1/4. Limb disuse in a newborn.

    osteomyelitis in the neonate is an entity distinct from that in older children. We report a case that had a benign onset, insidious course, and multiple foci of involvement. The unique metaphyseal blood supply of early infancy permitted coexisting septic arthritis. Signs of limb disuse, limitation of motion, and swelling over distal bone or joint space should prompt the emergency physician to entertain the possibility of osteomyelitis. early diagnosis and intervention on the part of the emergency physician may prevent or alleviate sequelae of this disease.

2/4. Familial nesidioblastosis: severe neonatal hypoglycemia in two families.

    Severe neonatal hypoglycemia with pathologic findings of diffuse nesidoblastosis of the pancreas is described in five children of both sexes from two families with unaffected parents. This appears to represent an autosomal recessive disorder of pancreatic development. Despite extensive testing, the diagnosis of hyperinsulinism was difficult in the index case of each family and delayed definitive treatment. Medical therapy with steroids and diazoxide was unsuccessful; pancreatectomy was required to treat persistent hypoglycemia. An abnormality of circulating glucagon found in one child with this disorder suggested that hyperinsulinism may not be the sole hormonal imbalance present, but rather that this disease is one of generalized disturbance of islet cell function. The history of severe, persistent neonatal hypoglycemia in an older sibling should lead the physician to investigate subsequent children for the presence of asymptomatic hypoglycemia.

3/4. campylobacter infections in pregnancy. Case report and literature review.

    campylobacter fetus is a major cause of venereally transmitted abortion and sterility in ungulates, and a recently recognized, common enteric pathogen in man. The role of the organism as a cause of human fetal wastage has not been delineated. We present a case of bacteremic campylobacter infection in a pregnant woman, and review the published cases of campylobacteriosis in pregnancy and the perinatal period. Systemic maternal infection may present either as prolonged febrile illness or fulminant sepsis, and transplacental spread may result in abortion, stillbirth, or early neonatal meningitis. enteritis that occurs in infants born to women with campylobacter diarrhea may represent fecal-oral spread at or near the time of delivery. With improved microbiologic techniques and greater awareness by physicians, the prevalence and importance of campylobacteriosis in pregnancy should become better appreciated.

4/4. Neonatal death following in utero traumatic splenic rupture.

    Perinatal death due to maternal injury is unusual unless associated with extensive maternal trauma or death. An unusual case of neonatal death due to in utero traumatic splenic rupture in the absence of significant maternal injury is presented. The case alerts physicians responsible for neonatal care to the existence of treatable causes of neonatal distress following maternal trauma.
