1/4. Autonomic dysfunction associated with multiple pelvic ganglioneuromas.

    We describe the case of a 59-year-old male in which several symptoms of autonomic dysfunction resulted associated with the presence of multiple ganglioneuromas in the adrenal glands and in paravertebral spaces. In a 2-year period, the patient developed sexual dysfunctions, micturitional disturbances and severe orthostatic hypotension and was erroneously diagnosed as having primary autonomic failure. However, the examination of all the autonomic functions showed that papillary function and cardiovascular reflexes were normal. CT scan of the abdomen revealed the presence of several small masses in the adrenal glands and along the lumbar sympathetic chain. Cytologic examination of the adrenal glands showed clusters of ganglionlike cells interspersed with schwann cells, leading to the diagnosis of ganglioneuromas. patients with signs and symptoms of autonomic dysfunction need an extensive clinical and laboratory examination of all the autonomic functions in order to exclude secondary causes of the symptomatology.

2/4. pain during sexual response due to bilateral Bartholin's gland adenomas. A case report.

    Benign adenomas of the major vestibular gland are very rare. Presented is a case of bilateral vestibular gland adenomas in a woman whose chief complaint was vulvar pain occurring with sexual arousal and orgasm. Surgical removal of the adenomas relieved the symptoms. The development of pain during sexual response in this case appears to be mediated by vascular engorgement.

3/4. Behavioral analysis of sexual dysfunction in Hansen's disease.

    Human sexual behavior is an interactive process including CNS, hormonal, and sex-gland activities. This process can be disrupted in males who have Hansen's disease if testicular atrophy occurs. Elevations of centrally mediated luteinizing hormone and deficient testosterone levels were found in a male with Hansen's disease whose insufficient erections were secondary to atrophic testes. Quasi-experimental (A-B) analysis provided evidence for the efficacy of testosterone treatment for such a condition.

4/4. Prostatic gland damage: a possible complication of bladder neck Y-V-plasty.

    A case of primary ejaculatory failure due to prostatic damage in a 23-year-old patient is presented. This occurred secondary to Y-V operation of the bladder neck which was performed 19 years earlier. To our knowledge, this complication has not been described previously.
