11/13. Pseudoinfarction ECG pattern caused by diaphragmatic hernia uniquely resolved by transthoracic echocardiography.

    A case of pseudoinfarction electrocardiographic pattern caused by diaphragmatic hernia is presented. The transthoracic echocardiographic examination revealed a large hiatal hernia enhanced by contrast injections below the inferior wall causing paradoxical systolic motion. The location of the hernia simulated a pericardial effusion.

12/13. Echocardiographic observation of ventricular volume changes and swinging septal position in massive pericardial effusion.

    Swinging interventricular septal position and ventricular volume changes associated with respiration were studied in a patient with massive uremic pericardial effusion and pulsus paradoxus. Inspiration was regularly associated with a posterior swinging septal position which dramatically increased right ventricular (RV) volume with a lesser change in left ventricular (LV) volume. Expiration was consistently accompanied by an anterior swing of septal position which nearly obliterated the RV with slight enlargement of the LV. The above changes produced a pendulum-like motion of the septal position as the patient breathed. This may reflect the ventricular volume changes that accompany respiration in severe pericardial effusion association with pulsus paradoxus.

13/13. Cardiogenic shock following recombinant alpha-2b interferon therapy for chronic hepatitis c. A case report.

    A 57-year-old woman with chronic hepatitis c was treated with alpha-2b interferon (IFN). Forty-five days after the initiation of IFN therapy, she developed cardiogenic shock. Acute perimyocarditis as a cause of cardiogenic shock was clinically suspected by the findings of complete atrioventricular block, regional wall motion abnormality and pericardial effusion. Since IFN therapy may induce cardiogenic shock in some patients, it is important to carefully monitor patients under treatment with IFN for abnormal cardiac signs.
