1/4. Pectoralis major tendon ruptures: when to operate.

    The treatment of pectoralis major tendon ruptures has been the subject of much debate. The classical history of the injury is forced abduction and external rotation. The cases of two patients (an amateur rugby union player and a recreational snowboarder) are reported. The diagnosis was made by clinical examination in both patients, and both were operated on more than two weeks after injury. After surgery and a graduated rehabilitation programme, both men were able to return to their sporting activities. These two cases highlight the advantage of operative management in this uncommon condition.

2/4. Surgical repair of complete proximal hamstring tendon ruptures in water skiers and bull riders: a report of four cases and review of the literature.

    OBJECTIVE: Hamstring strains are one of the most common muscle strains in athletes; however, complete rupture of the proximal hamstring origin is rare and results from significant trauma. The objective of this paper is to present our experience of management of complete ruptures where surgical repair resulted in good results in both acute and delayed cases. methods: Two water skiers and two bull riders sustained complete rupture of the proximal origin of the hamstring muscles. All underwent repair of the hamstring origin and sciatic nerve neurolysis. A post operative hamstring rehabilitation programme was instituted. Regular follow up was performed at 2, 3, 6, 9, and 12 months. RESULTS: At a minimum final follow up of 12 months all patients had regained functional knee flexion strength with no pain and a near normal range of knee flexion. All four individuals were able to return to their previous line of work and three were able to return to their pre-injury level of sport. CONCLUSION: Complete rupture of the hamstring origin is a potentially devastating sports injury that has implications affecting the individual's activities of daily living as well as potential as a sportsperson. Surgical repair restores the distorted anatomy, allows early functional rehabilitation, and avoids the potential debilitating neurological problem of gluteal sciatica.

3/4. Stress fracture in the world class athlete: a case study.

    Stress fracture presents a difficult problem in the high performance, world-class athlete. Competitive demands provide little tolerance for, or agreement with, prolonged periods of rest which are the first line of conservative treatment methods. The use of a specifically programmed, pulsed, low-intensity ultrasound device to shorten the time of healing was investigated in a well-known gymnast with an Olympic deadline. Prior animal, in vitro, and clinical studies had established the safety and effectiveness of this device in fractures. The location of the stress injury was in the mid-tibia which is considered to present the greatest challenge to an early healing result. The low-intensity ultrasound device was prescribed for daily use at home. At 3 wk after the start of low-intensity ultrasound, the stress fracture responded well and the patient was allowed use of tumble track, trampoline, and to do some weight-bearing activities, such as jumping in the pool and loading-type activities. At 4.5 wk, the patient progressed to full workout activities and participated in a trial meet for the Olympics. At 6 wk, the patient's participation in the women's gymnastic team event was a factor in the united states receiving a gold medal.

4/4. Intra-articular lipoma of the hip. A case report.

    This report describes a rare intra-articular lipoma of the hip which developed in a 31-year-old male after a football injury. A plain X-ray and an MRI scan of the hip showed no bony abnormalities. After the patient failed to improve with a rehabilitation programme, arthroscopy of the hip was performed, revealing a lump in the acetabular fossa associated with a labral tear. A histopathological analysis confirmed the diagnosis of an intra-articular lipoma of the hip joint, which to our knowledge is the first reported case in the literature.
