1/6. Two brothers with varying combinations of severe developmental delay, epilepsy, microcephaly, tetralogy of fallot and hydronephrosis.

    We report on a sib pair who manifest a pattern of anomalies which appears to be unique and for which we are unable to provide a cytogenetic or molecular genetic explanation. While a number of their physical features are distinct, their overall appearance and pattern of neurological impairment suggest they suffer from the same genetic disorder.

2/6. Cloacal outlet obstruction with an ectopic ureter.

    Cloacal malformation occurs in approximately 1 in 50,000 live female births. Prenatal ultrasound may lead to the diagnosis in selected cases. We report an unusual case of prenatally detected single-system hydronephrosis with a nonvisible bladder and worsening oligohydramnios. Labor was induced at 35 weeks' estimated gestational age. On physical examination, a single perineal opening was noted consistent with cloaca. endoscopy revealed an obstructed ectopic ureter at the level of the sphincter, an undeveloped bladder and vagina, and a fistula to the rectum. A low loop cutaneous ureterostomy and right upper quadrant loop colostomy were performed. The absence of a typical fluid-filled pelvic structure may confound the prenatal diagnosis of cloaca.

3/6. Ultrasound experience with prenatal genitourinary abnormalities.

    The ability to diagnose genitourinary abnormalities in the fetus frequently poses management dilemmas for the urologist. Our experience with 13 cases of abnormal fetal ultrasonography examinations thought to be genitourinary in nature underscores difficulties posed by this new technology. In 3 cases the prenatal diagnosis was eventually found to be incorrect. In 1 case, vesicoureteric reflux gave the appearance of hydronephrosis that resolved after birth. In 3 cases in which intervention was deemed necessary, the eventual outcome was unaffected. Prenatal ultrasound is most useful when detecting occult hydronephrosis that would have gone unnoticed in the routine newborn physical examination. However, our patients received no benefit from fetal intervention.

4/6. Ultrasonographic diagnosis of fetal hydronephrosis in utero.

    ultrasonography offers a noninvasive method to visualize antenatal or neonatal anatomy which will lead to early detection and treatment of abnormalities. In these 2 cases, ultrasound allowed prompt diagnosis of hydronephrosis despite normal physical examination and laboratory values at birth.

5/6. Imperforate hymen with bilateral hydronephrosis: an unusual emergency department diagnosis.

    Imperforate hymen is a rare diagnosis to make in the emergency department and important to treat immediately. hydronephrosis is a known but rare complication of an imperforate hymen. We present the case of a premenstrual adolescent with an undiagnosed imperforate hymen complaining of increasing abdominal pain. The article discusses the history, physical examination, evaluation, differential diagnosis, ultrasound findings, complications, and prompt surgical treatment necessary.

6/6. Ultrasonic diagnosis in a case of hydronephrosis.

    A case of hydroneophrosis is reported in which the diagnosis has been confirmed on the ground of renal echography. The physical principles of the method are outlined. Its expediency, safety and efficiency are emphasized. It is considered the most reliable diagnostic procedure if hydronephrosis is suspected in case of renal occlusion.
