1/150. Chronic factitial ulcer of chin cured by endodontic (root-canal) surgery for underlying periapical abscess.

    In a determined search for the cause of a "factitial" ulcer of the jaw, consultation with 3 dentists was required before an underlying periapical abscess was discovered. Within 3 months of endodontic surgery, this ulcer of 12 years duration had completely healed and remains healed. Too often dental infection is neither suspected nor detected as a cause of skin disease.

2/150. Herpetic whitlow in an AIDS patient.

    This case confirms that cutaneous herpes simplex virus (HSV) infections in many AIDS patients is important not only for the difficulty in diagnosis of herpetic lesions, but also for the possibility that co-infection by HSV and hiv can adversely affect prognosis in these patients.

3/150. Extensive oral mucosal ulcerations caused by misuse of a commercial mouthwash.

    This case report describes severe mucosal injuries following misuse of an undiluted over-the-counter mouthwash with a high alcohol content (70%), oil of peppermint and arnica. The mouthwash was to be diluted 5:1 with water. The patient used undiluted solution to better treat her self-diagnosed "contagious gum infection." She experienced burning sensation with each rinse and developed severe mucosal injuries subsequently. Her oral condition improved within 48 hours following discontinuation of use of the mouthwash and application of a mixture of Benadryl Elixir, Maalox Plain, and 2% viscous lidocaine. A detailed history and review of a patient's medical condition will help to differentiate self-induced mucosal injuries from those caused by an allergic reaction or skin diseases.

4/150. Perianal cytomegalovirus ulcer in an hiv infected patient: case report and review of literature.

    We report the case of a 25-year-old man with acquired immunodeficiency syndrome, presenting with perianal ulcer and diarrhea. He had positive immunocytochemical tests for cytomegalovirus (CMV) in circulating polymorphonuclear cells (PMN). The biopsy specimen was suggestive of CMV infection, and specific immunoperoxidase for CMV antigens positively stained endothelial cells and fibroblasts. In this report we review cutaneous CMV infection in immunocompromised patients.

5/150. cowpox infection: not yet consigned to history.

    Two patients presented with hard black eschars covering granulating ulcers. The ulcers healed within 6 weeks to leave small scars. The diagnosis of cowpox was confirmed by serology in both cases and in addition by polymerase chain reaction in the first. The source of infection was likely to be a rodent in the first case whilst the second was more typical in being transmitted by a cat.

6/150. Burli (mycobacterial) ulcers in Caucasians in nigeria.

    Seven Caucasians with Buruli ulcers have been treated. The disease did not behave differently in them from its behaviour in Africans nor has liability to infection any racial basis. Surgical removal with or without skin grafting is the treatment of choice.

7/150. Nasal tip ulceration from infection and extrusion of a nasal alloplastic implant.

    Nasal augmentation rhinoplasty is a common cosmetic procedure. Alloplastic or synthetic materials are frequently used with Silastic, the most commonly used type worldwide. In the Orient, this common procedure has a low complication rate. However, infection and extrusion of the implant through the skin may occur and patients may be reluctant to report the augmentation procedure to the dermatologist. Therefore, the dermatologist needs to be highly suspicious and include this complication in the differential of ulcerated nasal tip nodules.

8/150. Recurrent disseminated herpes zoster and cytomegalic perianal ulcer: a case report and review of the literature.

    We describe a patient with lymphocytic leukemia who developed multiple, disseminated, vesiculopustular eruptions in combination with perianal ulcer. Four years earlier, she had a herpes zoster (HZ) infection involving the ophthalmic division of her left trigeminal nerve with subsequent postherpetic neuralgia that was treated with steroids. After the studies, we concluded that the patient had a recurrent disseminated HZ infection and perianal ulcer caused by cytomegalovirus (CMV).

9/150. An unusual presentation of immunodeficiency with hyper-IgM.

    Hyper-IgM syndrome is a rare immunodeficiency disease characterized by markedly decreased serum IgG, IgA, and IgE levels but normal or elevated IgM levels. The most common clinical signs are infections, cirrhosis, arthritis, malignancies, and mucosal ulcers. Approximately two-thirds of patients have chronic neutropenia associated with oral and perirectal ulcers. We report a 14-month-old girl with hyper-IgM syndrome who has recurrent cutaneous ulcers restricted to the diaper area.

10/150. Diagnostic and therapeutic dilemmas of a large scrotal lesion in an AIDS patient.

    In the setting of hiv infection, chronic genital ulcerations may be challenging both diagnostically and therapeutically. The differential diagnosis of these lesions is very broad, and the causes can be multifactorial. We present a case of a chronic, extensive, ulcerating scrotal mass and review the salient clinical, diagnostic, and therapeutic considerations.
