11/17. neurologic manifestations of podophyllin toxicity.

    A young woman presented with podophyllin intoxication following topical application of podophyllin resin to genital condylomata acuminata. The disorder was marked by hallucinatory psychosis, bone marrow depression, and mild hepatic dysfunction. The psychosis and systemic disturbances resolved within 3 weeks, but were followed by autonomic and sensorimotor peripheral neuropathy. This case illustrates the transient central neurotoxicity and persistent peripheral neurotoxicity of podophyllin.

12/17. Paranoid delusions and auditory hallucinations associated with digoxin intoxication.

    The 83-year-old woman in this case report developed paranoid delusions and auditory hallucinations in association with toxic serum levels of digoxin, while remaining alert, unagitated, and coherent in thinking. No cardiovascular or metabolic abnormalities were discovered to account for her psychiatric symptoms. Her mental status rapidly returned to normal as serum digoxin declined to therapeutic levels.

13/17. Angel's Trumpet psychosis: a central nervous system anticholinergic syndrome.

    The authors warn physicians that intoxication by Angel's Trumpet (datura sauveolens) is becoming more frequent due to its use by adolescents and young adults as a legal, readily available hallucinogen. Ingestion of Angel's Trumpet flowers or a tea brewed from them results in an alkaloid-induced central nervous system anticholinergic syndrome characterized by symptoms such as fever, delirium, hallucinations, agitation, and persistent memory disturbances. Severe intoxication may cause flaccid paralysis, convulsions, and death. Treatment with intravenous physostigmine reverses the toxic effects of Angel's Trumpet.

14/17. clarithromycin-associated visual hallucinations in a patient with chronic renal failure on continuous ambulatory peritoneal dialysis.

    Visual hallucinations are a rare event in chronic renal failure and not related to uremia per se. Unreported in the literature is visual hallucinations occurring in association with the new macrolide antibiotic, clarithromycin. We describe such a case in a patient with end-stage renal disease (ESRD) maintained on continuous ambulatory peritoneal dialysis (CAPD). The combination of a relatively high dose of clarithromycin in face of chronic renal failure in a functionally anephric patient, with underlying aluminum intoxication, may have facilitated the appearance of this neurotoxic side effect. It is important to understand the pharmacokinetics of medications in face of chronic renal failure, the possibility of drug interactions, and how these factors should help guide medication therapy in the ESRD patient.

15/17. Visual hallucinations in a toddler: accidental ingestion of a sympathomimetic over-the-counter nasal decongestant.

    hallucinations are uncommonly encountered in the young child. The differential diagnosis of such behavior includes a number of potentially serious syndromes such as central nervous system malignancy, encephalitis, temporal lobe epilepsy, closed head trauma with frontal lobe confusion, hypoglycemia, drug intoxications, and childhood psychiatric syndromes. Organic explanations are most often discovered and frequently involve toxicologic causes. A case is presented of a toddler with hallucinations caused by inappropriately high doses of pseudoephedrine.

16/17. Disturbance of consciousness due to methamphetamine abuse. A study of 2 patients.

    The present paper reports 2 cases of methamphetamine abuse. Following consecutive methamphetamine administration, the patients developed acute intoxication, during which time they showed a unique type of disturbance of consciousness: delirium and twilight state. In both cases, mental status changed, passing through three distinct stages: restlessness and insomnia, hallucinatory paranoid state, and disturbance of consciousness. A review of the literature suggests that disturbance of consciousness may occur occasionally during the course of amphetamine and methamphetamine intoxication, although these symptoms can be overlooked easily due to confounding symptomatology.

17/17. The diagnosis of pathological intoxication.

    Subjects with a history of violent or psychotic behavior while under the influence of alcohol were infused with a 25% alcohol solution. Based on electroencephalograms, blood alcohol concentrations and behavioral observations, the results confirm the probable existence of a discrete syndrome of pathological reaction to alcohol without reference to the amount of alcohol consumed.
