1/8. Vena cava thrombosis associated with nephrotic syndrome in the puerperal gestational cycle.

    CONTEXT: The puerperal gestational cycle is accompanied by a state of physiological hypercoagulability. Thromboembolic phenomena may occur at this time. OBJECTIVE: To report on a clinic case involving a patient that presented a family history of thromboembolism and developed deep vein thrombosis in a lower limb and vena cava thrombosis during the puerperal gestational cycle, displaying nephrotic syndrome as the main complication. DESIGN: Case report.

2/8. Postpartum "psychosis" in mild argininosuccinate synthetase deficiency.

    BACKGROUND: urea cycle disorders are relatively rare but well-established causes of postpartum coma and death. Such clinical presentations have been reported previously in ornithine transcarbamylase and carbamyl phosphate synthetase deficiencies. CASE: We describe a woman, without prior symptoms of metabolic disease, who presented with hyperammonemia and psychiatric symptoms in the postpartum period. Initial diagnoses included acute fatty liver of pregnancy and postpartum psychosis. She was later found to have argininosuccinate synthetase deficiency after further metabolic investigations. Rare heterozygous mutations in the argininosuccinate synthetase gene were identified. CONCLUSION: urea cycle disorders may present initially with postpartum psychiatric symptoms and may represent an underrecognized cause of "postpartum psychosis." We recommend obtaining metabolic studies in women with neurologic or severe psychiatric symptoms in the postpartum period.

3/8. Postpartum hypopituitarism with preservation of the pituitary-ovarian axis.

    An 18-year-old woman developed postpartum hypopituitarism, presenting with personality and behavioral changes caused by severe hypoglycemia. In spite of well-documented deficiencies of thyroxine, cortisol, and growth hormone, she maintained normal ovarian function (reflected by normal menstrual cycles, normal estrogenization on examination, and normal FSH and estradiol levels). Classically, gonadal failure occurs very early in the course of pituitary failure, prior to loss of thyroid or adrenal function; preserved ovarian function is commonly used as clinical evidence of intact pituitary function. This patient emphasizes the importance of pursuing the possibility of pituitary disease in appropriate clinical settings, even in patients with intact ovarian function.

4/8. Psychosis and the menstrual cycle.

    A case of a puerperal psychosis in a 26-year-old woman who had a strong family history of schizophrenia is reported. Her symptoms resolved with chlorpromazine and electroconvulsive therapy, but recurred each month just before the onset of menses. The cyclical recurrence of symptoms was prevented by therapy with danazol, a synthetic steroid which inhibits ovulation and may influence several levels of the reproductive control mechanism from the hypothalamus to the uterus. This therapy may be helpful for other women who suffer from recurrence of severe psychiatric disorders in close association with the menstrual cycle.

5/8. Psychiatric complications of progesterone and oral contraceptives.

    progesterone plays a critical role in the menstrual cycle, pregnancy, and sexuality. Its role in premenstrual syndrome, dysmenorrhea, and postpartum disorders is outlined. Oral contraceptives seem to affect mood and behavior in some women without preexisting psychiatric illness, sometimes inducing depression and loss of libido. When used as psychotropic agents, they can have mood-stabilizing effects and relieve premenstrual syndrome.

6/8. Autoimmune progesterone dermatitis.

    Autoimmune progesterone dermatitis is a rare cutaneous disorder characterized by recurrent cyclic eruptions during the luteal phase of the menstrual cycle. We describe a woman with postpartum onset of a papulovesicular eruption. The possibility of autoimmune progesterone dermatitis was raised because of the cyclic nature of the outbreak. A reactive intradermal progesterone test confirmed the diagnosis. A review of individual case reports is also presented.

7/8. Traumatic macroglossia.

    A case of severe macroglossia resulting from trauma (tongue biting) during eclampsia and causing respiratory obstruction is described. Despite medical treatment with steroids and antibiotics for a week, followed by tracheostomy, no significant improvement was observed. After an energetic but cautious maneuver of reducing and restraining the tongue in the oral cavity, the swelling reduced dramatically in 24 to 48 hours. Earlier manual replacement of the tongue into the oral cavity is advised in order to arrest the cycle of venous and lymphatic obstruction and congestion that leads to further edema and increased tongue swelling. The mechanism of traumatic macroglossia is discussed.

8/8. Puerperal and menstrual psychoses: the proposal of a unitary etiological hypothesis.

    Puerperal and menstrual psychoses are both uncommon disorders and the occurrence of both in individual patients suggests the possibility of a common underlying pathogenesis. In this paper two cases are reported, the literature is reviewed and a unifying etiological hypothesis is postulated in which precipitous reductions in the brain estrogen environment precipitate episodes of psychosis in predisposed individuals. In the case of puerperal psychosis, estrogen cascade follows a lengthy period of sustained high brain estrogen environment; in menstrual psychosis, it is postulated that the occurrence in at least some cases of anovulatory menstrual cycles, wherein high levels of relatively unopposed estrogens are maintained until the next ovulatory cycle, play a role in priming the central nervous system prior to premenstrual estrogen cascade. Further research in this area using more sensitive techniques to follow hormonal fluctuation and mental state is called for.
