How do we distinguish differential diagnosis of leuckocoria?

DIFFERENTIAL DIAGNOSIS OF LEUKOCORIA

Cataract
Retinoblastoma
Toxocariasis
Coat´s disease
ROP
PHPV
Retinal detachment
Coloboma
Retinal dysplasia
Norrie´s disease

How to distinguish?
----------

Who's "we"?
The idea of leucocoria is that it's one sign that all of these things have in common. It's a nice thing to have, because even a layperson can sometimes notice these, and they all need to be referred to a professional to distinguish one from the other.
To distinguish any of these from another, you have to actually dilate the pupil and look deeper into the eye. A cataract is fairly anterior, at the level of the lens, then the vitreous (eg PHPV), and the retina's at the very back. Simply by looking at the structures you can determine what the problem is and where it lies. In small children, sometimes this has to be done under sedation or even anesthesia, but regardless, it's a dilated fundus exam that lets you determine one from the other.  (+ info)

ok here are a few of the pics i have with her eyes like this...let me know what you think and if you can see the photos
thanks in advance
the latest one is the one that worried me and made me call her dr they didnt act concered on the phone so i called a specilist and she has an apt mon. im am worrying my head off mon is not soon enough. after i saw it in the latest picture i looked backk at the others and found it odd that the same eye had a dif appearence than the other one
thank you all. she kind of has a squint in that eye but not noticable unless its just me cause im soo worried. but the only thing i have noticed is she will not learn her colors all the way. like shell look at it and say a completely dif color and ill have to tell her to look at it and make her look at it for a while b4 she gets the color right. then sometimes shell still get it wrong idk if that has anything to do with her eyes or not.

Its great that you have an appointment for her on Monday. They will be able to examine her thoroughly then. As I said in your other question, there can be other causes of this appearance too, not just retinoblastoma. But photos will not give us any more information to help with the diagnosis.

When you go on Monday for the appointment they will have a look inside her eyes at the health of the lens, retina, etc.

They will also measure the prescription in her eyes.

You added to your other question that you were worried about what you should be asking to make sure she has a thorough examination. The appearance in these photos is something that they will investigate thoroughly regardless of whether you ask any questions. Don't worry, they will do all the relevant checks.

You have a beautiful daughter, I do wish it is not retinoblastoma or anything else worrisome for her

Babies with retinoblastoma?

My son keeps taking pictures with that same left eye showing up like that...it DOES show up red in some picture and normal in some but the majority of the pictures have that glow...Does that look normal to you or do you think it could be a tumour or something...I am really worried...Taking him to a walk-in clinic in about 20 minutes but I just need info now I am going crazy since my brother brought this up to me!!!

http://i5.photobucket.com/albums/y159/FlapaliciousGoon/n660813987_844971_555.jpg
oh the walk in was closed...I copied my question from earlier because I didnt get an answer
----------

There is no way to be certain of what it is without a thorough exam... However, the picture is a little suspicious. It is very possible though that due to the angle of the flash/camera and the way it hit your sons eyes what you are seeing is not due to RB and is absolutely nothing. I do strongly suggest though that you have your son checked out. Other things to look for, a bloodshot eye, a dilated pupil, cross eyes and the presence of the white ring in low light conditions(like a cats would), or you could even try shining light directly in the eye and see if it looks as it does in the picture.
Also 25% of RB cases are inherited... Do you have a history of it in your family?
You can also try to make an appointment with an eye doctor if they could see you faster. It might actually be better to see an ophthalmologist.

I had retinoblastoma, yet i'm still alive?

Hey,
Hope you get what i mean as i'm not a native english speaker. About me;
My right eye started glowing at night when i was about 3 months old and doctors diagnosed tumor and said that i'd die in 9 months unless i had surgery, which my parents didn't have money to do. After few months, tumor stopped spreading, but caused my lens (or anterior chamber?) to become white, non transparent. I had surgery to remove non transparent lens when i was 8 years old. After that, my right eye started seeing colors (before that it could only see black and brown). Now my right eye has no lens, just a liquid produced by body if i'm not mistaken and I have lazy eye. My question is.. Am i still in danger about tumor, like would it spread to brain later on? How come i'm still alive altho i haven't had my right eye removed?

Thanks in advance.
Thank you for fast reply, yagman. If i had misdiagnosis, how come my right eye glowed at night and caused lens to be white? I hope you don't get me wrong (as in if it looked like i'm judging your knowledge). What could be medical name of my illness? Also, doctors cleaned the tumor and lens via inside of my eye, i still have stitch stains at white side of my eye.. Sorry for poor english&writing skills.
OK, turns out i didn't have retinoblastoma at all.. The tumor was at the lens, didn't get inside retina. I'll still have my eye checked once i get my paycheck. It's all my fault that i misdirected you because i didn't ask if tumor was inside retina and found name of term via google. Anyways, thank you very much for answering my question. How do i choose your reply as answer in here? I can't find button/menu for it.
----------

My intitial thought is that possibly you had a misdiagnosis. There are other problems that can cause the lens to become white. I would be very surprised that retinoblastoma would stop growing without removal of the eye. I would recommend another opinion from doctors who have no previous knowledge of your condition.

Edit: Congenital cataracts can cause the same appearance you describe. When the surgeons removed the "tumor" did they send it off for a histological exam? This means they sent the tissue to a lab to have it examined microscopically to assure that it actually is retinoblastoma.  (+ info)

fix your links.

What a cute kid! If ALL her pics turn out like that, take her in for an eye check. My neice has always had one eye glow like that and she's mostly blind in that eye. It got worse as she got older, but her mom was exposed to german measles while pregnant and she's got other health problems as well.  (+ info)

Would this picture worry you? What could it be? Retinoblastoma?

Some photos of my daughters eyes appear white instead of red.She was diagnosed with coloboma. Is this why?

It is never both eyes at the same time. One or the other will appear white and the other red - and sometimes neither is white and both are red. Could this be a side effect of the coloboma and the angle of the photo, or is it more than likely retinoblastoma? Her exam with the specialist that diagnosed her with coloboma was pretty extensive and we assume that he would have seen if it were retinoblastoma.
----------

Wow, I just happened onto this topic, and I saw the word "coloboma".... I also have coloboma. It is not common, so I hardly ever meet anyone else who has it. Anyway, to try and answer your question, what part(s) of her eyes does the coloboma affect? I have coloboma of the optic nerve in one eye, and of retinas in both. My eye doctors have always said that the eye with the optic nerve affected looks like a big, white flower when they dialate it and look into it. And, I have noticed every now and then one of my eyes will look white, or super reflective in a picture. If you daughter has colobomas of the optic nerves, maybe that is the reason that her eyes appear white in pictures sometimes. I don't know anything about retinoblastoma, but I do know that with coloboma of the retina like I have, part of the retina is just plain missing. That could be affecting the appearance in pictures, too. I suppose if you are truly worried about her having a secondary condition, you could get a second opinion from another doctor. Good luck, and if you have any other questions about your daughter's coloboma, please feel free to contact me. : )  (+ info)
