1/51. One-appointment composite crown: an alternative to conventional crown and bridge.

    When presented with little coronal tooth structure remaining, four options exist: (1) extraction only, (2) extraction and restoration, (3) root canal therapy and a cast restoration (which may involve crown lengthening and/or orthodontic extrusion), or (4) root canal therapy and a composite/amalgam crown. A technique for the composite crown option is described with case presentations.

2/51. Reattachment of a subgingivally fractured central incisor tooth fragment: report of a case.

    A case report of a 9-year-old boy with a fractured maxillary right incisor and ulcerated pulp at the fracture line is presented. On satisfactory completion of a root canal filling one week later, the access cavity was restored with glass ionomer cement. The fragment was reattached by a light activated hybrid composite during the flap surgery. tetracycline hydrochloride was applied on open root surface for a better healing.

4/51. Management of a horizontal root fracture after previous root canal therapy.

    This case report concerns a 9-year-old girl who had a horizontal root fracture of her maxillary left central incisor and had root canal therapy performed by her family dentist immediately after the injury. Because of the incomplete canal obturation, the root canal filling material was removed, and calcium hydroxide therapy was initiated. Healing at the fracture site occurred, with hard tissue forming between the root segments. The coronal segment of the root was then obturated with gutta-percha. The patient later had orthodontic treatment with some movement of the coronal segment. Six-year post-treatment follow-up shows apparent clinical and radiographic success. This case illustrates the potential for repair of a horizontal root fracture after endodontic retreatment of the coronal segment and long-term splinting.

5/51. Surgical extrusion of a cervically root-fractured tooth after apexification treatment.

    A case is reported in which an incisor fractured below the alveolar crest 6 months after completion of apexification treatment was surgically extruded for prosthetic coronal restoration. After the surgical procedure, a dowel post was placed in the root canal, a core was built using glass-ionomer cement, and a porcelain veneer crown restoration was completed. The 24-month follow-up examination after surgical, endodontic, and prosthetic treatments showed that the tooth was clinically and radiographically healthy and functioned well.

6/51. Enhanced post crown retention in resin composite-reinforced, compromised, root-filled teeth: a case report.

    The introduction of an intraradicular composite reinforcing technique, in conjunction with the reestablishment of matching post canal spaces, has allowed compromised, root-filled teeth to be restored with functional, esthetic post crowns. This clinical case report suggests that reconstituted post canals, in accurately adapting to passive, parallel-sided, matching, and well-fitting posts, can enhance the retention of post crowns. Other factors of clinical importance relating to the resin-reinforced technique are discussed, including fracture resistance, depth of polymerization, dentin adhesion, polymerization shrinkage, and coronal microleakage.

7/51. Bone-like tissue growth in the root canal of immature permanent teeth after traumatic injuries.

    Following a severe traumatic incident to permanent immature teeth, the growth of calcified tissue in the pulp space may occasionally occur. This calcified tissue may be diffuse or in intimate contact with the dentine. It has been suggested that a wide open apex, severe damage to the root sheath, and the absence of infection are only some of the predisposing factors leading to this metaplasia of pulp tissue into bone-like tissue. Five cases are described.

8/51. Investigations on a cell culture medium for storage and transportation of avulsed teeth.

    Non-physiologic storage of avulsed teeth leads to a high incidence of root resorption, resulting in poor prognosis. This study investigated the suitability of specially composed cell culture media for storage of extracted teeth for up to 48 hours. Autoradiographic investigations revealed that the proliferative activity of periodontal ligament (PDL) cells of teeth stored in cell culture medium for up to 48 hours increased with storage time. Studies on proliferation of PDL cells after storage of teeth in different media for up to 24 hours demonstrated that the proliferative activity is dependent on the composition of the medium. Immunohistochemical investigations with markers for cell proliferation revealed that pulp cells of extracted immature teeth show numerous proliferations after storage for up to 24 hours in a special cell culture medium but few proliferations after storage in Hanks Balanced Salt Solution (HBSS). The investigations indicate that a special cell culture medium can preserve cell viability of PDL cells adhering to extracted teeth for at least 48 hours. The in vitro results are confirmed by a case presented: After storage of two upper central incisors for 36 hours in the cell culture medium the teeth could be successfully reimplanted after extraoral insertion of titanium posts into the root canal (auto-alloplastic reimplantation). Clinical and radiological follow-up examinations for 12 months revealed normal periodontal healing.

9/51. Revascularization of an immature permanent tooth with apical periodontitis and sinus tract.

    A necrotic immature mandibular second premolar with periapical involvement in a 13-year-old patient was treated. Instead of the standard root canal treatment protocol and apexification, antimicrobial agents were used in the canal, after which the canal was left empty. Radiographic examination showed the start of apical closure 5 months after the completion of the antimicrobial protocol. Thickening of the canal wall and complete apical closure was confirmed 30 months after the treatment, indicating the revascularization potential of a young permanent tooth pulp into a bacteria-free root canal space.

10/51. Endodontic and periodontal treatments of a geminated mandibular first premolar.

    AIM: To describe a rare case of gemination involving a mandibular first premolar. SUMMARY: The complex morphology of geminated teeth renders their endodontic and periodontal management difficult. Root canal and periodontal treatments were performed on a geminated mandibular first premolar with three canals. Clinical examination showed two separated crowns with united roots. Radiographically, two distinct pulp chambers with two joined and a third independent canal were seen. Conventional root canal treatment resulted in complete healing of the apical lesion. However, the occurrence of a vertical fracture led to the extraction of the mesial segment. At the follow-up visit, the distal segment was clinically healthy and continued to satisfy functional demands.
