1/33. poems syndrome, steroid-dependent diabetes mellitus, erythema elevatum diutinum, and rheumatoid arthritis as extramedullary manifestations of plasma cell dyscrasia.

    poems syndrome is a rare synopsis of different multisystemic disorders (polyneuropathy, organomegaly, endocrinopathy, monoclonal gammapathy, and skin lesions) associated with plasma cell dyscrasia. We herein report the atypical case of a 44-year-old white man presenting with glomerulopathy, poems syndrome, and erythema elevatum diutinum with a few-year history of non-insulin-dependent diabetes mellitus (NIDDM) and seronegative rheumatoid arthritis (RA) as early manifestations of IgAlambda multiple myeloma. The prescription of 1 mg/kg/day prednisone improved the patient's features dramatically. skin lesions improved by the association of glucocorticoids and plasma exchange, recurred when plasmapheresis ceased, and remitted when plasma exchange was reintroduced. NIDDM requiring insulinotherapy recurred when corticoids were discontinued and remitted when prednisone was reintroduced. However, prednisone and plasmapheresis had no effect on polyneuropathy, M-paraprotein, and plasma cell dyscrasia in our patient, who developed indolent multiple myeloma a few years later. We thus concluded that poems syndrome, steroid-dependent diabetes mellitus, rheumatoid arthritis, RA, and skin vasculitis in our patient were triggered by plasma cell dyscrasia.

2/33. The relationship between facial annular erythema and anti-SS-A/Ro antibodies in three East Asian women.

    A distinct annular erythema developed on the cheeks of three East Asian women who had anti-SS-A/Ro (SSA) antibodies. The erythema was characterized by a wide, elevated border and central pallor. Histologically, there was a coat-sleeve-like infiltration of lymphocytes around the blood vessels, appendages, and secretory gland cells in the dermis. Immunohistological analysis clarified that the majority of infiltrating lymphocytes were CD4-positive T cells. Abnormal expression of hla-dr antigens in the perivascular, appendage, and secretory gland cells in the dermis was also observed. The differential diagnosis of the three patients lay between Sjogren syndrome (SjS), Sjogren/systemic lupus erythematosus overlap syndrome and an asymptomatic clinical state. These results are consistent with recent findings of major histocompatibility complex class II expression on target organs in various autoimmune diseases. Based on these findings, erythema appears to represent a broad cutaneous manifestation of these diseases. Furthermore, the presence of SSA antibodies, aberrant HLA-DR expression, and sun exposure may be responsible for the development of erythema.

3/33. Corneal laceration and forehead erythema: a syndrome in infants.

    PURPOSE: To illustrate a syndrome of transient erythema in the first division of the fifth cranial nerve caused by minor corneal laceration, occurring in infants. methods: case reports are presented, together with photographs. These must, of necessity, usually be obtained by the general practitioner because of the short duration of the signs. RESULTS/CONCLUSIONS: Opinion is sought as to whether other practitioners have recognized this association and whether it has been described elsewhere.

4/33. Persistent annular erythema of infancy associated with intestinal Candida colonization.

    We report a case of persistent annular erythema of infancy in a 4-month-old boy. Physical and laboratory parameters showed no sign of internal disease or specific infection except a massive candida albicans colonization (> 103 organisms/mm3) of the lower gastrointestinal tract. Oral treatment with amphotericin b for 2 weeks resulted in a complete remission of the skin lesions indicating Candida colonization as a trigger.

5/33. Tinuvin P in a spandex tape as a cause of clothing dermatitis.

    We report a 54-year-old female with allergic contact dermatitis from 2-(2-hydroxy-5-methylphenyl)benzotriazole (Tinuvin P) in a spandex tape sewn into underwear. Tinuvin P is used as an ultraviolet light absorber for stabilizing plastics and the other organic materials against discoloration and deterioration. It is effective in protecting synthetic and natural fibers, polyesters, chlorinated polyesters, polystyrene, polyvinyls, cellulose acetate, ethyl cellulose, acrylates, dyes, waxes, detergents, cosmetic formulations, etc. Our patient developed itchy erythema on her shoulders, chest and upper back after wearing underwear for 1 night. She showed positive reactions to spandex tape sewn into the underwear. She also reacted to 2-(2-hydroxy-5-methylphenyl)benzotriazole (Tinuvin P), which was contained in the spandex. No cross-reaction to other benzotriazoles was seen.

6/33. Unusual cutaneous manifestations of sarcoidosis.

    The diagnosis of sarcoidosis is best established when there is evidence of granulomatous involvement in two or more organs. Three patients with unusual cutaneous manifestations of sarcoidosis, including palmar and follicular lesions, nonscarring alopecia, and widespread plaque or erythrodermic sarcoidosis, are reported. In all three patients the skin biopsy was important in establishing the diagnosis.

7/33. Viral-associated trichodysplasia in patients who are immunocompromised.

    Viral-associated trichodysplasia of immunosuppression is a newly described clinicopathologic entity found in patients who are undergoing drug-induced immunosuppression to prevent organ transplant rejection. patients have numerous erythematous papules concentrated in the central portion of the face and variable degrees of hair loss, most severely affecting facial hair. Histologic findings of facial papules are highly distinctive and unique, and suggest that the entire machinery of the follicular bulb is devoted to the manufacture of inner root sheath-type keratin. Electron microscopy reveals intranuclear viral particles, but precise viral identification has not yet been achieved.

8/33. A recalcitrant, erythematous, desquamating disorder associated with toxin-producing staphylococci in patients with AIDS.

    Although staphylococcal infections are common in patients with AIDS, staphylococcal toxin-related disorders have rarely been described. Five cases of a staphylococcal toxin-associated syndrome characterized by prolonged erythema, extensive cutaneous desquamation, hypotension, tachycardia, and multiple organ involvement are described in patients with AIDS. These illnesses were recurrent and recalcitrant with a mean duration of 50 days. Toxic shock syndrome toxin-1-producing staphylococci were isolated from three and staphylococcal enterotoxins B and A from one patient each. Sources of organisms were blood, one patient, and soft tissues and nasal accessory sinuses, two patients each. Three of the five patients died of renal failure and central nervous system abnormalities. One survivor required intubation for respiratory failure. All individuals manifested a marked diminution of CD4 cells. Other laboratory abnormalities included azotemia and prolongation of partial thromboplastin time. oliguria occurred in three patients. Thus, this recalcitrant erythematous desquamative disorder appears to be a variant of staphylococcal toxic shock syndrome in certain subsets of immunocompromised individuals.

9/33. Erythemas caused by electrodes while monitoring neuromuscular blockade: three cases.

    skin erythemas formed in three patients during surgery at the sites where negative electrodes had been attached to stimulate the ulnar nerve for a neuromuscular transmission monitor (Relaxograph). The patients were all women, aged 52, 62, and 74 years, and general anesthesia lasted 8 h 20 min, 4 h 50 min, and 8 h 45 min, respectively. The electrodes used were disposable ECG electrodes in the first two patients and one designed for a neuromuscular monitor in the third; all were carbon-coated and then covered with gel. However, when the electrodes were detached from the lesion, they all showed loss or damage of the carbon coating under the gel. We recommend balancing the merit of monitoring with the risk of complications, even when applying an apparently safe, noninvasive monitor.

10/33. Nuclear bilateral Bell's palsy and ageusia associated with mycoplasma pneumoniae pulmonary infection.

    This case report describes a case of nuclear bilateral Bell's palsy and ageusia associated with mycoplasma pneumoniae infection. magnetic resonance imaging evidenced T2-weighted hyper-intense protuberantial lesions. Such topography leading to a nuclear palsy contrasts with previously reported infectious diplegia involving only peripheral facial nerves, and has not yet been described in the spectrum of M. pneumoniae post-infectious neurological manifestations.
