1/3. Epidural anaesthesia for caesarean section in a patient with extreme cardiovascular and respiratory disease.

    A 24-year old booked primigravida, with rheumatic heart disease in heart failure and lobar pneumonia presented in active labour. She was stabilized and prepared for an emergency Caesarean section that was successfully managed with Epidural Anaesthesia. She was admitted into the intensive care unit where the pneumonia and heart failure were managed by the physicians. The importance of proper follow up and treatment and, the need to perform more epidural techniques to meet the ever increasing challenges to the Anaesthetist are highlighted.

2/3. Parapneumonic empyema. A pitfall in diagnosis.

    Two patients eventually shown to have empyema were encountered in which the initial thoracentesis revealed fluid compatible with either a simple or a complicated parapneumonic effusion. In both cases, the diagnosis of empyema was made by a second thoracentesis done at a close interval of time from a different site. Therefore, the physician should approach parapneumonic effusions systematically, and remember that in some cases, multiple thoracenteses may be required to make the correct diagnosis of an empyema.

3/3. neuroleptic malignant syndrome.

    neuroleptic malignant syndrome is an uncommon, occasionally lethal reaction to drug therapy. patients taking neuroleptic medication are usually the victims of this complex disorder, but others, such as patients with Parkinson's disease, are also at risk. The classic presentation includes autonomic instability, rigidity, hyperthermia, confusion and other neurologic symptoms. family physicians may be the first to see these patients and must be able to make the diagnosis quickly to avoid delay in treatment.
