1/16. Benign periurethral masses in women.

    Benign periurethral masses were encountered in 4 women. The masses included 2 vaginal leiomyomas, 1 of which was found simultaneously with a urethral diverticulum, a fibrocystic urethral mass and hyperplastic glandular tissue obstructing the bladder outlet and resembling male prostate. The physical and endoscopic features were helpful in establishing the benign nature and extent of the lesions. Although the bladder base was deformed on an excretory urographic cystogram in each case the radiographic features were non-specific except as they applied to a urethral diverticulum. Transvaginal needle biopsy is recommended in contemplation of open surgical excision. Leiomyomas are best approached transvaginally and anterior urethral masses can be reached suprapubically.

2/16. Paraurethral cyst: is conservative management always appropriate?

    Paraurethral cyst, arising from cystic dilatation of a paraurethral gland in a girl, is rarely reported in infancy. Although the lesion has a reported incidence of between 1 in 2000 and 1 in 7000 live female births, only 41 examples have been reported previously in the English literature. The management of this lesion is controversial. Surgical excision has been advocated, but spontaneous rupture has also been reported. The latter has prompted some authors to recommend non-operative treatment. We report a female infant whose paraurethral cyst failed to resolve despite a 6-month observation period. She eventually required surgery. The management of our case and the experience in the literature is discussed.

3/16. Transvaginal sonography in the diagnosis, management and follow-up of complex paraurethral abnormalities.

    Most urethral diverticula are thought to result from infection and obstruction of the paraurethral ducts and glands. When rupture of a resulting retention cyst or abscess occurs into the urethra, a diverticulum is formed. We report two cases of paraurethral abnormalities: urethral diverticulum and paraurethral abscess. Transvaginal sonography, with its high-resolution visualization of the lower urinary tract, may aid in the diagnosis and treatment of such disorders. Using three-dimensional technology, the internal architecture of the paraurethral abnormalities and their spatial relationship to the urethra and bladder, important considerations at surgery, are clearly demonstrated on ultrasonography. Complete excision of complex paraurethral anomalies may be performed under transvaginal sonographic monitoring without inadvertent injury to the bladder or urethra.

4/16. Skene's gland abscess with suburethral diverticulum in an adolescent.

    Suburethral diverticula have been reported to occur in 3-5% of adult females. They are rarely found in adolescents. We describe such a lesion that arose secondary to an abscess of Skene's glands in a 14-year-old sexually active adolescent.

5/16. Pronounced dilatation of Cowper's gland duct manifest as a perineal mass: a recommendation for management.

    It is generally accepted that large dilatations of Cowper's gland duct presenting as a perineal mass should be managed by perineal excision. Experience with such a case suggests that if the lesion also is visualized endoscopically, transurethral unroofing should be the initial choice of management.

6/16. Urethral cyst in a man.

    We report a case of a bulbous urethral cyst in a man. This rare lesion, probably arising from Cowper's gland, was treated successfully by endoscopic excision of the cyst wall.

7/16. Benign periurethral mass lesions: the female prostate.

    Bladder outflow obstruction in the female is usually a functional complaint, but rarely may be of organic aetiology, due to peri-urethral mass lesions. These lesions are usually midline in location and present clinical and roentgenological features comparable with those of prostate in the male. Two such patients, one of pure fibroma and another of hyperplastic glandular tissue are presented and their management is discussed.

8/16. Skene's gland calculi produced by a ureaplasma urealyticum infection.

    We report a case of calculi in a Skene's gland abscess produced by ureaplasma urealyticum. The enzyme urease, produced by the ureaplasma urealyticum, is thought to be the etiological factor in stone production, vaginitis and urethritis.

9/16. Urethroid cyst.

    cysts found on the ventral surface of the penis have been pathogenetically ascribed to defective embryologic closure of the median raphe, to anomalous developmental rests of the periurethral glands of Littre, and to the ectopic development of apocrine cystadenoma in the penile skin. This case reports a previously unrecognized but compelling pathogenesis for at least some ventral penile cysts, namely the anomalous congenital outgrowth of the entodermal urethral lining, giving rise to what is here designated as a urethroid cyst.

10/16. An unusual presentation of hidradenitis suppurativa: case report and review of the literature.

    hidradenitis suppurativa is a chronic suppurative and cicatricial process of apocrine gland-bearing skin. The clinical course can range from mild to severe. We present a case of a urethral fistula and phimosis occurring secondary to hidradenitis suppurativa, and review the literature.
