1/9. A case of ventricular tachycardia refractory to medical treatment related to a cystic mass in left ventricle--a case report.

    This is a case report of a twenty-two-year-old woman who had attacks of ventricular tachycardia that could be prevented by the use of various antiarrhythmic drugs for three months. Electrophysiologic study showed that the mechanism of the tachycardia was "reentry." In cardiac catheterization a non-contractile mass having calcification was seen on the apical portion of the left ventricle. Angiographically, the mass resembled a diverticulum. recurrence of attacks was terminated by resection of this mass, which had a cystic appearance.

2/9. Perinephric urinoma secondary to neurogenic bladder with vesicoureteral reflux: report of an adult case.

    We report a case of infectious perinephric urinoma in a 73-year-old woman who had a neurogenic bladder with vesico-ureteral reflux. The patient was admitted to our emergency room with right lumbago and high fever. Ultrasounds and computed tomography demonstrated a right large perinephric cystic mass, bilateral hydronephrosis and much residual urine. Percutaneous drainage of the cystic mass was performed with an indwelling urethral catheter. The content of the mass was urine infected with escherichia coli. Antibiotic therapy was performed successfully and we then examined the cause of the urinoma. A urodynamic study demonstrated a low-compliance small bladder and detrusor-sphincter dyssynergia. A voiding cystourethrogram revealed right grade III vesicoureteral reflux. The patient was unable to be cleared with intermittent catheterization and had an indwelling urethral catheter inserted. In 1 year, the voiding cystourethrogram showed no vesicoureteral reflux and the patient was well with no evidence of recurrent urinoma without the urethral catheter. There have been only two reported cases of urinoma caused by neurogenic bladder with vesico-ureteral reflux in children and this is the first case reported in an adult.

3/9. Suspicion of prenatal pyriform sinus cyst and fistula: a case report.

    There has been no report describing suspected prenatal pyriform sinus (PS) cyst and fistula. We report a case suspected by prenatal ultrasonography and fetal MRI. A large cystic mass was found in the left neck of the fetus. After the baby was born, preoperative laryngoscopic catheterization of the fistula tract was used to confirm the diagnosis and greatly facilitated the identification and excision of the PS cyst and fistula.

4/9. neck mass after catheterization of a neck vein in a child with ventriculoperitoneal shunt.

    There have been many reports on various mechanical complications after shunt operations in children with hydrocephalus such as catheter fracture, obstruction, disconnection, dislocation and so on. However, there are no previous reports regarding subcutaneous mass formation due to cerebrospinal fluid (CSF) pseudocyst from direct puncture injury. The authors report with a review of the literature a case of a child with ventriculoperitoneal shunt who developed a neck mass after catheterization of the neck vein. The authors also advocate that percutaneous procedures in the vicinity of shunts be avoided whenever possible to prevent the possibility of CSF leakage resulting in pseudocyst formation.

5/9. iliac artery pseudoaneurysm following umbilical artery catheterization.

    We report a case of a large iliac artery pseudoaneurysm in a newborn infant following umbilical artery catheterization. The aneurysm was excised and a primary repair was performed with limb salvage and reestablishment of normal blood flow to the foot. While these aneurysms are rare, prompt surgical repair should be performed as soon as the diagnosis is made because the associated mortality is high.

6/9. Ultrasound-guided drainage of fluid-containing masses using angiographic catheterization techniques.

    A new method combining experience of ultrasonic diagnostic and puncture technique with the use of existing angiographic tools is described. Using this method, intraabdominal fluid-containing mass lesions were drained for diagnostic and therapeutic purposes. In three patients, severe life-threatening lesions were successfully diagnosed and treated solely by this method. In one other case the method greatly facilitated major surgery. The importance of integration of diagnostic radiology and ultrasound is stressed.

7/9. urinoma formation secondary to ureteral obstruction by metastatic squamous cell carcinoma of the appendix. Case report.

    We report a case of secondary squamous cell carcinoma of the appendix causing spontaneous extravasation of urine and urinoma formation. On the basis of clinical findings, we interpret this rare lesion as metastatic from a cervical carcinoma surgically resected 8 years earlier. Ureteral continuity was restored by retrograde catheterization and the urinoma was drained percutaneously. The final diagnosis was made at laparotomy.

8/9. Renal rupture after the Crede maneuver.

    We report a case of renal rupture with development of a large perinephric urinoma and deterioration of renal function after the Crede maneuver in a girl with neurogenic bladder and detrusor sphincter dyssynergia without vesicoureteral reflux. After percutaneous drainage of the urinoma, management was changed to clean intermittent catheterization, and renal function returned to normal. We recommend that the Crede maneuver not be used in children with neurogenic bladder and nondenervated urethral sphincter.

9/9. pulmonary valve cyst in an adult. Case report.

    A case is described of an asymptomatic woman who had typical findings of mild pulmonic stenosis. Right heart catheterization revealed a 44 mm. Hg gradient across the pulmonary outflow tract, and a filling defect was discovered by angiography. On surgical exploration, the defect was found to be a benign cyst attached to the anterior leaflet of the pulmonary valve.
