1/12. Paraneoplastic pemphigus caused by an epithelioid leiomyosarcoma and associated with fatal respiratory failure.

    A patient is described who initially presented with pemphigus vulgaris, limited to the oral cavity, and weight loss. Although the various laboratory studies pointed to the diagnosis of paraneoplastic pemphigus (PNP), the underlying neoplasm was not detected until 6 months later, when the patient developed shortness of breath and routine physical examination on admission revealed an abdominal mass, which eventually was proven to be an epithelioid leiomyosarcoma. In spite of radical excision of the tumour and intensive treatment of the dyspnoea, the patient died of respiratory failure 19 months after the PNP had been diagnosed. early diagnosis of PNP is stressed to possibly prevent fatal pulmonary involvement.

2/12. Crohn's disease with oral presenting signs masquerading as chronic osteomyelitis.

    BACKGROUND: A case of Crohn's disease (CD) was diagnosed following recognition of oral and systemic signs and symptoms in a 19-year-old male patient. methods: Clinical investigation utilized included blood tests (full blood count, electrolytes, urea, creatinine, liver function tests), computed tomogrphy scans, magnetic resonance imaging scans, oral biopsies, colonoscopy and biopsies of the terminal ileum and colon. RESULTS: A diagnosis of CD was made which then allowed appropriate medical treatment to be initiated. CONCLUSION: The importance of a thorough medical history and full physical examination with appropriate investigations as dictated by clinical findings is demonstrated.

    Many clinicians have encountered patients with a somatoform disorder without recognizing the clinical presentation. somatoform disorders can confound a diagnosis, consume inordinate amounts of treatment time, and may cause treatment failure. Recognition and management of somatoform disorders in prosthodontic patients is discussed in this clinical report to assist practitioners in avoiding initiation of prosthodontic treatment for patients with physical symptoms that are caused by a psychologic disorder rather than organic disease.

4/12. Hyalinosis cutis et mucosae: a 30 year follow up of a female patient.

    Hyalinosis cutis et mucosae is an inherited disease, characterized by deposition of a hyaline-like substance in the dermis and internal organs. In the present study, we report a 30 year follow up in a female patient with hyalinosis cutis et mucosae treated using dermabrasion and oral retinoids. In our opinion, the lesion on uncovered body areas should be corrected as soon as possible to enable a normal psychophysical development. The lesions on skin and oral mucosa may be successfully removed without remission with dermabrasion in early childhood. In addition, oral retinoids in adulthood may produce some improvement.

5/12. Recurrent ectopic calcification involving the maxillofacial skeleton: A potential harbinger of Albright's osteodystrophy.

    We present a case of osteoma cutis as the initial presenting sign of Albright's osteodystrophy. Albright's osteodystrophy represents a challenge in both diagnosis and treatment because the phenotype manifests as a broad spectrum of biochemical and physical findings. The syndrome may be overlooked, particularly in the early phases when serum calcium and phosphorous levels may be normal. Although surgery is the treatment of osteoma cutis, recurrence is common, creating frustration for the patient's family and the surgeon. In the present case, a girl 4 months and 2 years of age presented with refractory ectopic calcification emanating from the maxilla and demonstrated repeated recurrence after surgical resection. This ectopic calcification was not adequately controlled until Albright's osteodystrophy was properly diagnosed and treated. A differential diagnosis and surgical approach to deal with recurrent ectopic calcification of the facial skeleton is outlined.

8/12. Characterization of human papillomavirus type 13 from focal epithelial hyperplasia Heck lesions.

    focal epithelial hyperplasia Heck lesions of a Turkish patient were shown to contain papillomavirus-specific dna, which was molecularly cloned into bacteriophage lambda. It proved to be related to human papillomavirus (HPV) type 6 dna and HPV type 11 dna. Reassociation kinetics revealed a cross-hybridization of 4 and 3%, respectively. There was no cross-reactivity with HPV type 1, 2, 3, 4, 5, 8, or 10. This papillomavirus type will be referred to as HPV type 13. The dna was characterized by cleavage with several restriction enzymes, and the cleavage sites were physically mapped. Papules from two additional cases of Morbus Heck contained HPV type 13 dna as shown by Southern blot hybridization and by the characteristic cleavage patterns. This may indicate that HPV type 13 is more frequently associated with focal epithelial hyperplasia Heck than are other HPV types.

    The importance of stress and anxiety as contributing to the complex aetiology of dental disease is becoming more widely recognized. Clinical examples of organic dental disease which were relieved when their psychological causes had been uncovered are described. It suggested that specialized psychiatric skills are not always necessary in dealing with such cases. The dentist's own personal life experience, his human understanding and his interest his patients' well-being are usually sufficient. Equal attention must be given to all data in both physical and psychological spheres if the patient's best interests are to be served.

10/12. Oral histoplasmosis in india: a case report and an overview of cases reported during 1968-92.

    Oral histoplasmosis in a 30-year-old male with no history of travel outside india is described. An ulcerating lesion was located on the hard palate. A chest X-ray was normal. Based on physical examination, regional lymph nodes, liver and spleen were not involved. The diagnosis was established by demonstrating yeast-like budding cells in a biopsy of the lesion and by isolating histoplasma capsulatum in pure culture. The identity of the isolate was confirmed by a chemiluminescent dna-probe assay and the exoantigen test. A review of the Indian literature from 1968 to 1992 revealed the occurrence of 25 authentic cases of histoplasmosis in india. In 19 cases, lesions were confined to the oral cavity confirming prior observation that histoplasmosis in Indian patients tends to occur primarily in extrapulmonary sites, particularly the oral cavity.
