1/9. Treatment of stingray injury with topical becaplermin gel.

    Stingray injuries are most commonly characterized by localized pain, ulceration, and edema; however, potentially serious sequelae may result, including tissue necrosis and life-threatening infection. This case report describes a stingray injury that had progressed to ulceration and was treated with topical application of recombinant human platelet-derived growth factor-BB (becaplermin gel 0.01%) and a standardized regimen of wound care.

2/9. The clinical application of a fetal membrane on a diabetic patient's injury.

    A description of the successful use of a fetal membrane in the recovery of a injured patient is presented. A diabetic female patient, 63 years old, was accidentally injured with a winnowing fork, which opened the skin of the front side of her left foot. For first two weeks, she was submitted to a treatment involving antibiotics, analgesics and antiinflammatories, but the infection did not disappear and the pain was almost unbearable. At the end of the third week, the injury underwent surgical washing with epidural blockage; suitable insulin dose and oral antidiabetics controlled the high-glucose concentration. However, due to the intensity of the pain, a risky epidural blockage was administered every four hours for 12 days. Finally, a radiosterilised human fetal membrane, which was collected at HCSAE and processed at ININ, was used as a biological wound dressing. Biological control of the tissue was performed at Facultad de Quimica de la Universidad Nacional Autonoma de mexico (The chemistry faculty of the National Autonomous University of mexico) (UNAM). As a result, the pain first diminished and then stopped. The healing process started in a few days and led to a complete recovery in 2 1/2 months. At present, the patient is in good condition, living a normal life.

3/9. Complete dorsal tarsometatarsal dislocation of the first metatarsal.

    The authors describe a rare case of Lisfranc's dislocation that occurred after a 13-year-old boy kicked the ground while playing soccer. He presented with complete pure dorsal tarsometatarsal dislocation of the first metatarsal. He was treated succesfully by closed reduction and percutaneous K-wire fixation. No complication was found and he returned to his preinjury lifestyle at 3 months. No sequelae are found 3.5 years later.

4/9. Vipera berus adder bite in the water, complicated by rapid shock. A case history.

    A case of a child who presented with severe and rapid shock after receiving a common adder (vipera berus berus) bite in sea water is presented. Although most poisonous snakebites in europe tend to be relatively minor and uncomplicated, the present case highlights the need to regard all viper bites as life-threatening accidents, before proved otherwise by a medical professional.

5/9. Indication and practice of reverse flow extensor digitorum brevis muscle flap transfer.

    Although the pedicled extensor digitorum brevis (EDB) muscle flap is a versatile flap, there are not many reports about it. Furthermore, there are few reports about the reverse vascular flow EDB muscle flap. The lateral tarsal artery coming from the dorsalis pedis artery nourishes the EDB muscle flap. Cutting the dorsalis pedis artery proximal to the flap can elevate this flap with reverse vascular flow. The authors treated eight patients with a reverse vascular flow EDB muscle flap. All flaps survived, with minor repair in two cases. The follow-up period ranged from 4 months to 1 year. This flap has two pivot points for creating a reverse vascular flow pedicled flap and a large arc of rotation for coverage of the dorsal foot. The authors confirmed that this flap is very versatile for soft-tissue reconstruction of the distal dorsum of the foot. The blood supply is reliable and the operative procedure is not complicated and can be performed under regional anesthesia. With this flap, a well-vascularized bed can be prepared for coverage with a skin graft. The major disadvantage is numbness in the first web of the foot, but this does not cause problems in daily life.

6/9. Land mine injury: functional testing outcome.

    We present a case of a war patient treated with external fixation for a complex land mine injury involving the fracture of the tibial and fibular bones, which occurred at the front line during the war in croatia and Bosnia and Herzegovina. Excessive destruction and foreign body penetration into the distal two-thirds of the right leg and foot endangered not only the patient's lower extremity but his life as well. Nevertheless, the patient's life, as well as extremity, was saved due to an intensive treatment. A 100-month follow-up showed a bridge callus between the tibia and fibula, tibial bone defects, tibial anterior angulation of 5 degrees, and arthrosis of the right upper ankle joint. Despite a relatively unsatisfactory X-ray finding, the functional testing on the dynamometry system Cybex 300 showed surprisingly good results. There was a satisfactory functional recovery of the treated extremity: the patient could walk without any help even on rocky grounds and was actively involved in his sheep farm duties.

7/9. A case of Freiberg's disease in an adult patient.

    Freiberg's disease is an osteochondrosis of the IInd metatarsal head that prevalently develops during the second decade of life and that is the cause of important painful symptoms that resist conservative treatment. The disease is quite rare and must be treated surgically during its early phase in order to prevent progression that may result in permanent changes in the metatarsal head. It is the purpose of this study to describe the clinical case of a patient aged 30 years affected with Freiberg's disease, diagnosed at the age of 15 years, and never submitted to either conservative treatment or surgery.

8/9. Microsurgical reconstruction in children's burns.

    There have been few reports on microsurgical procedures performed on children who have been left with disability and deformity as a result of burn injury. The reality is that whilst there are many children who could benefit from such procedures they do not have access to the resources that can provide them. Where these resources are available there has been a reluctance to use them partially due to a misconception that such procedures are technically challenging and carry significant risk. This paper presents a series of 21 children in whom 29 microsurgical procedures were performed. There were two failures but in all other cases there was significant improvements in the functional ability and/or the aesthetic appearance of the child. This series includes novel variations on the antero-lateral thigh flap; tissue engineering, dual flap harvest, size and age. Refinements of inset are described to prevent pin-cushioning and linear scar contracture. In selected cases microsurgical reconstruction can produce life long benefits, with minimal surgical and donor site morbidity.

9/9. An unusual case of talonavicular dislocation with associated ipsilateral foot fractures.

    We describe a previously unreported form of a talonavicular dislocation complicated by navicular, talar neck and open calcaneal fractures. Prompt recognition and rapid reduction of fractures and dislocations gives the best possible outcome. The presence of serious distracting injuries can alter priorities, however the "lesser" injuries often lead to long term morbidity after recovery from the life-threatening injuries.
