1/10. Primary intraosseous carcinoma associated with impacted third molar of the mandible: review of the literature and report of a new case.

    This study investigated the clincopathologic features of primary intraosseous carcinoma (PIOC) related to the third molar of the mandible. Six cases (5 from the literature and 1 new case; mean patient age 73 years) of PIOC associated with completely impacted lower third molar were analyzed. For comparison, 8 cases of PIOC presenting after extraction of the lower third molar were also evaluated, with a significantly lower mean patient age of 56 years. Most lesions were deceptively similar to those related to the lateral type of dentigerous cyst, and half of the impacted cases revealed an asymptomatic fracture of the mandible. In all patients except for the present case, metastasis had not been recorded. Surprisingly, no patients are known to have died of PIOC. In our experience the prognosis of PIOC associated with a completely impacted third molar is much better than commonly believed, although the number of cases involved is too small to make a general conclusion. However, delay in establishing the correct diagnosis followed by extraction without awareness of the carcinoma is know to adversely affect the prognosis.

2/10. Oro-dental manifestations of the Schwartz-Jampel syndrome.

    A boy with the Schwartz-Jampel syndrome (chondrodystrophic myotonia) had a number of oro-dental complications. These included difficulty in tooth extraction and orthodontic care due to a small oral aperture and rigidity of the temporo-mandibular joints. General anaesthesia was hazardous because of a propensity to malignant hyperthermia, and endotracheal intubation was difficult because of shortness and rigidity of the neck and the small size of the laryngeal structures. awareness of these potential problems is crucial for anaesthesia and comprehensive dental management. The radiological demonstration of dentigerous cysts is a hitherto unreported observation in this disorder.

3/10. Orthodontic extraction: conservative treatment of impacted mandibular third molar associated with a dentigerous cyst. A case report.

    A 53-year-old man exhibited a large dentigerous cyst associated with a deep vertical impaction of his left mandibular third molar where a close anatomic relationship existed between the root and the mandibular canal. After consultation, it was decided to perform marsupialization of the cyst, to extrude the third molar orthodontically, and then to extract it. This combined orthodontic-surgical approach was successful.

4/10. Endodontic management of dens evaginatus of maxillary central incisors: a rare case report.

    Dens evaginatus is a disturbance in tooth development that produces a tubercle of hard tissue on the surface of the tooth. While prophylactic pulp capping is recommended for vital teeth, teeth with necrotic pulps require endodontic therapy or extraction. This rare case of a 24-yr-old Indian male with affected maxillary central incisors demonstrates the clinical consequences of dens evaginatus.

5/10. Inflammatory dentigerous cysts of children treated by tooth extraction and decompression--report of four cases.

    Inflammatory dentigerous cysts are only found in the mixed dentition. The four cases presented here illustrate the uncomplicated behavior of these cysts when properly treated. By extracting the infected primary teeth, opening the cyst and ensuring continuous drainage, it is possible to achieve spontaneous eruption of the involved permanent teeth into the dental arch even if they are badly dislocated. Simultaneous with the eruption of the permanent teeth, ossification of the bony defect can take place. The reparatory process is completed in one to two years.

6/10. Dentigerous cysts in primary dentition: report of 2 cases.

    Dentigerous cysts are benign odontogenic cysts associated with the crowns of permanent teeth. They are usually single in occurrence and located in the mandible. The purpose of this case report was to describe the management of 2 dentigerous cysts in children. The treatments instituted were the extraction of the deciduous tooth involved followed by marsupialization in the first case and enucleation in the second one. Both treatments allowed rapid healing of the lesion and eruption of the permanent teeth without the need for orthodontic treatment.

    The ameloblastoma is an unusual neoplasm in the pediatric population. A maxillary ameloblastoma was discovered during evaluation for premolar extractions. Treatment and the importance of obtaining a histological evaluation are discussed.

8/10. Impacted teeth: prophylactic extractions or not?

    Pathologic changes related to impacted teeth are infrequent. However, considerable damage to bone and adjacent teeth may result if they do occur. Four patients are described in whom different circumstances existed with impacted teeth. Extractions are indicated when the anatomic location of the impacted teeth is most favorable and when the patient's systemic and hematologic condition is least compromising, even if the teeth are asymptomatic at that time.

9/10. dentigerous cyst with inflammatory etiology from a deciduous predecessor--report of a case.

    Dentigerous cysts arising from overlying infected predecessor teeth are very rare. This report describes one such case where extraction of the infected deciduous tooth with marsupialization led to the eruption of the premolar. Periodic radiographic evaluation is required to note and prevent recurrence and other serious complications arising from these cysts.

10/10. Adenomatoid odontogenic tumour in a 12-year-old boy.

    We present a rare finding of adenomatoid odontogenic tumour (AOT) in a 12-year-old boy who was referred to the Department of Paediatric dentistry at the Eastman Dental Hospital for the extraction of a carious maxillary permanent molar. First presentation revealed that the maxillary right permanent lateral incisor and canine were unerupted. Radiological examination revealed a circumscribed radiolucent area associated with the distal aspect of the maxillary right permanent lateral incisor. The patient was admitted to our day care Unit where the carious maxillary molar was extracted and the radiolucent area associated with the maxillary right permanent incisor was explored. Pathological examination confirmed the lesion as adenomatoid odontogenic tumour. We present a brief review of the literature and consider how differential diagnosis of this tumour from more common odontogenic lesions can be established.
