2/10. Acute poststreptococcal glomerulonephritis and acute rheumatic fever: occurrence in the same patient.

    Group A streptococcus can cause both acute glomerulonephritis and acute rheumatic fever. The occurrence of characteristic acute poststreptococcal glomerulonephritis and acute rheumatic fever in the same patient is rare. We describe a 10-year-old girl with acute rheumatic fever who presented with the typical clinical and biopsy findings of acute poststreptococcal glomerulonephritis.

4/10. Simultaneous occurrence of acute poststreptococcal glomerulonephritis and acute rheumatic fever.

    The simultaneous occurrence of acute poststreptococcal glomerulonephritis and acute rheumatic fever is a rarity. We report here such an occurrence in a 9-year-old Saudi boy whose kidney biopsy showed diffuse exudative glomerulonephritis with subepithelial humps on electron microscopy in the presence of active rheumatic carditis.

5/10. Poststreptococcal neprhitis and acute rheumatic fever in two adults.

    In two adult subjects who suffered acute episodes of rheumatic fever, sever kidney involvement developed with proteinuria and renal insufficiency. The clinical and histologic picture was typical of poststreptococcal (infectious) glomerulonephritis. During a follow-up period of more than three years, no clinical or biochemical abnormalities have been detected; thus, it is likely that both patients have recovered from their renal diseases. The coexistence of rheumatic fever and acute glomerulonephritis, which is uncommon even in your age groups, may be observed in adult subjects.

    We report the coexistence of acute rheumatic fever and acute glomerulonephritis in a 15-year-old boy, which may have resulted from nonsuppurative complications of a streptococcal infection.

7/10. Acute rheumatic fever with extracapillary glomerulonephritis and the nephrotic syndrome.

    A 34 year old woman is described in whom carditis, arthritis, fever, leukocytosis and a high sedimentation rate developed two weeks after a streptococcal infection. The patient also had the nephrotic syndrome and rapidly progressive renal insufficiency. The renal biopsy specimen showed acute extracapillary (crescentic) glomerulonephritis. The initial response to corticosteroid therapy was good, but later a slowly progressive renal function impairment was seen.

8/10. Renal lesions in acute rheumatic fever.

    In the past 4 years, four patients with acute rheumatic fever and abnormalities of renal function, urinary sediment, or both have undergone percutaneous renal biopsy at this institution. Distinctly different renal lesions were found and included focal glomerulonephritis, classic acute exudative poststreptococcal glomerulonephritis, mesangioproliferative glomerulonephritis, and severe interstitial nephritis. In each case, the clinical abnormalities were transient. In this report we describe the clinical and pathologic findings in each of these patients to stress the variability of renal lesions that may accompany acute rheumatic fever.

9/10. Rapidly progressive glomerulonephritis complicating acute rheumatic fever.

    A case of acute rheumatic fever and glomerulonephritis following streptococcal throat infection is presented. The coincidence of rheumatic fever and post-streptococcal glomerulonephritis is uncommon, but well recognised. This case is of additional interest since the nephritis was crescentic.

10/10. Acute rheumatic fever and poststreptococcal acute glomerulonephritis caused by T serotype 12 streptococcus.

    We present a rare case of a 10 year old Japanese boy with acute rheumatic fever accompanied with poststreptococcal acute glomerulonephritis. We isolated group A streptococcus serotype T 12, a strain that was thought to be nephritogenic but not rheumatogenic, from throat culture. Although rare, physicians should be aware that acute renal disease may accompany rheumatic fever.
