1/8. Fibrous pseudotumor of tunica vaginalis and epididymis.

    Paratesticular fibrous pseudotumors are rare, and less than 10% of them affect the epididymis. We report a case of testicular trauma that progressed to a painless palpable tumor in the right hemiscrotum with increased local volume. orchiectomy followed by anatomic-pathologic examination revealed a paratesticular fibrous pseudotumor. The best treatment is scrotal exploration and frozen biopsy. On confirmation of the diagnosis, only the tumor and the tunica vaginalis are resected. Should the benign nature not be possible to determine, orchiectomy is the procedure of choice. We also reviewed published reports for this possibility in the differential diagnosis of testicular masses.

2/8. azoospermia due to testicular amyloidosis in a patient with familial mediterranean fever.

    We describe a patient suffering from familial mediterranean fever (FMF) who presented to our clinic with secondary infertility of 2 years due to amyloid A amyloidosis. His spermiogram disclosed azoospermia. A testicular biopsy revealed hyalinized tubules devoid of full spermatogenesis and containing abundant amyloid, confirmed by congo red stain. We suggest that testicular amyloidosis be taken into consideration when dealing with azoospermic FMF patients. In view of the progressive nature of amyloid accumulation in the testis we propose to follow routinely the spermiogram of FMF patients with renal amyloidosis. Furthermore, consideration of sperm cryopreservation is suggested in these cases. In FMF patients with azoospermia consideration of testicular biopsy is recommended as early as possible in order to increase the chance of sperm retrieval.

3/8. Testicular epidermoid cyst--is pre-operative diagnosis possible?

    A case of testicular epidermoid cyst which had initially been misdiagnosed as a malignant testicular tumour is presented. The role of clinical and ultrasound examination in the pre-operative assessment is discussed. Although the benign nature of this condition must be demonstrated histologically before conservative surgery is contemplated, it is equally important that the patient should not be told that the tumour is malignant without adequate evidence.

4/8. Epidermoid cyst of the testis--benign teratoma in an adult. Case report.

    Epidermoid cyst of the testis, which is regarded as benign monodermal development of differentiated teratoma and comprises less than 1% of all testicular tumors, was found within the tunica albuginea in a 23-year-old man. Following peroperative histologic confirmation of its benign nature, the cyst was locally removed, thereby avoiding orchiectomy.

5/8. Testicular malacoplakia.

    We report the twentieth case in the world literature of testicular malacoplakia. This lesion occurs mainly in middle-aged men, appearing clinically as epididymo-orchitis or enlargement of the testicle with fibrous consistency and some soft areas. malacoplakia is now considered a nonspecific inflammatory granulomatous lesion subsequent to an infection by gram-negative bacteria. In our case escherichia coli was cultured in the urine and testicular tissue. The ultrastructural study shows the histiocytic nature of the cellular infiltrate as well as the morphology of Michaelis-Gutmann bodies. Bacilliform structures were not found. The cellular and humoral immunological studies did not show alterations.

6/8. Malakoplakia of testis.

    Two cases of malakoplakia of the testis are reported. Ultrastructural studies were performed in both cases and confirmed the histiocytic nature of the large cells seen in this granulomatous process. Characteristic Michaelis-Gutmann bodies are identified in those macrophages and seem to originate from altered lysosomes. From these observations and a review of the literature, the possible physiopathogenesis of the disease is summarized, and comments are made on the treatment of malakoplakia.

7/8. Inclusion cyst of the tunica albuginea: demonstration by ultrasound.

    A 35-year-old man discovered a 1-cm nodule at the upper pole of the left testicle after blunt focal trauma. While the pain, tenderness, and location suggested hematoma or appendiceal torsion, the demonstration by ultrasound of the size, cystic nature, and extraparenchymal location was consistent with the rarely documented cyst of the tunica albuginea.

8/8. Bilateral intratesticular cysts--a specific entity.

    A case of bilateral testicular simple cysts was seen in a 79-year-old Chinese man. The cysts contained clear fluid and were lined with simple cuboidal epithelia. The histochemical and immunohistochemical findings of this case were presented with an effort to clarify the nature of the cyst. In addition, the clinicopathological features, differential diagnosis and proposed histogenesis of the documented testicular simple cysts in the literature were also reviewed.
