1/4. Anterior sacral meningocele and the scimitar sign. Report of a case.

    Anterior sacral meningocele is an unusual lesion that usually presents as a presacral mass. This is a case presentation of a young woman who presented with amenorrhea. On physical examination, a large presacral mass was found. ultrasonography revealed a large cystic structure. radiography of the pelvis demonstrated a sacral deformity or "scimitar sign" that is pathognomonic for anterior sacral meningocele. The diagnosis was confirmed by computed tomography with myelographic enhancement.

2/4. behavior therapy in a gait-training program for a child with myelomeningocele. A case report.

    A behavior therapy program consisting of positive reinforcement contingent upon walking with braces and crutches was conducted with a four and one-half-year-old child with myelomeningocele. Behavioral physical therapy was begun first in the clinic setting and then in the home. Follow-up clinic assessments document maintenance of functionally independent ambulation over a seven-month period. The results support the efficacy of using behavioral techniques in a physical therapy program for a child with myelomeningocele. The need for further research and development of physical therapy programs using behavioral techniques is briefly discussed.

3/4. Intraoperative vecuronium anaphylaxis compounded by latex hypersensitivity.

    BACKGROUND: anaphylaxis to latex or muscle relaxant drugs has been demonstrated in the perioperative period but there are no reports of a patient with anaphylaxis to both of these substances. methods: We report a patient with meningomyelocele who was initially thought to have anaphylaxis to latex during induction of anesthesia. After recurrence of anaphylaxis following removal of latex as the suspected agent, the muscle relaxant vecuronium was found to be the actual etiology. RESULTS: skin testing to several muscle relaxants and other anesthetic medications confirmed a hypersensitivity to vecuronium. skin testing and subsequent physical contact with latex also established a hypersensitivity to this substance. CONCLUSIONS: This report demonstrates the need to scrutinize closely every notation in the anesthesia record when evaluating perioperative anaphylaxis and to consider that hypersensitivity to more than one substance may be present.

4/4. tissue expansion for the treatment of myelomeningocele. Case report.

    A baby boy who had been born with a large myelomeningocele had a ventriculoperitoneal shunt inserted at the age of 6 months. Two months later it was revised, after which he developed satisfactorily both mentally and physically. He presented at the age of 13 months because the epithelialisation of the myelomeningocele was both fragile and incomplete. Enough soft tissue was obtained to cover the defect, which was 13 x 10 cm, by the use of two 600 ml expanders placed subcutaneously one on either side of the deformity.
