1/16. Managing a dropped nucleus during the phacoemulsification learning curve.

    Three patients had a pars plana vitrectomy to remove retained nuclei within 72 hours after phacoemulsification performed by a surgeon making the transition from extra-capsular cataract extraction to phacoemulsification. After vitrectomy, the nuclei were brought to the midvitreous cavity from the retinal surface with a posterior segment phacofragmenter, emulsified, and completely removed. Then, a posterior chamber intraocular lens was implanted in the sulcus through the previous cataract surgery incision and remained well centered postoperatively. postoperative complications included cystoid macular edema in 1 patient and choroidal detachment in another. No other complications were detected. Final visual acuity ranged from 20/60 to 20/30.

2/16. Traumatic anterior lens dislocation: a case report.

    A 45-year-old man presented to the emergency department complaining of decreased vision and pain in the left eye after blunt trauma to the eye. On evaluation, the vision was limited to detecting hand motions, and the intraocular pressure was 37 mmHg. Secondary acute angle-closure glaucoma, with pupillary block due to anterior dislocation of the lens, was diagnosed. The intraocular pressure remained elevated after medical therapy, and the patient underwent intracapsular cataract extraction and anterior vitrectomy. The possibility of elevated intraocular pressure due to lens dislocation or other types of secondary glaucoma should be considered after blunt ocular trauma.

3/16. eye injuries associated with paintball guns.

    AIMS: This study identifies the various types of ocular injuries sustained after blunt trauma with a paintball fired from a paintball gun. methods: We report two patients who sustained injury to an eye after being shot with a paintball and review similar cases presented in the world literature. The type of injury sustained and the final visual acuity obtained after a paintball hit to the eye are examined. RESULTS: The two boys presented were hit in the eye with a paintball resulting in lens subluxation, hyphema formation, and angle recession. cataract extraction was required in both cases. One boy also had an optic neuropathy and a choroidal rupture. A review of the literature reveals a variety of injuries occur after a paintball hit to the eye. In some of the cases, the damage to the eye has led to loss of vision and at times loss of the eye. CONCLUSIONS: Paintball guns can cause devastating ocular injuries. Wearing protective eye and face gear during this game is essential. We recommend that an anti-fog face mask with a one-piece polycarbonate eye shield be worn by those participating in paintball games.

4/16. Clear lens extraction and intraocular lens implantation in Marfan's syndrome.

    Intracapsular clear crystalline lens extraction and intraocular lens (IOL) implantation were performed in 4 highly myopic eyes of 2 patients with Marfan's syndrome. One eye of each patient received an anterior chamber IOL and the other, a scleral-fixated posterior chamber IOL. The preoperative spherical equivalent ranged between -14.50 and -28.00 diopters (D) and axial length range, between 25.32 and 36.02 mm. The SRK II formula was used. Mean uncorrected visual acuity improved from counting fingers to 20/80. Postoperative spherical equivalent correction ranged from -0.75 to 2.75 D. One eye had vitreous loss that was managed by anterior vitrectomy. Modern surgery for cataract and management of its complications suggest that clear crystalline lens extraction and IOL implantation can be attempted in selected cases with Marfan's syndrome.

5/16. Bilateral intraocular foreign bodies simulating crystalline lens.

    PURPOSE: To report a case of large bilateral intraocular foreign bodies mistaken for crystalline lens on computed tomography (CT). DESIGN: Case report. methods: A 24-year-old man was referred after bilateral open globe repair following a motor vehicle accident. Preoperatively, the CT scan had been read as "Right eye posteriorly dislocated lens. No evidence of foreign bodies." RESULTS: The patient underwent left eye cataract extraction with removal of a 7 x 5 x 5 mm piece of glass buried in the crystalline lens. The patient subsequently underwent right eye pars plana vitrectomy, removal of another piece of glass measuring 6 x 5 x 5 mm, retinal detachment surgery, and corneal grafting. CONCLUSIONS: Current safety standards require auto glass to fracture into pieces of a specific size to minimize laceration and missile injury. These pieces of glass may have a shape and size similar to the crystalline lens but have higher radiodensity on CT scan.

6/16. Management of the dislocated crystalline lens with a perfluorocarbon liquid.

    vitreoretinal surgery allows the effective removal of soft to moderately dense crystalline lens fragments, using the vitrectomy probe and the ultrasonic fragmentation probe. Hard lenses cannot be removed with these instruments. Previously described techniques for removal of hard lenses include trapping the lens anteriorly with needles, cryoextraction of the lens in an air-filled eye, and manipulation with sodium hyaluronate. These methods are difficult and dangerous in some cases. We used perfluorocarbon liquid to facilitate the safe removal of a surgically luxated hard crystalline lens. Injection of the perfluorocarbon liquid floated the lens off the retinal surface and into the anterior vitreous cavity. In this location, the lens was delivered by using standard extracapsular cataract extraction techniques. This technique allowed removal of the hard crystalline lens with minimal manipulation, and was helpful in the setting of poor media clarity.

7/16. Genetic spontaneous late subluxation of the lens. A study of two families.

    patients with genetic spontaneous late subluxation of the lens may present signs and symptoms other than lens displacement due to secondary anatomic alteration of the anterior segment. Two families are described, one of them with affected individuals in five successive generations. The vast majority of the patients sought consultation because of progressive myopia or intermittent acute intraocular hypertensive crisis which obscured the basic condition. Upon recognition of the disease, the affected patients underwent an uneventful intracapsular extraction. Histopathologic examination of the removed lenses suggested a dysplastic development of lens zonules.

9/16. Unplanned extracapsular cataract extraction in postvitrectomy eyes.

    A 70-year-old man, a 59-year-old woman, and a 57-year-old woman underwent pars plana vitrectomy. In all three cases, cataracts developed in the eyes that had been operated on. During cataract extraction, which included massage of the globe after retrobulbar anesthesia and a conventional ab externo incision, loss of lens material into the vitreous cavity occurred in all three postvitrectomy eyes.

10/16. Staphyloma: a complication of surgery for hyphema.

    Anterior staphylomas developed in two young patients as a postoperative complication of hyphema extractions through corneoscleral sections. The patients, aged five and nine years, underwent evacuation of the hyphemas after rebleeding caused medically unmanageable intraocular pressure elevations. Postoperatively, both patients received corticosteroids and had elevation of intraocular pressure. To repair the staphyloma, a scleral overlay patch graft was performed in one patient, but the intraocular pressure remained elevated and the staphyloma gradually recurred. Enucleation of the eye was eventually required. Resection of the staphylomatous area with lensectomy and complete vitrectomy was performed in the second patient. After four years of follow-up this patient continues to do well.
