1/21. Flexural erythematous eruption following autologous peripheral blood stem cell transplantation: a study of four cases.

    Autologous bone marrow transplantation and autologous peripheral blood stem cell transplantation (APBSCT) are alternative therapeutic options in the treatment of various malignancies. We describe four patients undergoing APBSCT for malignancies; they developed a cutaneous eruption characterized by confluent erythematous and hyperpigmented patches within the flexural areas during the first month after transplantation. The lesions were poorly circumscribed without epidermal changes such as scaling, xerosis, erosions or atrophy. The skin patches were treated with topical corticosteroids and resolved within a few days with discoloration. Histopathological findings were characterized by focal vacuolar degeneration of the basal layer with epidermal dysmaturation. We believe that these cutaneous eruptions are consistent with an interplay of high-dose chemotherapy and local factors such as friction, local skin temperature and eccrine gland distribution, which could explain the constant location of this eruption in the axillae and genital area.

2/21. Cowden disease. Report of two additional cases.

    Two patients with Cowden disease, a syndrome consisting of cutaneous papules and nodules on the face, oral mucous membranes and dorsal aspects of the forearms and hands in association with tumours of the thyroid gland, breasts, gastro-intestinal tract, and female reproductive tract, are reported. This disease is important to the physician because recognition of these cutaneous lesions may allow early diagnosis of internal neoplasia.

3/21. Surgical potpourri.

    Six unusual surgical case are reported and the literature reviewed. The cases are: 1. Adenoid cystic carcinoma of the submaxillary gland 2. Parathyroid carcinoma 3. Squamous cell cancer of the pancreas 4. leiomyosarcoma of the abdomen (two cases) 5. myelolipoma of the adrenal gland These are representative cases from a general surgeon's practice, emphasizing the variety of interesting patients that the general surgeon cares for, and the scope of surgical knowledge that must be continually maintained. One of the attractions of having a practice in general surgery is that it provides the opportunity to diagnose and treat patients with a variety of conditions requiring surgical intervention. Reviewed here are six recent operative cases, their presentation, and management.

4/21. Special sunrise & sunset solar energy stored papers and their clinical applications for intractable pain, circulatory disturbances & cancer: comparison of beneficial effects between Special solar energy Stored paper and qigong Energy Stored paper.

    Various phases of solar energy were evaluated for possible medical application, using the Bi-Digital O-Ring Test. A 2-4 minute interval of highly beneficial phase during sunrise and sunset which is comparable or is stronger than ( ) qigong Energy was detected. This energy was stored on 3 x 5 inch index cards. The sun energy stored on the exposed surface had a Bi-Digital O-Ring Test extremely strong positive ( ) response, and the opposite side of the index card which was not exposed to the sun showed an equally strong negative (-) response. When the Bi-Digital O-Ring Test strong positive side ( ) was applied to the patient's skin above various intractable painful areas with circulatory disturbances, including gangrenous pain, muscle pain, joint pain, & migraine headache, most of the pain disappeared or was significantly reduced within between 10 seconds and 5 minutes, with accelerated wound healing compared with qigong energy stored paper of the same exposure, which caused pain to disappear within between 1.5 minutes and 15 minutes. When this Special solar energy Stored paper was applied either directly to the skin above cancer positive areas or the midline of the upper chest above the thymus gland representation area, or the occipital area above the medulla oblongata, various cancer related parameters returned to close to normal values, with immediate clinical improvement. The beneficial effects of 10-60 seconds of application of the Special solar energy Stored paper lasted for between 7 and 40 days, depending on the individual and their environmental electromagnetic field, how the special solar energy was stored, and how it was applied to the patient.

5/21. Correlation of Doppler US tumor signals with neovascular morphologic features.

    Abnormal Doppler ultrasound signals were detected in 44 of 47 patients with primary malignant tumors of the liver, kidney, adrenal gland, or pancreas (94%). Two different signal types were noted: a high-velocity signal (n = 38) with Doppler shifts exceeding 3 kHz (at an insonating frequency of 3 MHz) and a very low-impedance signal (n = 9) demonstrating little systolic-diastolic variation. In three patients, both types were present. In 19 patients, histologic (n = 12) and/or angiographic (n = 16) correlation was available. Among 13 patients with angiographic studies and signals over 3 kHz, arteriovenous shunting was demonstrated in six. The ratio of the systolic to diastolic Doppler shift is a function of vascular impedance. This systolic/diastolic index was less than 3 in eight patients with histologic correlation. All eight had prominent vascular spaces, and the flow in such thin-walled, endothelium-lined spaces would account for the low-impedance signals. Of nine patients with systolic/diastolic indexes of 3 or less and angiographic correlation, three had marked and four had moderate tumor staining.

6/21. Presence of corticotropin-releasing factor-like immunoreactivity in human tumors.

    Corticotropin-releasing factor (CRF)-like immunoreactivity was measured by radioimmunoassay in human organs and tumors associated with and without ectopic adrenocorticotropic hormone (ACTH) syndrome. It was found to be distributed widely in the stomach, pancreas, adrenal gland, and various tumors (e.g., medullary thyroid carcinoma, small cell carcinoma of the lung, pheochromocytoma, and adenocarcinoma of the gastrointestinal tract and pancreas) in a concentration less than one tenth of that of the hypothalamus. Dilution curves of CRF-like immunoreactivity in tissue extracts paralleled that of synthetic rat (human) CRF. Sephadex G-50 gel filtration showed that a major CRF-like immunoreactivity in tissue extracts coeluted with synthetic rat (human) CRF. Results suggest that a material(s) closely related immunologically to CRF is present widely in normal and tumor tissues outside of the central nervous system.

7/21. hypercalcemia complicating childhood malignancies: a report of seven cases with some pathophysiological considerations.

    A group of seven children with different malignant processes presenting with hypercalcemia was studied. Bone destruction, diffuse metabolic abnormalities, abnormal acid-base homeostasis and recurrent hypercalcemia characterized these patients. A different mechanism leading to the production of hypercalcemia and/or bone destruction by cancer cells is considered. The results of this report suggest that parathyroid hormone production (P.T.H.) by the parathyroid glands is normal and that ectopic secretion of PTH or PTH-like material is negligible in these cases.

8/21. Metastatic melanoma of the lacrimal sac.

    The case history of a patient with metastatic melanoma obstructing the lacrimal sac is presented. To our knowledge, this case represents a unique metastatic pattern that has not been previously reported. The patient was first treated for a melanoma of the left arm, which was excised in continuity with the axillary lymph glands. She was free of disease for 3 years until she developed metastatic disease causing obstruction of the ipsilateral lacrimal sac. Anatomical details of the metastasis were provided by computed axial tomography. Treatment consisted of excision of the metastatic lesion with reconstruction using forehead and intranasal mucosal flaps, followed by irradiation, hyperthermia, and multiple-drug chemotherapy. Emphasis is placed here on the differential diagnosis of orbital adnexal tumors.

9/21. Papillary carcinoma arising from ectopic breast tissue in an axillary lymph node.

    A cystic carcinoma was located in an axillary lymph node of a 90-year-old woman. The lymph node also contained cytologically benign mammary ducts and a few small glands that had epithelium cytologically intermediate between the normal epithelium and the neoplastic cells. In a subsequent mastectomy specimen, a small intraductal papillary carcinoma was found, differing cytologically and architectonically from the nodal carcinoma. The nodal carcinoma is considered primary because of its morphologic differences from the mammary lesion, the in situ nature of the mammary tumor, and the transition of benign to atypical epithelium in the nodal inclusions. The occurrence of two tumors is viewed as a field effect.

10/21. Malignancies of the endocrine glands.

    patients with malignant tumors of the endocrine system can be effectively treated by surgery, the 5 year survival rate varying between 30-90 per cent, depending on organ involvement. In the Federal Republic of germany, 1 out every 5 persons dies as a result of malignancy. Less than 1 per cent of the total annual rate of such deaths relates to malignancies originating in the endocrine glands.
