2/5. Temporary trigeminal disorder as a result of pneumocephalus after subarachnoid block.A patient was scheduled for inguinal herniorrhaphy under subarachnoid block. Lumbar puncture was difficult and several attempts were needed before it could be achieved. During the immediate postoperative period, the patient developed paraesthesia and anaesthesia on the right side of the face, mostly in the nose, cheek and upper lip areas. A CT scan showed a small pneumocephalus at the level of the brainstem. 3/5. pneumocephalus following attempted epidural anaesthesia.This report describes iatrogenic pneumocephalus in an obstetrical patient following attempted epidural anaesthesia using the loss of resistance technique. On the fourth attempt at epidural injection, an apparent loss of resistance was identified and 5 ml air was injected. The patient complained immediately of severe bifrontal headache followed by emesis. The baby was eventually delivered by Caesarean section, with general anaesthesia and avoiding nitrous oxide. Rapid diagnosis was obtained by computerised tomography. This condition should be suspected whenever a patient demonstrates delayed recovery or neurological deficit after craniotomy and nitrous oxide anaesthesia. 5/5. Late tension pneumocephalus following frontal craniotomy.Tension pneumocephalus is an uncommon but life-threatening complication of craniotomy which requires urgent diagnosis and treatment. It most often occurs perioperatively following subsequent anaesthesia using nitrous oxide. All previously reported postoperative cases followed posterior fossa or upper spinal cord surgery.
