    varicocele is a relatively common entity that is mostly studied because of the effects on the testes and a presumed role in infertility. hemorrhage secondary to a varicocele, however, is a known but rare morbidity. A case is presented of a ruptured varicocele due to blunt abdominal trauma with a sudden increase in intra-abdominal pressure with transmission to the varicocele. Such a mechanism may also provide an explanation for the occurrence of a more commonly recognized phenomenon of idiopathic spermatic cord hematoma. ( info)

    Testicular microlithiasis associated with seminoma was found in a 32-year-old infertile man. Gray-scale and color Doppler ultrasound findings are discussed and a brief review of the literature is presented. ( info)

    A case of asthenospermia and decreased semen volume with consequent infertility is reported. The patient had large bilateral varicoceles but refused an operation. Significant improvement of volume and motility occurred after treatment with fluoxymesterone and pregnancy was achieved. ( info)

76/80. Sclerosing lipogranuloma of the scrotum following a laparoscopic herniorraphy and varicocelectomy--a case report.

    "Sclerosing lipogranuloma" was first coined to describe a lesion of adipose tissue thought to be due to trauma. Later, analysis of the fat within these lesions showed the presence of exogenous paraffin hydrocarbons and a link with paraffin injection was postulated. In japan, lipogranulomas without a history of trauma or paraffin injection have been described more frequently. Our patient, developed such a lesion in his scrotum following a laparoscopic left herniorrhaphy and varicocelectomy (intra-abdominal approach). He had no prior history of trauma or paraffin injection. A possible relationship is postulated. ( info)

77/80. The diagnosis and treatment of intratesticular varicocele.

    A case of varicocele is reported. The dilated veins were situated in the left testis. The diagnosis required color Doppler sonography. Successful treatment was performed by transcatheter embolization of the left spermatic vein. The control sonogram revealed complete regression. A literature search yielded only three previous cases of intratesticular varicocele. No data were found on the treatment of this entity. ( info)

78/80. Intratesticular varicocele.

    varicocele is a common condition, occurring in approximately 15% of males. We present a case of intratesticular varicocele with concomitant extratesticular varicocele. A routine sonographic examination of the left testis revealed multiple hypoechoic, serpiginous, tubular intratesticular structures of various sizes with low-level internal echoes. Duplex Doppler and color flow examination confirmed a low-flow venous pattern with phasic variation that increased during Valsalva's maneuver. These findings were consistent with intratesticular varicocele. varicocele was also present in the left pampiniform plexus. The main differential considerations in a patient with intratesticular varicocele include cyst, hematoma, epidermoid cyst, and tubular ectasia. ( info)

79/80. The ultrasound appearances of intratesticular varicocoele.

    Extratesticular varicocoele is a common clinical finding, with well recognized sonographic appearances. We report the ultrasound findings in a patient with intratesticular varicocoele (ITV), an extremely rare entity, which was an incidental finding during scrotal sonography. There are differences in both the imaging findings and clinical presentation from previously reported cases. The history of undescended testis and subsequent orchidopexy in our patient contribute to the aetiology of the ITV. ( info)

80/80. Juxta-renal varicocelectomy for recurrent varicocele following retroperitoneal operation.

    Two patients with recurrent painful left varicocele following retroperitoneal varicocelectomy were subjected to juxta-renal varicocelectomy by a transverse supraumbilical incision. Ligature of the testicular vein was carried out just below the renal vein. Minute periarterial veins were disrupted by desheathing the testicular artery. Both patients were cured of their varicocele as judged by scrotal ultrasonography and by color duplex ultrasonography. Average follow-up period was 5 months. ( info)
