1/25. Acute ethanol intoxication during pregnancy and consecutive fetal cardiac arrest: a case report.

    Chronic alcohol exposure during pregnancy and the resulting toxic effects for the fetus has been the subject of many investigations. In contrast, acute alcohol intoxication during pregnancy is a rare event and less is known about the consequences for fetal life. We report a case of the acute ethanol intoxication of a pregnant woman at the 35th week of gestation and the consecutive cardiac arrest of the neonate. Despite the life threatening event, the newborn recovered after resuscitation and intensive care treatment and could be discharged from hospital in good physical condition. We suggest that acute alcoholized pregnant women should be transferred to Perinatal Centers to cater for the possible need for emergency cesarean section and resuscitation of the newborn.

2/25. Acute iron intoxication in pregnancy: case report and review of the literature.

    Limited reports exist regarding acute iron intoxication during pregnancy. The maternal and fetal effects of accidental or deliberate ingestion of large amounts of iron may be catastrophic. A case report of acute iron intoxication, management strategies, and a review of the literature are presented. The cornerstones of effective therapy are aggressive management with emesis induction or gastric lavage, bicarbonate instillation, vigorous intravenous hydration, and chelation therapy with deferoxamine.

3/25. Maternal water intoxication as a cause of neonatal seizures.

    A term infant was admitted at 6 h of age with seizures related to hyponatraemia. During the last hours of labour the infant's mother had drunk 3 L of water. After delivery the serum sodium was 121 mmol/L in the mother and 126 mmol/L in the infant. Both resolved spontaneously. We discuss this case and the impact of maternal fluid intake during labour on the fetus and neonate. Women should be advised that excessive oral fluid intake during labour could adversely affect both mother and infant.

4/25. Mega-dose carbamazepine complicating third trimester of pregnancy.

    A case of coma due to carbamazepine ingestion with the intention of committing suicide at 33 weeks' gestation is presented. Management included activated charcoal administration and exchange plasmapheresis. A fetal non-stress test was non-reassuring but the apgar score, cord blood gases and early neonatal outcome were normal. Differential diagnosis of coma in pregnancy should include investigation for drug intoxication.

5/25. Telencephalic dysgenesis associated with presumptive maternal carbon monoxide intoxication in the first trimester of pregnancy.

    We report the association of telencephalic dysgeneses (expected to occur around week 6 of gestation) with presumptive maternal carbon monoxide intoxication. This case supports the hypothesis that carbon monoxide intoxication at critical periods of human brain development may lead not only to decreased brain size and hypoxic-ischemic lesions, but also to dysgeneses.

6/25. Diffuse metanephric adenoma after in utero aspirin intoxication. A unique case of progressive renal failure.

    Diffuse persistent glomerular immaturity and focal proximal tubular ectasia were seen in bilateral open renal biopsy specimens for an infant with fluid and salt depletion and slowly progressive renal failure. Subsequently, diffuse tubulopapillary renal adenoma subtotally replaced each kidney, thereby, necessitating renal transplantation. Origin of diffuse metanephric adenoma from persistent primitive epithelium of the proximal nephron is postulated and partly substantiated. We propose that this case of persistent proximal nephronic epithelial immaturity and diffuse metanephric adenoma is a variant of nephroblastomatosis and that in this case, a first trimester suicide attempt with aspirin may have initiated the maturation defect that preceded neoplastic transformation.

7/25. fetal death due to nonlethal maternal carbon monoxide poisoning.

    fetal death due to acute carbon monoxide poisoning is rarely reported in the medical literature. Of the eight cases found in literature review, only one documented the fetal carboxyhemoglobin concentration. This paper reports a fetal death due to accidental nonlethal maternal carbon monoxide intoxication in which both maternal and fetal carboxyhemoglobin concentrations were obtained. The corrected carboxyhemoglobin concentration was 61% at the time of death in utero, while the maternal carboxyhemoglobin was measured at 7% after one hour of supplemental oxygen. The authors review the mechanisms of fetal death and emphasize the different carbon monoxide kinetics in the fetal circulation.

8/25. The effects of acute alcohol intoxication on biophysical activities: a case report.

    A computer-assisted biophysical assessment was performed in a woman with chronic alcoholism at 37 weeks' gestation. She was first seen in a state of acute alcohol intoxication (322 mg/dl). Although fetal breathing movement incidence was normal (30%), fetal tachypnea (67 breaths per minute) and decreased fetal body movements (0.11%) were seen. Because these findings differ from those previously reported with regard to normal pregnancies exposed to lower levels of alcohol, their implications are discussed.

9/25. paraquat intoxication during pregnancy: a report of 9 cases.

    The details of nine patients who deliberately ingested paraquat (24% solution) while pregnant are presented. Two patients refused treatment. paraquat levels in maternal, fetal and cord blood in 1 case shows that paraquat crosses the placenta and is concentrated to levels 4-6 times greater than the maternal blood. Amnioscopy in another case showed paraquat levels in amniotic fluid nearly twice that of maternal blood. All fetuses died, whether or not emergency cesarean operation was performed. The condition of the fetus worsened at delivery, or in utero if the gestational age was greater than 30 weeks. One of the 2 survivors has had a normal pregnancy since, with no evidence of teratogenicity from the earlier paraquat intoxication. This patient has shown a gradual return to normal spirometry values from the marked reduction that occurred at the time of paraquat intoxication.

10/25. Nutmeg intoxication in pregnancy. A case report.

    A case of acute anticholinergic hyperstimulation in a pregnant woman was associated with excessive ingestion of nutmeg. The typical symptoms and signs, differential diagnosis and therapy are reviewed, with a special emphasis on the pregnant state.
