1/4. Congenital candida pneumonia and sepsis: a case report and review of the literature.

    A term newborn with candida albicans infection of the lungs and blood is described. Although no maternal risk factors were identified, this patient's rapid clinical deterioration and postmortem findings suggest congenital infection. Related cases in the literature are reviewed. This case suggests that a diagnosis of fungal pneumonia should be considered in any infant presenting with severe respiratory distress.

2/4. An immunohistological evaluation of pseudomonas aeruginosa pulmonary infection in two patients with cystic fibrosis.

    pseudomonas aeruginosa is the principal pulmonary pathogen in patients with cystic fibrosis. All attempts to date to prevent or eradicate P. aeruginosa infections in these patients have been unsuccessful. vaccination against P. aeruginosa has been proposed as a preventive strategy but it has not been adequately evaluated. The purpose of this study was to determine whether P. aeruginosa, present in the lungs of patients with cystic fibrosis, express surface antigens similar to those grown in vitro; this issue is of critical importance when choosing bacterial products as vaccine candidates. lung sections from two patients who died of the pulmonary complications of cystic fibrosis were studied. bacteria, both in lung sections and isolated from the lung sections and grown in vitro, reacted strongly with polyclonal and monoclonal antibodies against P. aeruginosa mucoid exopolysaccharide and outer membrane proteins F and H2; this suggested that these antigens are surface exposed in vivo. It was also found that bacteria in both lung sections were associated in situ with IgG, IgA, and C3 but not with IgM or C4.

3/4. The Fogarty balloon catheter as an aid to management of the infant with esophageal atresia and tracheoesophageal fistula complicated by severe RDS or pneumonia.

    Early ventilatory failure in an infant with EA/TEF may prove difficult to manage because of the low resistance "vent" provided by the TEF. In this Case Report Fogarty balloon occlusion of the vent was possible and aided management of the patient who was not a candidate for primary repair of the anomaly.

4/4. brain death during pregnancy: tocolytic therapy and aggressive maternal support on behalf of the fetus.

    We report a case of maternal brain death at 25 weeks gestation in which aggressive maternal hemodynamic, respiratory, and metabolic support and tocolytic drug therapy resulted in prolongation of pregnancy for 25 days. The indication for delivery was torulopsis giabrata amnionitis, which may have occurred due to transmembrane or transplacental route. The baby was treated for fungal sepsis, and did well. Premature labor may occur spontaneously after maternal brain death, and may be precipitated by infection or by maternal drug therapy. The myriad of hemodynamic and endocrine issues associated with maternal brain death complicate the choice of tocolytic drugs, but this case illustrates that uterine activity can be successfully blocked, potentially diminishing risks to the newborn, following the tragedy of maternal brain death during pregnancy.
