1/10. Obturator internus muscle abscess in children: report of seven cases and review.

    Obturator internus muscle (OIM) abscess is an uncommon entity often mistaken for septic arthritis of the hip. We describe seven children with OIM abscess and review seven previously reported cases. The most common presenting symptoms were hip or thigh pain (14 patients), fever (13), and limp (13). The hip was flexed, abducted, and externally rotated in 11 patients. magnetic resonance imaging and computed tomography (CT) were diagnostic for OIM abscess in the 14 patients. Associated abscesses were located in the obturator externus muscle (5 patients), psoas muscle (2), and iliac muscle (1). The etiologic agents were staphylococcus aureus (8 patients), streptococcus pyogenes (2), neisseria gonorrhoeae (2), and enterococcus faecalis (1). Three patients underwent CT-guided percutaneous drainage, and three had surgical drainage. Three patients had ischial osteomyelitis in addition to OIM abscess. The 11 children with uncomplicated OIM abscess were treated for a median of 28 days. All patients had an uneventful recovery.

2/10. Postmenopausal tubo-ovarian abscess due to pseudomonas aeruginosa in a renal transplant patient: a case report and review of the literature.

    BACKGROUND: pseudomonas aeruginosa is an uncommon cause of infection in the female genital tract. We report a case of postmenopausal tubo-ovarian abscess (TOA) due to P. aeruginosa in a renal transplant recipient. The presentation included mild abdominal symptoms with rapid progression of peritonitis and surgical abscess drainage. This is the first such case in an organ transplant recipient described in the English literature. methods AND RESULTS: Published reports of 1040 cases of TOA were reviewed. The most common features were a history of sexually transmitted disease or pelvic inflammatory disease, and symptoms including abdominal pain and fever. escherichia coli, bacteroides spp., and klebsiella pneumoniae were the most frequently encountered pathogens. neisseria gonorrhoeae and chlamydia trachomatis, which are frequently isolated from cervical cultures, are uncommonly isolated from tubo-ovarian abscesses. Forty percent of patients were treated with antibiotics alone, 18.8% with abdominal surgery, and 32% with surgery and antimicrobial therapy. CONCLUSION: This report illustrates the muted presentation and atypical microbiology of gynecologic infection in an organ transplant recipient.

3/10. Perianal abscess due to neisseria gonorrhoeae: an unusual case in the post-antibiotic era.

    Described here is the case of a 21-year-old homosexual male patient who presented with perianal abscess without urethritis that was caused by infection with neisseria gonorrhoeae. Incision and drainage of the abscess and oral penicillin therapy resulted in full healing, without the development of an anal fistula. The spectrum of gonococcal abscesses and the relevant aspects of their management are discussed.

4/10. neisseria gonorrhoeae foot abscess. A case report.

    A cutaneous abscess involving neisseria gonorrhoeae is extremely rare outside the perineum. Such remote lesions are a result of disseminated gonococcal disease. A 12-year-old girl with arthrogryposis multiplex congenita developed a gonococcal abscess on the plantar aspect of her lateral forefoot after a two-week prodrome of pain, erythema, and swelling. The infection was successfully treated with open drainage, acetic acid dressing changes, and the administration of penicillin.

5/10. Oral cavity abscess due to neisseria gonorrhoeae.

    A patient with an abscess in the oral cavity caused by neisseria gonorrhoeae is presented. The suspected gonococcal cause was confirmed by the presence of gonococci in smears of material evacuated from the abscess and the growth of bacteria in a modified Thayer-Martin culture medium. This represents, to our knowledge, the first reported case of oral cavity abscess due to N. gonorrhoeae.

6/10. Preputial abscesses caused by beta-lactamase-producing gonococci.

    Preputial abscesses occurred in three men who had had sexual intercourse with prostitutes in thailand. neisseria gonorrhoeae could be grown from the lesions. Two of the strains were tested for beta-lactamase production and proved positive. While all three patients showed no clinical signs of urethritis, urethral swabs were positive for gonococci in two.

7/10. Ruptured tubo-ovarian abscess in pregnancy: recovery of a penicillinase-producing strain of neisseria gonorrhoeae.

    This report describes a patient, 15 weeks pregnant, who developed progressive disease of the female genital tract due to a penicillinase-producing strain of neisseria gonorrhoeae. Standard therapy for gonorrhea did not abort progression of disease, which culminated in a ruptured tubo-ovarian complex in the second trimester of pregnancy.

8/10. Cutaneous gonococcal abscess: a case report.

    A 29-year-old man was diagnosed as having a gonococcal abscess of the prepuce and urethritis. Such abscesses, caused by fetal monitoring during labor of a mother infected with neisseria gonorrhoeae, have been observed on the scalp of newborns. If antibiotic treatment is to be effective, these abscesses must be excised and drained.

9/10. Gonococcal septicaemia presenting as a subcutaneous abscess.

    In a case of disseminated gonococcal infection the diagnosis was delayed until neisseria gonorrhoeae was grown from pus from a discharging abscess of the biceps muscle. This unusual skin manifestation appears not to have been reported before.

10/10. Gonococcal scalp abscess in a newborn.

    We have described a 6-day-old male infant who presented with neisseria gonorrhoeae conjunctivitis and a scalp abscess. Delivery was complicated by rupture of the membranes 24 hours before delivery and fetal monitoring for four hours. The rare scalp abscesses caused by gonococci cleared with penicillin therapy.
