11/3572. Atrophie blanche lesions closely resembling malignant atrophic papulosis (Degos' disease) in systemic lupus erythematosus.

    Two patients with systemic lupus erythematosus are described who in the course of their disease developed small atrophie blanche lesions that closely resembled those found in malignant atrophic papulosis. Preliminary investigation of these two cases indicates that considerable similarities probably exist in the pathogenesis of malignant atrophic papulosis and the atropie blanche lesions of systemic lupus erythematosus. It is concluded that a diagnosis of malignant atrophic papulosis should only be made after systemic lupus erythematosus has been excluded by full investigation. ( info)

12/3572. Treatment of reticular erythematous mucinosis with a large dose of ultraviolet B radiation and steroid impregnated tape.

    To date, there are no successful treatments for reticular erythematous mucinosis (REM) other than antimalarial drugs such as chloroquine; these have many adverse side effects and are prohibited for use in japan. We report a case of REM improved by a large dose of ultraviolet B (UVB) radiation and a steroid impregnated tape. The large dose of UVB radiation improved the erythema after 5 weeks. Application of steroid impregnated tape to a lesion where a large dose of UVB had been given produced an additive clinical effect. UVB radiation and steroid impregnated tape are considered to be effective treatments for REM. ( info)

13/3572. A case of edematous striae distensae in lupus nephritis.

    A 17-year-old girl with systemic lupus erythematosus presented with painful edematous abdominal striae. She had been treated with systemic steroid for the systemic lupus erythematosus. At the time of presentation, she had abruptly gained 10 kg due to combined lupus nephritis. The histopathologic finding of the edematous striae distensae included dermal edema with separation of collagen fibers and small fragmented elastic fibers. Edematous striae distensae are uncommon but can develop from the combined effects of glucocorticoid and generalized edema. ( info)

14/3572. dermatofibrosarcoma protuberans: an early non-protuberant phase of the tumour.

    A 40-year-old female patient presented with a flat scar like plaque on the upper chest. The patient's perception of subtle change in the lesion was of paramount importance in the decision to biopsy this lesion of innocuous clinical appearance. Histopathological findings were those of dermatofibrosarcoma protuberans. This case illustrates the early clinical features of the tumour and a recommendation for taking a biopsy of any scar-like plaque where there is no clear history of preceding trauma. ( info)

15/3572. A case report including EM and dna repair investigations in a dermatosis associated with multiple skin cancers: epidermodysplasia verruciformis.

    This report describes the clinical, histological and electron microscopic observations in a 51-year-old male with epidermodysplasia veruciformis (EV). cells with early signs of malignant transformation were found closely connected with virus infected epidermal regions. skin cancers appeared initially on sun-exposed areas, such as the face and ear lobes. The UV-induced dna repair synthesis was therefore studied, utilizing peripheral leukocytes. The patient had 40% lower UV-induced dna repair synthesis than the mean of nine healthy subjects of the same age. These results suggest that a decrease in UV-induced dna repair synthesis in combination with a possibly oncogenic viral infection may enhance the disposition for somatic mutations and malignant transformation in patients with EV. 16/3572. Behavioral and dermatologic changes and low serum zinc and copper concentrations in two premature infants after parenteral alimentation.

    Two premature infants were observed to develop behavioral and dermatologic changes and low serum zinc and copper concentrations following cessation of prolonged parenteral alimentation, while being fed exclusively with human milk. Following treatment with exogenous oral zinc supplementation, prompt relief of symptoms and increases of serum zinc and copper concentrations were observed in both infants. These patients comprise about 5% of our premature infants who are treated with parenteral alimentation for more than two weeks. We recommend that premature infants on prolonged parenteral alimentation should be monitored for changes in serum zinc and copper concentrations and, if a marked fall is observed, supplementation should be considered. 17/3572. The "vanilla fudge" cyst.

    A white cyst, partially covered by jet black splotches and streaks, had a surface appearance of vanilla fudge ice cream. Histologically, the cyst was lined with two distinctly different types of follicular epithelium--one identical to that of the infundibulum and the other to that of the matrix. The black pigment was melanin, produced by numerous dendritic melanocytes that were situated only in the region of the epithelium-resembling matrix. ( info)

18/3572. Cutaneous manifestations of hypothyroidism including purpura and ecchymoses.

    hypothyroidism has distinctive cutaneous manifestations which may be diagnostic. biopsy and histologic study of the skin may provide a simple and useful tool for diagnosing hypothyroidism. purpura and ecchymoses, although not often mentioned among cutaneous manifestations of hypothyroidism, occurred in nine of 222 hypothyroid patients at Ochsner Clinic between 1958 and 1972. 19/3572. Lichen spinulosus: response to therapy.

    Two cases of lichen spinulosus and the response to therapy with Keralyt under occlusion are presented. It is evident that Keralyt under occlusion for two consecutive weeks will clear these lesions. However, twice weekly application of Keralyt must be used in order to keep these areas free of lesions. An undesirable side effect in the two patients presented was a burning, stinging sensation immediately following application of Keralyt. ( info)

20/3572. Axillary and inguinal striae induced by systemic absorption of a topical corticosteroid.

    A 21-year-old woman developed huge atrophic striae in the axillary and inguinal areas from the topical application, with occlusion, of potent corticosteroid preparation applied to the axillary or inguinal areas. It is postulated that the mechanism is systemic absorption of enough medication to produce a state of hyperadrenalism with subsequent striae formation as a sequela. ( info)
