1/28. Cutaneous angioleiomyoma.

    Leiomyomas are smooth-muscle tumors that may be encountered in the practice of podiatric medicine when they affect the arrector pili muscles or the smooth muscle of a vessel wall. In the present case, the lesion was located on the heel, an area with no hair growth; this further supports the diagnosis, as the most likely origin of the lesion was the smooth muscle of a blood vessel. Vascular leiomyomas show a greater predilection for the lower extremities than their superficial counterparts. Surgical excision is the treatment of choice for these benign lesions, whose prognosis is excellent. When a patient presents with a painful mass in the lower extremity, the diagnosis of leiomyoma should be considered.

2/28. Multiple piloleiomyomas associated with solitary angioleiomyoma.

    A 58 year-old male had reddish scattered papules strictly confined to the right side of his upper back and neck, right shoulder, and right upper arm. Additionally, he also noticed a subcutaneous nodule on his left thigh. Histopathological examinations revealed that the papules on the back, nape of the neck, shoulder, and arm were multiple piloleiomyomas, as shown by the proliferation of bundles of smooth muscles in the dermis. The subcutaneous nodule of the thigh was angioleiomyoma with a well-circumscribed lesion composed of smooth muscles and blood vessels. Ipsilaterality and segmentality of the distribution of the papules of piloleiomyomas and probable family history that his mother had similar papules with a similar distribution suggest the nevoid character of our case. Piloleiomyomas are often reported to be associated with leiomyoma of the uterus or other organs. This is the first reported case of multiple piloleiomyomas with solitary angioleiomyoma in the literature.

3/28. Angiomyomatous hamartoma and associated stromal lesions in the right inguinal lymph node: a case report.

    Angiomyomatous hamartoma is a rare disease with a predisposition for the inguinal lymph nodes. A 51-year-old male patient visited a local hospital because of a right inguinal mass, measuring 3 x 4 cm in size, which was resected. The resected specimen showed irregularly distributed thick-walled vessels in the hilum, extending into the medulla and focally into the cortex of the node, eventually becoming more dispersed and associated with smooth muscle cells splaying into sclerotic stroma. These findings are compatible with an angiomyomatous hamartoma. Another tumor-like mass appeared shortly after the resection at the same location, but was not an angiomyomatous hamartoma, rather it was composed of edematous stromal tissue with proliferating smooth muscle cells. The stromal component included thick-walled blood vessels and lymphatics. Although it could not be determined whether these associated changes in the surrounding stroma are a cause or an effect of angiomyomatous hamartoma, they indicate the clinical difficulty in determining an appropriate area of resection and may provide clues to the pathogenesis of angiomyomatous hamartoma.

4/28. Benign renal angiomyoadenomatous tumor: a previously unreported renal tumor.

    We describe a unique benign tumor of the kidney in a 93-year-old man. Microscopically the tumor was composed of an intimate intermixture of epithelial component and smooth muscle tissue, which formed the stroma of the tumor. In some parts the epithelial cells in adenomatous tubular formations were endowed with clear snouts. These clear snouts had a blister-like quality and grew on the secretory cells lining the tubules. No atypias, mitoses, or pleomorphism were present in the tumor. The muscular component consisted of poorly cellular, HMB-45-negative, leiomyomatous bundles, which greatly differed from that of angiomyolipoma. It encircled the whole tumor and intimately intermingled with the epithelial component. These leiomyomatous bands formed focally abortive vessels, which had incomplete and irregular walls and lacked an elastic layer. Even more interesting was a peculiar vascularization of the tumor. All epithelial tubular structures of the tumor revealed an intimate association with small capillaries. A fine labyrinth of the capillaries rimmed the rows of the epithelial cells. To the best of our knowledge this tumor differs from all previously described.

5/28. Angiomyofibroblastoma of the male inguinal region.

    We present a case of benign angiomyxoid tumor arising in the inguinal region of a 27-year-old man. The tumor was a gelatinous mass completely encapsulated by a thin fibrous capsule with no hemorrhage or necrosis. Histologically, a proliferation of spindle cells as well as occasional pleomorphic cells was observed within the myxofibrous stroma, intermingled with abundant capillary-sized blood vessels. Immunohistochemical staining of the tumor demonstrated spindle, oval, and pleomorphic cells equally positive for vimentin, desmin, and CD34, but not for alpha-smooth muscle actin. Based on these histologic and immunophenotypic features, we conclude that this angiomyxoid tumor of the male inguinal region is indistinguishable from the female angiomyofibroblastoma of the pelvic and perineal regions.

6/28. Hepatic angiomyoma: CT and MRI findings.

    angiomyoma is a benign tumor that arises from soft muscular tissue within the blood vessel wall. This lesion has been found in different organs. The preferential location of these tumors is the lower extremity. We describe the rare case of a hepatic angiomyoma and present the radiologic findings of computed tomography and magnetic resonance imaging.

7/28. Tc-99m RBC perfusion and blood-pool scintigraphy in the evaluation of vascular leiomyoma of the hand.

    A 62-year-old man with a soft, non-tender, movable mass 2.5 x 2.5-cm in diameter in the volar surface of the right index finger over the proximal phalanx underwent Tc-99m RBC perfusion and blood-pool scintigraphy to evaluate the vascular nature and extent of the mass. Highly increased activity on early and delayed blood-pool images with increased perfusion was demonstrated in the mass. The lesion with high flow rates and large blood pool spaces was considered highly suggestive of one of the various types of peripheral hemangioma. angiography revealed a vascular neoplasm with tumor vessels. Microscopic examination of the resected tumor revealed vascular leiomyoma containing numerous dilated vascular channels. These scintigraphic abnormalities were regarded as resulting from hypervascularity demonstrated angiographically and blood pooling within the dilated vascular channels demonstrated histologically. It is concluded that Tc-99m RBC perfusion and blood pool scintigraphy may be an important non-invasive approach to demonstrate vascular leiomyoma prior to surgical biopsy or resection.

8/28. Auricle angioleiomyoma.

    Angioleiomyomas are subcutaneous benign tumors composed of numerous blood vessels with disoriented and hyperplasic smooth muscle layers. The incidence in female is higher than in male, with a ratio of 1.7 to 1.0. They are usually located at the lower extremities and rarely located at head and neck region. The case we report is a 66-year old male patient. The chief complaint was a brownish tumor over helix of left auricle with progressive enlargement noted for three years. physical examination revealed an elastic, non-tender nodular lesion 15 x 15 x 6 mm in size and ovoid in shape. No other similar lesion was found on his face, neck and extremities. The tumor was excised entirely with clear margins. The diagnosis was angioleiomyoma via histology.

9/28. Angioleiomyoma in the tonsil: an uncommon tumour in a rare site.

    An angioleiomyoma was excised from the palatine tonsil in a 30-year-old woman who complained of a painless swelling in the region of the tonsil. The mucosa covering the lesion showed prominent dilated blood vessels, necessitating vascular laboratory investigations to exclude any possibility of an aneurysm arising from the carotid system. The mass was excised with little blood loss. This was an uncommon tumour in an unusual site and required radio-imaging and angiographic studies of the great vessels of the head and neck before any operation was attempted. To the best of our knowledge, this is the first case reported in English.

10/28. Angiomyofibroblastoma of the vulva: report of a case with immunohistochemical and molecular analysis.

    A 43-year-old woman presented with a mass in the subcutaneous tissue of the right labium majus. A lipoma or Bartholin gland cyst was suspected and excision of the lesion was performed. The lesion was well circumscribed, and histological examination revealed a typical angiomyofibroblastoma. The lesion was composed of alternating hypocellular edematous and hypercellular areas with abundant vessels, and plump tumor cells were loosely dispersed or aggregated mainly around the vessels. Tumor cells were immunoreactive for vimentin and desmin but negative for muscle actins. Ultrastructurally, the tumor cells contained a moderate amount of rough endoplasmic reticulum and abundant intermediate filaments, and had primitive junctions. Pinocytotic vesicles or basal lamina were not evident. Immunohistochemical studies also revealed that the tumor cells expressed basic fibroblast-growth factor, vascular-endothelial-growth factor, and stem-cell factor, factors that may contribute to the rich vascularity and mast cells within the tumor. reverse transcription-polymerase chain reaction detected high mobility group I-C (HMGI-C) transcripts in the tumor tissue. Because the expression of HMGI-C is regulated by developmental and differentiation processes and is not found in adult normal tissues, HMGI-C may be involved in the tumorigenesis of angiomyofibroblastoma.
