1/9. Selective uppercase dysgraphia with loss of visual imagery of letter forms: a window on the organization of graphomotor patterns.

    We report a patient who, after a left parieto-occipital lesion, showed alexia and selective dysgraphia for uppercase letters. He showed preserved oral spelling, associated with handwriting impairment in all written production; spontaneous writing, writing to dictation, real words, pseudowords, and single letters were affected. The great majority of errors were well-formed letter substitutions: most of them were located on the first position of each word, which the patient always wrote in uppercase (as he used to do before his illness). The patient also showed a complete inability to access the visual representation of letters. As demonstrated by a stroke segmentation analysis, letter substitutions followed a rule of graphomotor similarity. We propose that the patient's impairment was at the stage where selection of the specific graphomotor pattern for each letter is made and that the apparent selective disruption of capital case was due to a greater stroke similarity among letters belonging to the same case. We conclude that a visual format is necessary neither for spelling nor for handwriting.

2/9. The overstimulated state of dyslexia: perception, knowledge, and learning.

    dyslexia is far more than a learning disorder; it has significant impact on personality organization. While dyslexia usually begins to manifest most clearly in early latency when the challenge of learning to read is at its height, often the dyslexic child's ego development and functioning has already been adversely affected. The literature from neuropsychology suggests that dyslexia is a subtle language-processing disorder that affects emotional, cognitive, and social development. The neuroanatomical literature also suggests a significant correlation between the neurodevelopmental basis for dyslexia, the caregiving environment, and psychological development.These two bodies of literature and analytic observations of a dyslexic patient suggest that the dyslexic individual may have a neurological deficit that increases vulnerability to overstimulation. The author hypothesizes that emotional and cognitive states result and reappear within the analytic encounter. This complicates clinical assessment and technical decisions. The author presents an analytic case and examines (1) the impact of deficit on the development of conflict; (2) the impact of the overwhelmed ego on the mastery of developmental tasks; and (3) the impact of dyslexia on dysgnosia, transference, and analytic process and technique.

3/9. The role of level of representation in the use of paired associate learning for rehabilitation of alexia.

    patients with phonological alexia (difficulty reading pseudowords) frequently have concomitant difficulty reading functor words and verbs compared with concrete nouns. The current study compares two techniques for helping two patients with phonological alexia regain the ability to read functors and verbs. One technique follows the approach of reorganization of function, while the other relies on the stimulation approach. Study 1, employing a reorganization approach, resulted in both patients increasing their reading accuracy from approximately 10 to 90% or greater. Study 2, using a stimulation approach, resulted in significant improvement, however neither patient was able to achieve accuracy greater than 59%. Study 3 reverted back to the reorganization approach using the same words from Study 2. Both patients demonstrated significant success, achieving 90% or greater accuracy. Whereas the reorganization approach meets with far greater success than the stimulation approach, both approaches can be seen as instances of paired associate learning. An explanation of the advantage of the reorganization approach is developed which focuses on the nature of the pairings in the paired associate learning paradigm: it is proposed that pairings within the same level of representation are easier to learn than pairings that cut across levels of representation.

4/9. The reach of neurology.

    Neurologists experienced in the interpretation of disease in terms of disordered action of the nervous system should be well suited to extend their field of interest to the more complex disorders of human behavior, including hysteria, delirium, ill-defined pain syndromes, unexplained fatigue, disorders of thought, atypical depression, and delusions. To illustrate the potential of neurology in approaching the more complex disorders of behavior, several examples from clinical neurology are presented in which phenomena calling for inquiry and analysis in neurological terms are described. The categories are temporal lobe epilepsy, delirium, drug toxicity, disease processes of the cerebrum, obscure pain, dyslexia, and hysteria. Inquiry into complex disorders of behavior is inseparable from the broad subject of normal mental activity, the neural organization subserving all human thought, emotion, and action. Because of this close association, the comment on hysteria includes an introduction to the important question of whether we humans possess a free will to choose our course of behavior.

5/9. Morphological units in the Arabic mental lexicon: evidence from an individual with deep dyslexia.

    An ongoing debate in Arabic morphology concerns the nature of the smallest unit governing lexical organization and representation in this language. A standard model maintains that Arabic words are typically analyzable into a three-consonantal root morpheme carrying the core meaning of words and a prosodic template responsible mostly for grammatical information. This view has been largely supported by research in both theoretical linguistics and psycholinguistics. An alternative theory holds that the meaning of words in Arabic is, rather, encoded in the 'etymon' comprising two unordered consonants of the root only. Results from a recent priming experiment have shown that the etymon induces strong morphological priming effects, supporting its morphological/lexical status. In this paper we present data from a patient with deep dyslexia questioning the role of the etymon as a psychologically real representational unit in Arabic and arguing, instead, for the central role of the root in both morphological and lexical representation in this language.

6/9. Gradual recovery from dyslexia and related serial magnetoencephalographic changes in the lexicosemantic centers after resection of a mesial temporal astrocytoma. Case report.

    letter-perception centers are not held in as high regard as motor- and language-related cortices during planning of neurosurgical procedures, and there have been no reports suggesting cortical reorganization of reading ability. The authors describe a patient with a left mesial temporal glioma in whom two letter-perception centers (the anterior portion of the left superior temporal gyrus and the left fusiform gyrus) were successfully localized before surgery by performing magnetoencephalography (MEG) during reading tasks. Control MEG examinations of 15 healthy volunteers were also performed to assist in a careful interpretation of patient results. Although a radical resection of the mesial temporal glioma, which involved the left fusiform gyrus, caused severe dyslexia, the patient's impaired reading skills improved gradually during a 1-year postoperative period. In the meantime, the spared left superior temporal gyrus displayed an overshot recovery of MEG responses. During the postoperative period there was no obvious recovery in MEG signals and no compensatory activity in the contralateral fusiform gyrus. This case demonstrates that lexicosemantic centers involved in the reading process can be noninvasively localized using MEG and that the results obtained are highly reliable for surgical planning. The results of the repeated MEG reflected sequentially the patient's recovery from dyslexia. This is the first report in which MEG studies have been shown to predict preoperatively the risk of dyslexia and demonstrate its serial physiological recovery.

7/9. Neuroanatomical and behavioral asymmetry in an adult compensated dyslexic.

    Individual differences in cortical anatomy are readily observable, but their functional significance for behaviors such as reading is not well understood. Here, we report a case of an apparent compensated dyslexic who had attained high achievement in visuospatial mathematics. Data from a detailed background interview, psychometric testing, divided visual field tasks measuring basic word recognition (word naming, nonword naming, and lexical decision), and more controlled word retrieval (verb, category, and rhyme generation), and measurements of his atypical brain structure are described. The findings suggested that enhanced "top-down" processing could provide the means to compensate for deficient "bottom-up" word decoding skills in this case. Relative to controls, this individual also evidenced unusually large asymmetries on several divided visual field lexical tasks, an extreme leftward asymmetry of the planum temporale, and a rare form of Sylvian fissure morphology (Steinmetz type 4, [Steinmetz, H., Ebeling, U., Huang, Y., & Kahn, T. We suggest that certain forms of brain organization may be associated with successful behavioral compensation for dyslexia, and that anatomical variations in the right hemisphere may be important contributors to individual differences in reading acquisition and achievement.

8/9. Developmental dyslexia and dysgraphia persistence in middle age.

    J.R. is a middle-aged man whose developmental dyslexia and dysgraphia have persisted in a problematic fashion for the 30 years since he left school. He is a phonological dyslexic. Whereas surface dyslexics operate on reading units that are too small, Mr. R operates on units that are too large. Spelling exhibits both regularity and lexicality effects. These seemingly contradictory results are consistent with a two-route model of spelling in which each route is functioning with approximately 50% efficiency. Performance on paralinguistic rhyming tasks is poor and shows no recovery in comparison to younger developmental phonological dyslexics. Difficulties with higher-order sound segmentation, analysis, or organization may underly the lifelong literacy problems.

9/9. Developmental dyslexia: four consecutive patients with cortical anomalies.

    We report the neuroanatomical findings in 4 consecutively studied brains of men with developmental dyslexia. The patients, who ranged in age between 14 and 32 years, were diagnosed as dyslexic during life. Nonrighthandedness and several autoimmune and atopic illnesses were present in the personal and family histories. All brains showed developmental anomalies of the cerebral cortex. These consisted of neuronal ectopias and architectonic dysplasias located mainly in perisylvian regions and affecting predominantly the left hemisphere. Furthermore, all brains showed a deviation from the standard pattern of cerebral asymmetry characterized by symmetry of the planum temporale. The neuroanatomical findings in these 4 patients are discussed with reference to developmental cortical anomalies, cerebral asymmetries, reorganization of the brain after early lesions, and the association between learning disorders, left handedness, and diseases of the immune system.
