1/3. Autonomic studies in hypertensive patient with unusual sexual dysfunction: response to reserpine.

    The authors examined a 48-year-old man with a history of hypertension (6 years) and a history of erection and ejaculation during cold exposure and confrontation with fear (20 years). Exposing the foot to cold water induced ejaculation in 10 seconds. A variety of antihypertensive medications resulted in control of hypertension, without improvement in ejaculatory dysfunction. Similarly, prazosin controlled the blood pressure, but not the episodes of ejaculation. Treatment with reserpine resulted in gradual reduction in blood pressure and an improvement in ejaculatory dysfunction.

2/3. Coital blood pressure in hypertensives. Cephalgia, syncope, and the effects of beta-blockade.

    During the continuous monitoring of intra-arterial blood pressure in ambulant hypertensive subjects, 18 episodes of coitus were recorded in 11 subjects. Peak values of up to 300/175 mmHg found, the mean for men being 237/138 mmHg and for women 216/127 mmHg. Two patients with coital symptoms (cephalgia and syncope) showed no unusual responses and, when restudied after beta-blockade, one subject showed no reduction in peak coital blood pressure. The results complement those found previously in normal subjects and demonstrate the potential of the technique in investigating coital symptoms.

3/3. Resisting the pressure to become a surrogate: a case study.

    Masters and Johnson's discussion of their surrogate program and informal interviews with surrogates reveal that the surrogate role is stressful. The surrogate hopes to bypass explosive relationship issues in order to lower the patient's level of tension. Such bypassing is difficult, sometimes impossible, and of limited therapeutic value. An alternative tactic has been developed for Individual sex Therapy and is exemplified by a case study. The case described involves an involuntary ejaculator who tried to sidestep the pressure that he felt from relationships in general. This case exemplifies the pressure in the Individual sex Therapy situation, how the surrogate approach attempts unsuccessfully to bypass this pressure, and the processes by which the pressure can be countered.
