1/5. ultrasonography of Zenker's diverticulum: special reference to differential diagnosis from thyroid nodules.

    There have been only two case reports so far in literature on ultrasonographic features of Zenker's diverticulum. We report a case of Zenker's diverticulum and discuss the ultrasonographic images in comparison with those of thyroid nodules. awareness of changeable internal echoes and not mistaking strong echogenic foci caused by air for calcification were most important for making the differential diagnosis of Zenker's diverticulum from thyroid nodules.

2/5. Sonographic demonstration of a pharyngoesophageal diverticulum.

    We report a case of pharyngoesophageal (Zenker's) diverticulum in a 91-year-old woman. Sonography of the thyroid gland showed diffuse enlargement of the gland and a well-defined, heterogeneous hyperechoic mass that appeared to be in the posterior left lobe. The mass had a smooth hypoechoic wall with a layered appearance anteriorly. Real-time sonography performed during the patient's ingestion of water showed transient changes in the size, margins, and echogenicity of the lesion, which subsequently reverted to its original appearance.

3/5. Incidentally found pharyngoesophageal diverticulum on ultrasonography.

    The pharyngoesophageal diverticulum in the lower part of the neck can be detected using neck ultrasonography. We present a case of pharyngoesophageal diverticulum mimicking a thyroid nodule, which was found incidentally by ultrasonography, and discuss its peculiar findings, which might be useful to diagnose pharyngoesophageal diverticulum and to prevent invasive procedures such as needle aspiration biopsy.

4/5. Fine-needle aspiration of a paraesophageal diverticulum masquerading as a thyroid nodule.

    Fine-needle aspiration (FNA) is a widely utilized cost-effective method for the initial evaluation and triage of thyroid nodules. On occasion, a variety of non-thyroidal lesions can be mistaken for thyroid nodules on physical examination or in imaging studies and aspirated. Paraesophageal/ pharyngoesophageal diverticula masquerading as a thyroid nodule have been reported in the radiology literature but, as far as we know, not in the cytology literature. We describe FNA of a paraesophageal diverticulum that had been reported as a thyroid nodule on ultrasound. Cytologists should be familiar with this entity and suggest the diagnosis when appropriate.

5/5. Extra-pharyngeal neck pathology complicating pharyngeal pouch surgery.

    One case each of: (1) low grade thyroid lymphoma; (2) supraclavicular and para-oesophageal metastasis of a uterine adenocarcinoma; and (3) recurrent multinodular goitre have been encountered in very intimate relationship with the neck of a pharyngeal pouch within the tracheo-oesophageal gutter raising the possibility that the two conditions were interrelated. The practical importance of these cases is that a surgeon excising a pouch from the neck ought to be able to resect a thyroid lobe should it prove necessary, and occasionally endoscopic diverticulotomy is the only reasonable option.
