1/5. October 1998--61 year old male with brain tumor and oral, lung, and palpebral masses.

    In Jan. 97 a gliosarcoma was diagnosed in a 61-year- old man after a 6-month history with neurological deficits. A total physical examination, laboratory tests, chest x-ray and abdominal ultrasound scanning revealed no gross abnormalities. Surgery was followed by brain radiation therapy and 6 months later there were metastases to the oral cavity, right palpebra and both lungs. The histological findings of the oral and palpebral metastases revealed only the sarcomatous component. We are aware of 15 cases of gliosarcoma with extraneural metastases, and in 4 of these, the metastases contained only the sarcomatous component. We believe that our case represents the fifth case of pure sarcomatous metastases.

2/5. Malignant lymphoma presenting as unilateral eyelid mass.

    A 58 year-old man presented with a large mass in the upper eyelid of his only eye. He had no systemic symptoms, but his physical examination revealed slightly enlarged preauricular and axillary lymph nodes. Biopsies of the upper eyelid mass and the ipsilateral preauricular lymph node revealed a poorly differentiated lymphocytic lymphoma. This case is of ophthalmic interest because the eyelid mass was the initial manifestation of the lymphoma.

3/5. Orbital infiltration by eyelid skin carcinoma.

    This paper presents 5 patients seen in the eye Clinic, Royal victoria Hospital, Belfast with orbital infiltration of malignant eyelid tumours. The physical features and natural progression of the disease are described. The value of CT scans in determining the extent of the disease and patient management are described.

4/5. neuroblastoma diagnosed by fine needle aspiration biopsy.

    Small-cell malignancies of childhood are a source of diagnostic difficulty, both in cytology and histology. Four cases of neuroblastoma are presented that were diagnosed by fine needle aspiration cytology. The importance of a complete history, physical examination and laboratory evaluation is stressed. In addition to the usual description of cytologic detail obtained by routine methods, a new method of study is introduced: the potential use of plastic embedding and histochemical studies in small-cell tumors is discussed.

5/5. An adjuvant afterloading brachytherapy device for use after orbital exenteration in patients with orbital malignancies.

    In cases of malignant diseases situated within the orbit and threaten to extend beyond it, removal of the orbit and periorbital contents may be indicated, followed by adjuvant chemotherapy or irradiation. An effective radiotherapy device for treatment of residual or suspicious cancer in the enucleated orbit consists of an external-beam radiation source (60Co) and an intraoperatively fitted mould as a guide for the flexible afterloading tubes using a 192Ir source. Considering the physical dose distribution and the local situation, this therapy permits a high dose with homogeneous dose distribution to the target area of the orbit but also allows significant dose reduction to adjunctive critical structures. Four patients with different orbital malignancies were treated by means of a combination of percutaneous radiotherapy with afterloading brachytherapy following orbital exenteration. The individual mould of the orbital cavity is modeled at the end of the operation. The technique of radiotherapy, dose distribution, and follow-up of the disease are demonstrated. Orbital malignancies with an extremely poor prognosis may profit from the application of this combined radiotherapy to avoid supraradical surgical intervention.
