1/4. Traumatic intramuscular orbital haemorrhage.Retrobulbar haemorrhage is an uncommon but well-documented complication of retrobulbar anaesthetic blocks in ocular surgery. Peribulbar anaesthesia has been advocated to be equally effective but safer. The incidence of retrobulbar haemorrhage with peribulbar blocks is very low. We report a case in which a peribulbar anaesthetic for cataract surgery resulted in sight-threatening retrobulbar haemorrhage within the medial rectus muscle and required an anterior orbitotomy and direct decompression of the muscle. The surgery was successful. 2/4. Globe luxation in histiocytosis X.A boy with histiocytosis X, first diagnosed at the age of 9 months, presented at the age of 5 years with left globe luxation. Under general anaesthesia the globe was reduced and a temporary paracentral tarsorrhaphy carried out. He was started on a course of oral steroids. 3/4. Luxation of the globe and Crouzon's disease--the first reported case in singapore.A case of spontaneous luxation of the globe and Crouzon's disease is described. The luxation was reduced under general anaesthesia. Bilateral permanent tarsorrhaphies were performed to prevent further luxations and to decrease the lagophthalmos and corneal exposure. There was also mild bilateral optic atrophy and a left superior oblique palsy. The patient exhibited turricephaly, maxillary hypoplasia and shallow orbits with proptosis. 4/4. A complication of peribulbar block in a patient with exophthalmos.A patient with marked exophthalmos secondary to thyroid eye disease presented for tarsorrhaphy and removal of orbital fat. A single superolateral peribulbar injection was performed. After injection of 3.5 ml of local anaesthetic solution, the globe suddenly dislocated anteriorly. This complication has not been described previously.
