSE: To describe a case of bilateral labial edema in an adolescent patient and review the literature on this rare occurrence. MATERIAL: A 16-year-old girl was treated for widespread genital condyloma acuminata with CO(2) laser under general anesthesia and local lidocaine injections. Severe bilateral labial edema occurred after 48 hours. RESULTS: lidocaine and latex dermal patch tests for delayed hypersensitivity were negative. The edema resolved following treatment with local ice packs and magnesium sulfate compresses for 2 weeks. CONCLUSION: This is the first description of severe bilateral labial edema after CO(2) laser treatment for condyloma acuminata. We suggest that transient local lymphatic obstruction was the reason for this complication. The literature on this rare occurrence is reviewed.

8/14. Two unusual cases of dental (periapical) infection.

    Cutaneous sinus tracts of dental origin are often a diagnostic challenge. A delay in correct diagnosis can result in inappropriate treatments. We describe two unusual cases of periapical infection. The first patient presented with a fistulized tumoral mass under the chin present for 1 year. The patient had only one tooth, and that was anatomically unrelated to the tumor. Radiological examination showed a dormant tooth that was infected periapically. The second case was a man who complained of asymmetry and change in his left nasolabial fold of 4-5 week's duration. On examination, the nasolabial folds were asymmetric without any sign of facial palsy. On intraoral examination, there was a severely decayed tooth on the same side. Radiological survey confirmed periapical infection. A few weeks after proper treatment, the nasolabial folds became symmetric.

10/14. Genital oedema in patients treated by continuous ambulatory peritoneal dialysis: an unusual presentation of inguinal hernia.

    Four patients receiving continuous ambulatory peritoneal dialysis presented with scrotal or labial oedema but no detectable cough impulse. Two of the patients later developed clinical evidence of inguinal hernia, and in the other two inguinal hernias were found at operation. These cases suggest that genital oedema occurring in a patient receiving continuous ambulatory peritoneal dialysis is a sign of a small inguinal hernia which may not be detectable clinically.
