1/6. arthroscopy-assisted reduction and percutaneous fixation of a multiple glenoid fracture.

Glenoid fractures are rare. The traditional method for treating them is open reduction and internal fixation in arthrotomy. Arthroscopic reduction with percutaneous fixation is used in selected fractures (of tibial plateau, ankle, distal radius). We describe the surgical technique adopted to treat a multiple, Y-shaped articular glenoid fracture using arthroscopy and percutaneous fixation.

2/6. The limited-contact dynamic compression plate in the management of proximal humeral non-union.

Non-union is an uncommon but troublesome complication of proximal humeral fractures. A number of surgical techniques have been reported. We describe the use of a contoured limited-contact dynamic compression plate in the surgical management of proximal humeral non-union.

3/6. Isolated avulsion fracture of the lesser tuberosity of the humerus: report of two cases.

Isolated avulsion fractures of the lesser tuberosity of the humerus are exceptionally rare. Two cases are reported here, each involving a 12-year-old boy. The mechanism of injury appears to be a strong external rotatory force applied while the arm is at maximum external rotation and approximately 60 degrees of abduction. In children and adolescents the fracture most likely occurs through the apophyseal plate of the lesser tuberosity (traction epiphysis). In adults, clinical differentiation from the far more common calcific peritendinitis of the rotator cuff can be difficult. Whereas large, displaced fractures can be easily demonstrated on anteroposterior radiographs with internal and external rotation of the humerus, the axillary view is often necessary to detect smaller fragments with little displacement.

4/6. Comminuted fractures of the proximal humerus treated with hook plate.

Good results were obtained using a hook plate to fix bone in 18 cases of fracture of the proximal humerus in which the bone was greatly comminuted or dislocated. The hook plate protects the fixed area against rotation, shear stress and flexion much better than other fixation methods.

5/6. Interposition of the tendon of the long head of biceps in fracture separation of the proximal humeral epiphysis.

Three cases of interposition of the tendon of the long head of biceps and anterior periosteum occurring in fracture separating in the proximal humeral epiphyseal cartilage plate are recorded. The operative findings are described and the treatment briefly discussed.

6/6. Stress fracture of the proximal humeral epiphysis in an elite junior badminton player.

An elite junior badminton player presented with a chronic painful dominant shoulder after an intense training course. An acute stress fracture to the proximal humeral epiphysis was found. Two-plane radiography will identify abnormalities of the growth plate but comparative films of the unaffected side may also be required to differentiate subtle changes. rest with subsequent rehabilitation is the appropriate management of these injuries although ideally they should be subjected to primary prevention.
