1/4. The subcutaneous fascial analogue of myositis proliferans: electron microscopic examination of two cases and comparison with myositis ossificans localisata.

Two cases of the so-called fascial analogue of myositis proliferans were investigated by histological and electron microscopic methods. It was found that the structure of the fascial variant corresponds almost completely to the true myositis proliferans localized within the musculature. The electron microscopic observations show a preponderantly histiocytic differentiation of the cells and strongly activated proliferating capillaries, and exclude a myogenic origin of the characteristic ganglion-like giant cells. Ultrastructurally a traumatic genesis appears possibly, the cells of the lesion could derive from multipotent cells of the microvasculature. The relations to myositis ossificans and fascitis nodularis are discussed.

2/4. Clinical holistic medicine: whiplash, fibromyalgia, and chronic fatigue.

Holistic treatment of the highly complex, "new diseases" are often possible with the tools of consciousness-based medicine. The treatment is more complicated and the cure usually takes longer than for less-complex diseases. The problem with these patients is that they have less easily accessible resources than most patients, as they suffer from a combined socio-psycho-physical problem with depression, poor social standing, low confidence, and low self-esteem. Often, they have also already tried most of the specialist and alternative treatments on the market. To cure them, the most important thing is to coach them to improve their social life by changing their behavior to be of more value to others. Holding and processing must be especially careful and the contract with the patients must be extremely explicit in order to work on their personal development for 6-12 months. The new diseases can be cured with consciousness-based medicine if the patients are motivated and keep their appointments and agreements. Low responsibility, low personal energy, little joy of life, and limited insight into self and existence are some of the features of the new diseases that make them difficult to cure. The important thing is to keep a pace the patient can follow and give the patient a row of small successes and as few failures as possible. The new diseases are a challenge, a unique chance to improve communication, holding, and processing skills.

3/4. An embodied geography of disablement: chronically ill women's struggles for enabling places in spaces of health care and daily life.

Using data gathered from 18 interviews with women who have developed arthritic illnesses we illustrate how becoming a chronically ill woman is a geographically uneven process. By examining changes in the spatial configuration of everyday life, performances of the ill female self in places, and relations with places of life important to these women, such as the health care clinic and workplace, we show how women's capacities to struggle effectively for enabling identities are challenged. Further, we argue that embodying illness involves women and others, including doctors, assigning meanings to their changing material, lived and represented places in the world.

4/4. Common pain scenarios.

BACKGROUND: Vertebral fractures and fibromyalgia, while very different and unrelated clinical conditions, are common pain conditions that can, at times, be difficult to diagnose and manage. OBJECTIVE: To discuss the investigation and treatment of two common pain conditions. DISCUSSION: Following a structured program for assessment and treatment, most people with both of these conditions can experience substantial improvement in the quality of their life.
