1/21. Sinus polyp-associated soft tissue lesion and unilateral blindness: complications of extraction in leukemic patient.

    A case of an inflammatory polyp-associated lesion extending through an extraction socket appearing as an intraoral nodular lesion and unilateral blindness secondary to leukemic optic nerve head infiltration is reported. The patient was a 28-year-old male whose his upper first molar had been extracted fifteen days previously. The lesion was an asymptomatic soft tissue mass, red in color and hot tender to palpation, involving the alveolar ridge in the maxillary molar area. Although this is apparently a rare occurrence, the nature of the lesion was suggested by the history, clinical appearance, and radiographic findings. Excision of the inflammatory lesion was followed by complete healing with closure of the lesion. Unfortunately, the blindness was irreversible. The patient is still under leukemia therapy. review of the literature did not yield any other such cases. The role of oral lesions as a diagnostic indicator and the importance of dental surgeons in the diagnosis of leukemic patients are discussed. It is concluded that proper precautions and meticulous early diagnosis are required in these patients and that dental practitioners should be aware of the diagnostic features and possibilities of oral complications associated with leukemia.

2/21. Dermoid (hairy polyp) of the nasopharynx.

    Dermoids or hairy polyps, the most primitive forms of teratoma are rare lesions affecting the nasopharynx. They arise during early embryogenesis, being derived from only two germinal layers, ectoderm and mesoderm and are invariably benign. They usually present at or soon after birth with signs of upper aerodigestive tract obstruction. A case of hairy polyp of nasopharynx in a neonate is reported for its rarity. The etiology of teratomas, their classification in head and neck region, clinical features, and management in neonates are discussed.

3/21. Coexistence of benign schwannoma and lymphoma in a nasal polyp.

    The existence of a combined benign schwannoma and lymphoma presenting as a nasal polyp has not been described in the English literature. We are reporting this rare combination in a 50-year-old male whose presenting symptoms were nasal obstruction, nasal deformity and headache. Examination of the left nasal cavity revealed a mass which was confined to the nose on computed tomography (CT) scan examination. Histopathology of the mass revealed a major component to be a benign schwannoma and a minor component a large B-cell lymphoma.

4/21. MR imaging appearance of frontal sinus barotrauma.

    We present the case of a flight passenger who experienced acute and severe headache during landing. MR imaging was performed because the patient had a history of vascular malformation and revealed an incidental venous angioma. A mass lesion in the frontal sinus, consistent with submucosal hematoma secondary to barotrauma, was thought to be the cause of the headache. To our knowledge, this is the first case of sinus barotrauma described in the radiologic literature and the first to describe the associated MR imaging findings.

5/21. Pai syndrome: an adult patient with bifid nose and frontal hairline marker.

    Objective: A 17-year-old previously unreported patient with Pai syndrome is described. The boy had median cleft of upper lip, a polypoid skin mass over the columella, a minimal cleft of the upper central incisors, frontal alopecia of the anterior hairline, and bifid nose. magnetic resonance imaging showed pericallosal lipoma. No mental retardation was present, and a chromosomal study showed normal male 46, XY karyotype.

6/21. Unusual clinical manifestation of virus-associated hemophagocytic syndrome.

    A 22-year-old man presented with bilateral painless cervical lymphadenomegaly, difficulties in nasal breathing and bilateral conductive hearing loss. Rhinoscopy and computer tomography disclosed mucosal polyps in the nasal cavity and a polypoid soft mass almost completely filling the whole nasal cavity and the paranasal sinuses. Thoracic and abdominal computer tomography showed mild hepatosplenomegaly and a solitary round lesion in the right lung. No fever or abnormal laboratory parameters were detected. The biopsy from the nasal polypoid lesions and from the cervical lymph nodes showed extensive proliferation of histologically benign erythrophagocytic histiocytes. The diagnosis of virus (Epstein-Barr virus)-associated hemophagocytic histiocytosis was confirmed by immunohistochemical reactions, by polymerase chain reaction, and by Epstein-Barr-Encodes (Early)-rna in situ hybridization. This case illustrates an unusual clinical manifestation of virus-associated hemophagocytic histiocytosis presenting as mucosal polyps of the upper respiratory tract.

7/21. Rosai-Dorfman disease revealed in the upper airway: a case report and review of the literature.

    Rosai-Dorfman disease (RDD) is a systemic disease and its etiology is not well understood. It is a very rare but established disease. We report a case of RDD diagnosed in the upper airway that became a life-threatening condition because of the stricture of the subglottic space. A 49-year-old female complained of nasal obstruction, hoarseness and lacrimation. After evaluation following by fiberscopic examination and CT scanning of the head and neck, sinusitis and laryngeal masses were detected. Endoscopic sinus surgery and endolaryngeal microsurgery were performed simultaneously. The masses in the nasal cavity and larynx showed the same histological findings. Proliferative large histiocytes with voluminous clear cytoplasm and rounded nuclei, and lymphophagocytosis known as emperipolesis are the characteristic features of RDD. In this case, extranodal manifestations in the subglottic space gradually emerged postoperatively. tracheotomy was performed to ensure an airway and prevent suffocation. An extensive treatment is not required in most patients with RDD, however, some patients who have extranodal manifestations involving the airway may detoriorate into a life-threatening condition.

8/21. Pai syndrome (median cleft palate, cutaneous nasal polyp, and midline lipoma of the corpus callosum): a case report and literature review.

    In this article, we describe a patient with Pai syndrome. This infant was born with an unusual median cleft of the upper lip, a pedunculated cutaneous mass that protruded from the right nostril, double frenulum of the upper lip, and median alveolar cleft. MRI showed a midline corpus callosal lipoma. Mental development was normal and chromosomal analysis revealed a normal male 46, XY karyotype.

9/21. Choanal polyp originating from the inferior turbinate presenting as nasal polyposis.

    Although choanal polyps frequently arise from the maxillary sinus, a choanal polyp originating from the inferior turbinate is a rare entity. A 14-year-old girl who had a history of bilateral nasal obstruction with snoring, mouth breathing and nasal discharge for 10 years was presented. On endoscopic examination, a polypoid mass completely obliterated the right nasal cavity. The left choana was also totally occluded by the polypoid mass. Endoscopic polypectomy was performed and it was observed that the polyp originated from the right inferior turbinate. Choanal polyps arising from the inferior turbinate should be kept in mind in the differential diagnosis of unilateral nasal polypoid masses.

10/21. Median clefting of the upper lip associated with cutaneous polyps.

    Details are presented of a patient with median clefting of the upper lip and cutaneous polyps. Four similar published case reports are considered to share the same condition. The spectrum of the disorder encompassed by these five cases is discussed.
