1/5. Sentinel node biopsy for staging of aggressive digital papillary adenocarcinoma.

    BACKGROUND: Aggressive digital papillary adenocarcinoma is a rare malignancy with a propensity for metastases and recurrence. The role of lymph node staging in this tumor is poorly defined. We describe the use of sentinel lymph node mapping and biopsy in staging this tumor. OBJECTIVE: To describe and discuss the use of lymphatic mapping in staging aggressive digital papillary adenocarcinoma. methods: Sentinel lymph node mapping and biopsy was performed after excision of an aggressive digital papillary adenocarcinoma of the toe. RESULTS: Metastatic tumor cells were absent in sentinel lymph nodes by hematoxylin and eosin staining and immunocytochemistry analysis. CONCLUSION: We describe the first reported case of staging lymph nodes in a patient with aggressive digital papillary adenocarcinoma utilizing sentinel lymph node mapping and biopsy.

2/5. Acquired progressive lymphangioma.

    Acquired progressive lymphangioma is a very rare disorder that presents in childhood or early adolescence as a lymphatic proliferation that gradually enlarges. It most commonly involves an extremity and is typically characterized as an indurated plaque that may be brownish-red, violaceous, or yellow. We report the case of a 15-year-old boy with a 10-year history of acquired progressive lymphangioma on the sole.

3/5. Perilymphatic atrophy of skin. An adverse side effect of intralesional steroid injections.

    A less frequently reported adverse side effect of local steroid injections has been described. Perilymphatic atrophy results in atrophy of the dermal structures and depigmentation along the course of the lymphatic vessels draining the site of injection. The five cases reported here all occurred in dark-skinned patients. Three of the five cases (Cases Three to Five) resolved spontaneously, the longest taking 9 months from the time of injection. The outcome of two of the patients (Cases One and Two) remain unknown since the patients were lost to follow-up. This report did not form any conclusions from the material reported here, but simply sought to make practitioners aware of the occurrence of this adverse side effect.

4/5. Bullous morphea profunda.

    A deep sclerotic process developed on the shins of a 58-year-old man, and eosinophilic fasciitis or morphea profunda was suspected clinically. Bullae later arose on the plaques, and histologic examination of a skin biopsy specimen revealed sclerosis and inflammation of the deep dermis, panniculus, and fascia, with subepidermal edema causing formation of bullae. No lymphatic obstruction or vasculitis was seen. Two plaques of typical morphea on the penis were noticed 10 months later. The patient had no peripheral or tissue eosinophilia, hypergammaglobulinemia, hematologic abnormality, or history of exertion before the onset of the disease. The sclerotic process involved more than the fascia. In describing this deeper variant of morphea, the term "morphea profunda" appears to be more appropriate than "eosinophilic fasciitis."

5/5. Actinomadura pelletieri mycetoma from the Transvaal. A case report.

    The osteophilic red grain mycetoma due to Actinomadura pelletieri is rare in southern africa. This report describes an elderly patient with extensive bone involvement in which lymphatic spread to the inguinal lymph nodes had occurred. The diagnosis and management are discussed.
