1/4. thrombosis and infection: a case of transient anti-cardiolipin antibody associated with pylephlebitis.

Infections with bacteroides species have been noted to occur in association with cases of thrombophlebitis. This association has led to the speculation that the microorganisms themselves may contribute to the pathogenesis of thrombus formation through elaborated enzymes, including heparinases, or by interactions between the clotting cascade and the unique structure of the bacteroides lipopolysaccharide. Anti-phospholipid antibodies have been linked with hypercoagulable states and thrombus formation. Although a number of infections have been associated with the transient production of anti-cardiolipin antibodies, the effect the antibodies may have in contributing to thrombus formation is not well understood. The occurrence of bacteroides species infection with transient anti-cardiolipin antibody has not been previously reported.

2/4. Septic phlebitis caused by candida albicans and diagnosed by needle aspiration.

We have described a patient who had candida albicans suppurative thrombophlebitis at the site of an indwelling intravenous catheter. The microbial etiology was established by needle aspiration of the venipuncture site, affording rapid and accurate identification of the infecting organism and providing prompt institution of therapy. Phlebectomy and a brief course of amphotericin b prevented further dissemination of the infection.

3/4. Papillophlebitis associated with systemic thrombophlebitis: case report.

This 36-year-old white man developed signs and symptoms of mild reversible papillophlebitis associated with calf thrombophlebitis and subsequent mammary and antecubital vein phlebitis after fluorescein angiography. This in flammatory process in conjunction with phlebitis elsewhere in the body strongly suggests that localized phlebitis of the central retinal vein, perhaps near its exit from the optic nerve, is etiologic in papillophlebitis. Response to corticosteroids tends to support this theory.

4/4. Pylephlebitis: a case report and review of outcome in the antibiotic era.

Pylephlebitis or septic thrombophlebitis of the portal vein, a precursor of hepatic abscesses, is an extremely rare and frequently fatal complication of diverticulitis. The following report describes a patient presenting with pylephlebitis and complicated diverticulitis. diagnosis was confirmed by computed tomography. The patient had a favorable outcome with medical and surgical therapy, prompting us to evaluate historical treatment of pylephlebitis.
