1/4. Chronic rhinitis: a manifestation of chronic lymphocytic leukemia.

    Chronic rhinitis is a condition that occasionally reflects underlying systemic disease. In such cases, physical examination, nasal endoscopy, and computed tomography may be nonspecific. diagnosis and treatment of the underlying illness may improve nasal symptoms, which may prove refractory to standard rhinitis therapy. A case of chronic rhinitis secondary to nasal involvement of chronic lymphocytic leukemia (CLL) is presented. biopsy was important in clarifying the cause of the rhinitis. Treatment of the CLL with chlorambucil was successful in alleviating nasal symptoms, and systemic effectiveness was reflected by a decrease in the patient's white cell count. Chronic rhinitis as a manifestation of CLL can be successfully managed through systemic treatment of the underlying disease. biopsy can be helpful in confirming the cause of rhinitis and should be considered in refractory cases of rhinitis.

2/4. Otorhinolaryngological aspects of xeroderma pigmentosum.

    OBJECTIVE: to evaluate the probable presence of otorhinolaryngological pathology accompanied by head and neck region skin findings in patients with xeroderma pigmentosum. methods: a total of 19 patients with xeroderma pigmentosum were investigated for otorhinolaryngological findings. The patients gave their anamnesis and underwent physical examination, audiological tests and endoscopic examination. RESULTS: various malignancies developed in 14 patients on the sun-exposed areas of the head and neck region. Multiple malignancies were found in six of them. There was no other pathological condition secondary to this rare clinical entity. CONCLUSION: xeroderma pigmentosum causes skin lesions. Some otolaryngological findings such as rhinitis, sinusitis etc. were thought to be coincidental.

3/4. hypersensitivity reaction to mizolastine: study of cross reactions.

    A 26-year-old male suffering from acute rhinitis took the first dose of Zolistan (mizolastine, 10 mg), orally, and 15 minutes later he developed intense generalized pruritus, cutaneous rash, oropharyngeal pruritus, edema on his face, difficulty in swallowing, and mild dyspnea. He was treated with methylprednisolone and epinephrine and improved within 30 minutes. The patient had not taken mizolastine before and he has avoided it since the reaction. Cutaneous tests with Zolistan and its excipients proved negative. Simple-blind oral challenge tests with the excipients and then with Zolistan were positive only with Zolistan. In order to confirm the absence of cross-reactivity between mizolastine and other benzimidazoles, we tested omeprazole, domperidone and mebendazole, all of which yielded negative results. To our knowledge, this is the second case of immediate hypersensitivity to mizolastine documented to date. In our case, the clinical history, physical examination and provocation tests allow us to establish the diagnosis of hypersensitivity to mizolastine and exclude the cross reactivity with other benzimidazole derivatives.

4/4. diagnosis of allergic disease.

    The diagnosis of allergic disease has to be individualized to each patient. In general, it consists of a good clinical history, a physical examination, and identification of specific relevant allergies by both in vitro and in vivo tests. These will contribute to an assessment of the patient's clinical status and severity of disease. One must always be aware that nonallergic mechanisms can also trigger an attack in atopic individuals. These are clinically indistinguishable from those provoked by an allergen-IgE immune response. We predict that the computer and accurate in vitro tests will join us in our offices. Before the end of this century, this high technology will make the clinical allergist a very precise diagnostician.
