1/25. Bilateral femoral osteomyelitis with knee arthritis due to salmonella enteritidis in a patient with systemic lupus erythematosus.

    A bilateral knee septic arthritis due to salmonella enteritidis developed in a female patient affected by long-standing systemic lupus erythematosus (SLE) with cardiac and renal involvement treated with immunosuppressants and corticosteroids. Because avascular necrosis and multiple osteomyelitic areas were detected at the same time in both right and left femoral condyles, an early localisation of Salmonella into the bone was assumed. Involvement of the joints was regarded as consequence of local dissemination of infection. Ampicilline (0.2 g/kg body weight daily for 2 months) plus ciprofloxacin (1.5 g daily for 12 months) and withdrawal of immunosuppressants appeared to be effective in preventing complications of infection.

2/25. Subdural empyema with extension to vertebral canal secondary to salmonellosis in a patient with systemic lupus erythematosus.

    CASE REPORT: A case of supratentorial subdural empyema extending to the superior subdural cervical space in a 14-year-old patient with systemic lupus erythematosus is presented. The presumed etiology of the empyema was an intestinal nontyphoidal salmonella infection. DISCUSSION: We review the neurological and neurosurgical complications in systemic lupus erythematosus.

3/25. salmonella typhimurium mediastinal abscess in a patient with systemic lupus erythematosus.

    We describe a case of a salmonella typhimurium mediastinal abscess in a patient with systemic lupus erythematosus (SLE). patients with SLE are predisposed to nontyphoidal salmonella infection with a high incidence of bacteraemia and abscess formation. Our case is the first report of a mediastinal abscess from salmonella typhimurium in an SLE patient and highlights the need for thorough assessment and treatment of SLE patients who have this organism identified.

4/25. Recurrent bacteraemia caused by different Salmonella species in a systemic lupus erythematosus patient.

    Non-typhoidal salmonella infections are increasingly being encountered. Recurrent systemic salmonellosis has previously been reported in both immunocompromised and immunocompetent hosts. However, recurrent salmonella bacteraemia by different species of salmonella has rarely been described. We report a patient with systemic lupus erythematosus having salmonella enteritidis and salmonella typhimurium infections after ingesting raw eggs and vegetables. Multiple internal organs, including left knee joint, left ovary and bones, were involved. The patient recovered with a prolonged course of antibiotics. This case illustrates that recurrent systemic salmonellosis can be caused by different species of pathogens, especially in immunocompromised hosts. Detailed food history and education on food hygiene is essential in making the diagnosis and preventing recurrence. A prolonged course of antibiotics may be needed to manage these cases.

5/25. CAPD-related peritonitis due to salmonella enteritidis in a patient with SLE.

    patients with systemic lupus erythematosus are prone to various infections, often associated with high mortality. Salmonella species are a rare cause of continuous ambulatory peritoneal dialysis (CAPD) peritonitis, but patients with lupus are susceptible to infection caused by Salmonella species. A 47-year-old woman who had systemic lupus erythematosus with end-stage renal disease and was undergoing CAPD presented with fever, watery diarrhea, abdominal pain, and turbid dialysate effluent. The effluent culture yielded salmonella enteritidis. Although she was administered antibiotics to which the organism was susceptible in vitro, peritonitis was not cured. Her clinical condition stabilized after removal of the Tenckhoff catheter and prolonged antibiotic use. In Salmonella-related CAPD peritonitis, early catheter removal may be necessary to achieve complete treatment.

6/25. Ovarian abscess caused by nontyphoidal Salmonella in a woman with systemic lupus erythematosus: a case report.

    BACKGROUND: Nontyphoidal Salmonella infection in a patient with systemic lupus erythematosus (SLE) is rare. CASE: A 24-year-old woman with a 9-year history of SLE, on corticosteroids, presented with severe abdominal pain, fever, vomiting and diarrhea for 1 day. Transabdominal sonography and abdominal computed tomography showed a right-sided well-defined pelvic mass. Laboratory study revealed leukocytosis. An emergency laparotomy revealed a right ovarian mass with purulent exudate. A right partial oophorectomy was performed. Pathologic examination revealed a hemorrhagic ovarian cyst with focal abscess. culture of the ovarian mass grew salmonella typhimurium. Postoperatively the patient did well and was discharged 5 days later. CONCLUSION: patients with SLE are at risk of Salmonella infection. Gynecologists should be aware of the possibility of an ovarian abscess caused by Salmonella in such patients presenting with a pelvic mass and fever.

7/25. Salmonella group C necrotizing fasciitis: a case report and review of the literature.

    Localized salmonella soft tissue infections have been rarely described in humans. We report a case of necrotizing fasciitis caused by Salmonella serogroup C in a patient with systemic lupus erythematosus who was successfully treated with surgical debridement and cefoperazone-sulbactam. In addition, we provide a literature review on risk factors and treatment of this uncommon disease sequelae.

8/25. Coexisting pneumocystis carinii pneumonia, cytomegalovirus pneumonitis and salmonellosis in systemic lupus erythematosus.

    Infection with opportunistic organisms, either singly or in combination, is known to occur in immunocompromised patients. A patient with systemic lupus erythematosus who developed pneumocystis carinii pneumonia, cytomegalovirus pneumonitis, and salmonellosis is reported. She responded to early treatment with intravenous trimethoprim-sulphamethoxazole (20 mg/kg).

9/25. Epididymo-orchitis and testicular abscess caused by salmonella enteritidis in immunocompromised patients in kuwait.

    OBJECTIVE: To report relatively uncommon presentations of epididymo-orchitis and testicular abscess caused by Salmonella spp. in 2 immunocompromised patients. CLINICAL PRESENTATION AND INTERVENTION: A 56-year-old man, a known case of systemic lupus erythematosus on azathioprine and prednisolone therapy, developed urinary tract infection followed by bacteremia and epididymo-orchitis. Both urine and blood cultures yielded salmonella enteritidis strains, which were demonstrated by pulsed-field gel electrophoresis typing method to be genotypically identical. The second patient, a 55-year-old diabetic (type II), presented with a testicular abscess from which a pure culture of S. enteritidis was obtained. Both were treated with intravenous piperacillin and amikacin followed by oral ciprofloxacin, responded well to the therapeutic regimen and were discharged home well. Their follow-ups were uneventful. CONCLUSION: This report shows that it is important to consider Salmonella infection in the differential diagnosis of inflamed and tender testis in immunocompromised patients and to include blood, urine and stool cultures in all cases.

10/25. Salmonella arizona arthritis and septicemia associated with rattlesnake ingestion by patients with connective tissue diseases. A dangerous complication of folk medicine.

    snakes constitute the main reservoir of Salmonella arizona, which are opportunistic pathogens in patients with serious underlying diseases. The 2 may meet when such patients ingest uncooked snake flesh, most often as a folk remedy for arthritis or other conditions. We have seen 11 patients in whom Salmonella arizona infection was documented. Six had systemic lupus erythematosus and another had dermatomyositis and are described in detail. All 7 had received prednisone, which was combined with azathioprine in 3. Five developed septic arthritis, including the site of a hip prosthesis in one patient. A history of dessicated rattlesnake ingestion as a "natural" remedy in either capsule or powder form was obtained in all but one of the 7 patients. patients often think that if natural or folk remedies are not helpful they also are not harmful and, therefore, safe and worth trying. We disprove that belief and call attention to the perils of one such remedy: dessicated rattlesnake, particularly when ingested by patients with connective tissue diseases who may be immunocompromised.
