1/10. An unusual cause of facial pain: malignant change in a calcified pleomorphic adenoma in the deep lobe of the parotid gland.

    We describe a case of a malignant change in a calcified pleomorphic adenoma of the deep lobe of the parotid gland in a patient whose initial symptom was facial pain. We also discuss the relative merits of the different imaging modalities as they apply to the parotid gland.

    Pleomorphic adenoma may occur in any site in which there is salivary gland tissue. It has rarely been described arising in the nasal cavity. We report a case of septal pleomorphic adenoma and discuss the clinical presentation and surgical management.

3/10. Orofacial pain: diagnosis and treatment.

    Orofacial pain can have an inflammatory, neurologic or musculoskeletal cause. Inflammatory diseases include dental abscess, sinusitis, temporal arteritis, sialolithiasis and infections of the parotid gland. Common neurologic diseases that cause facial pain are trigeminal neuralgia, glossopharyngeal neuralgia, paratrigeminal neuralgia and cluster headaches. Musculoskeletal causes include temporomandibular joint syndrome and myofascial pain dysfunction syndrome. A clear understanding of pertinent anatomy and an organized approach to diagnosis will facilitate the evaluation of patients with orofacial pain.

4/10. maxillary sinus lymphoma: a consideration in the diagnosis of odontogenic pain.

    A patient with a known history of diffuse large cell histiocytic non-Hodgkin's lymphoma in the neck and left parotid gland presented with acute right maxillary odontogenic pain and swelling. Following endodontic treatment of the nonvital maxillary right second molar, minor masticatory discomfort persisted in the sextant but a dental etiology could not be established. Two months after the onset of symptoms, the right maxilla expanded uncontrollably and biopsy confirmed an antral lymphoma. The patient succumbed to the lymphoma and secondary complications 2 months later.

5/10. temporomandibular joint dysfunction and orofacial pain caused by parotid gland malignancy: report of case.

    A 41-year-old female with 2 years of mandibular and maxillary facial pain sought multiple medical evaluations. Symptoms were similar to those accompanying many benign temporomandibular, salivary gland, and neurological disorders. Through manual palpation, a slight swelling in the salivary gland was discovered; a malignant carcinoma was removed by parotidectomy.

6/10. Munchausen's syndrome: oral presentations.

    Munchausen's syndrome is a rare psychiatric disorder in which patients repeatedly confabulate so as to cause unnecessary investigations and operative treatments to be carried out on them. Two cases are reported and the literature reviewed. The first patient complained of repeated dental pain and subsequently underwent repeated endodontic treatment; the second had salivary gland pain and glossopharyngeal neuralgia. Munchausen's syndrome can only be diagnosed by the exclusion of organic and other disease.

8/10. temporomandibular joint dysfunction and facial pain caused by neoplasms. Report of three cases.

    The most frequent causes of painful limitation of temporomandibular joint function are myofascial pain dysfunction or anterior disk displacement. A potential problem in dealing with those patients is the risk of misdiagnosing the rare patient who has a neoplastic disease, primary or metastatic to the epipharyngeal region, parotid gland, jaws, or temporomandibular joint. Three cases are presented in which progressively worsening limitation of mandibular movement and increasing pain led to the diagnosis of a neoplasm in the temporomandibular joint region. The article illustrates the necessity of performing a thorough clinical and radiographic examination.

9/10. Submandibular sialolithiasis with concurrent sialoadenitis in a child.

    Sialolithiasis is a relatively common condition in adults, but it is rarely observed in children. We report a case in a 7-year-old child with a sialolith in the anterior right Wharton's duct resulting in a submandibular sialoadenitis. Under local anesthesia the calculus was removed by means of a sialodochotomy. Post-operative recovery was uneventful and follow-up examinations showed recovery of the function of the affected gland.

10/10. Calcified stylohyoid ligament: unusual pressure symptoms.

    Two previously unreported clinical features, namely obstruction of the submandibular salivary gland and discomfort on yawning, were encountered in a 42-year-old male with gross calcification of the stylohyoid ligament. The diagnosis was confirmed on clinical grounds, routine radiography, sialography and CT scanning.
