1/3. Genital oedema in patients treated by continuous ambulatory peritoneal dialysis: an unusual presentation of inguinal hernia.

    Four patients receiving continuous ambulatory peritoneal dialysis presented with scrotal or labial oedema but no detectable cough impulse. Two of the patients later developed clinical evidence of inguinal hernia, and in the other two inguinal hernias were found at operation. These cases suggest that genital oedema occurring in a patient receiving continuous ambulatory peritoneal dialysis is a sign of a small inguinal hernia which may not be detectable clinically.

2/3. Labial adhesions presenting as urinary incontinence in postmenopausal women.

    Most discussion on labial adhesions is about the pediatric group. A case is presented of a postmenopausal woman whose main complaint was urinary incontinence. Etiology, symptoms, signs, and diagnosis are discussed. Guidelines for treatment and prevention are proposed. Because the number of postmenopausal women is steadily increasing, it is important to draw attention to this condition.

3/3. Donovanosis (granuloma inguinale) with vulval pseudo-elephantiasis.

    A 24 year old Melanesian female from the Eastern Highlands of papua new guinea presented with chronic painless ulcers of the external genitalia, anus and thighs of many years' duration and marked left labial hypertrophy for one year. Scrapings from the ulcers contained large foamy mononuclear cells in which Donovan bodies were demonstrated. The ulcers responded to tetracycline therapy however she absconded after three weeks before surgical correction of the labial pseudo-elephantiasis could be carried out. Vulval pseudo elephantiasis of the magnitude present in this case is rarely seen in papua new guinea.
