11/18. Deep vein thrombosis detection by 99m TC-MDP scanning.

    A new observation is reported wherein 99m Tc-MDP is incorporated into recanalizing thrombophlebitis. In one patient with subacute disease, inflammatory vascularity is appreciated during the first one to two minutes after injection. Both patients showed evidence of late phosphate uptake in their greater saphenous veins two hours later. Intravenous isotope injections may be given in an arm vein: foot injections are not required.

12/18. The association of septic thrombophlebitis with subperiosteal abscesses in children.

    An association of septic thrombophlebitis with acute osteomyelitis in four children is described. Each patient presented with physical findings consistent with thrombophlebitis. Venography, done in two, demonstrated significant acute thrombophlebitis, and another had an infected venous cutdown. A subperiosteal abscess was the predominant bony involvement found in all three patients who underwent surgical drainage. The initial radiographs were normal in each case. Bone scans showed diffuse increase in activity in the extremity, consistent with hyperemia, but no focal areas of increased uptake to suggest osteomyelitis. Computerized tomography, done in two patients, failed to reveal any bony abnormality, although subperiosteal abscesses were drained within 48 hours of each study. The existence of reverse collateral venous flow through the bone, associated with a rise in intramedullary pressure, may be responsible for these findings.

13/18. Surgical approach to extensive suppurative thrombophlebitis of the central veins of the chest.

    Suppurative thrombophlebitis is a potentially lethal intravascular infection associated with the usage of intravenous catheters. Conventional approaches to management include ligation and excision of involved venous structures. These measures may not be feasible in all situations. This paper describes the successful medical/surgical management of a patient with suppurative thrombophlebitis of the major vessels of the chest. When suppurative thrombophlebitis extends into the large central veins and antibiotic therapy fails, surgical intervention may be necessary.

14/18. Tampon-related toxic-shock syndrome. Histopathologic and clinical findings in a fatal case.

    Toxic-shock syndrome (TSS) is consistently associated with penicillin-resistant staphylococcus aureus in the vagina of menstruating women who use tampons. Increasing evidence implicates Staphylococcal toxin(s) in the pathogenesis of the condition, but the nature and biological effects of such toxin(s) are not established. Comprehensive postmortem studies and histopathologic data are few and largely unknown. The autopsy of a 15-year-old girl with this syndrome showed extensive superficial ulcerations and thrombophlebitis of the vagina, with bacteria on the mucosal surface only. Systemic phlebitis and capillaritis, visceral interstitial edema and acute inflammation involving various organs suggest a toxic effect, which is primarily manifested by widespread vascular damage. The histogenesis of the vaginal ulcerations remains unclarified, although they probably contribute to the absorption of toxin(s).

15/18. pyomyositis: tropical disease in a temperate climate.

    Two cases of pyomyositis or bacterial abscess of striated muscle in adults are presented. One patient was initially diagnosed as having acute thrombophlebitis of the lower extremity. Computerized tomography was helpful in establishing the correct diagnosis. The other patient presented with a closed compartment syndrome following blunt trauma. Both patients responded to open drainage and antibiotic therapy, although the diagnosis was delayed for over three weeks in one patient. Although common in the tropics, pyomyositis is unusual in the temperate zone. Unfamiliarity with this entity remains the major obstacle to appropriate management.

16/18. thrombophlebitis of the internal jugular vein secondary to pharyngitis.

    A case of septic thrombophlebitis of the internal jugular vein after pharyngitis is reported. Because of the common use and effectiveness of antibiotics against throat infections, this complication is now rarely seen. This condition has characteristic signs and symptoms, and a diagnosis can be obtained when the physician is aware of these characteristics. Radionuclide scan and doppler effect flow studies of the internal jugular vein are useful diagnostic adjuncts. If recognized early and treated promptly with antibiotics, resolution is the rule. The literature has been reviewed.

17/18. Bilateral suppurative thrombophlebitis due to staphylococcus aureus.

    Suppurative thrombophlebitis is an infection of the wall of a superficial vein, usually is associated with intravenous catheter placement, and accounts for about 10% of all nosocomial infections. Suppurative thrombophlebitis occurs most commonly in patients with burns, patients with cancer, and persons receiving steroids. skin flora organisms (e.g., staphylococcus aureus and to a lesser extent enterobacteriaceae) are the most common pathogens. Suppurative thrombophlebitis should be suspected when a patient having phlebitis presents with a temperature of 102 degrees F or higher. The diagnosis of suppurative thrombophlebitis is usually straightforward and made by demonstration of pus coming from the wound of the removed intravenous device or by aspiration of pus percutaneously from the involved vein. Treatment of superficial suppurative thrombophlebitis consists of venotomy of the affected vessel and systemic antimicrobial therapy. We present a case of S. aureus bilateral suppurative thrombophlebitis, which is most unusual.

18/18. Primary deep venous septic thrombophlebitis in a child.

    We describe a 12-year-old boy in whom primary staphylococcus aureus thrombophlebitis of the femoral and iliac veins developed without history of trauma and with no association with acute osteomyelitis. The patient responded to therapy with intravenously administered antibiotics for 6 weeks, although oxacillin-associated hepatitis developed during the course of treatment.
