1/21. Surgical simulation of Class III edentulous patient using a 3D craniofacial model: report of a case.

    A case of edentulous prognathism in a 46-year-old Japanese male is presented. We described the outcome of the patient who underwent simultaneous sagittal splitting ramus osteotomy of the edentulous mandible, interpositional bone graft of severely atrophic edentulous jaws, and delayed placement of titanium implants for reconstruction. We highly recommend performing a surgical simulation using a craniofacial model of the patient's anatomy created using CT image data. The procedure provides almost ideal maxillary and mandibular contours.

2/21. Advanced alveolar crest atrophy: an alternative treatment technique for maxilla and mandible.

    A concept of oral implantology for the treatment of advanced crest atrophy is presented: The lateral insertion technique with disk-design implants is less invasive than bone grafting. An implant case demonstrates the simultaneous surgery of an edentulous maxilla and a mandible. Implant loading zones in the anterior and posterior areas of the arch are created with disk-design implants. Their support is tricortical or multicortical. Seven to eight days after surgery, the implants are immediately loaded with fixed esthetic temporaries. After another 40 days, the definitive restorations on the implants can be fabricated. The procedure is safe and shortens chairtime.

3/21. Immediate loading of dental implants in the edentulous mandible: a preliminary case report from an international prospective multicenter study.

    The ability to predictably achieve long-term osseointegration in patients with compromised anatomical resources has been demonstrated numerous times in modern oral implantology. Recently, clinical attention has focused on new methods of reducing treatment time. One-stage surgical procedures and immediate loading of implants at the time of placement are two techniques that have demonstrated promising clinical results. A prospective clinical study of immediately splinting and loading a new, one-stage implant is currently in progress in the united states and france. An overview of the implant design and presentation of one case study from the University of Pittsburgh demonstrates how this promising technique is performed.

4/21. Reconstruction of the severely resorbed atrophic edentulous ridge of the maxilla and mandible for implant rehabilitation: report of a case.

    We describe a case with a severely resorbed atrophic edentulous ridge in both the maxilla and mandible. The maxilla was reconstructed using a sinus-lifting procedure and onlay bone graft. The mandible was reconstructed by anterior osteotomy with an interpositional sandwich iliac bone graft at the symphysis area, subperiosteally with iliac bone chips mixed with hydroxylapatite bilaterally at the posterior atrophic ridge, and vestibuloplasty with a split thickness skin graft (STSG). After full-mouth implant rehabilitation, a 5-year follow-up of this case shows a satisfactory result.

5/21. Complete denture treatments for a cerebral palsy patient by using a treatment denture. A case report.

    This article describes the prosthodontic treatment for a patient with cerebral palsy, in which complete dentures were successfully stabilized using treatment dentures. A 69-year-old edentulous male with no medical complications or mental retardation presented to our clinic. Opening movement of the jaw was possible, but a conspicuous mandibular shift towards the right was observed. He had never received any prosthodontic treatment. Initially, treatment dentures with flat tables were fabricated to rectify his erratic mandibular movements. During the first 3 weeks, the treatment dentures functioned poorly. Eventually, the patient could make tapping movement to some degree and have a meal with less effort. Indentation marks from the cusps of the opposing maxillary denture could be clearly seen on the flat tables. After six weeks, as he did not complain of any pain, definitive dentures were fabricated. When flat table treatment dentures are used, it is considered that the mucosa provides information regarding the vertical stop and bite force. In addition, it is speculated that there is an increase in the response from masseter muscle. In the present case, flat tables were effective for rehabilitation of the mandibular movement.

6/21. Teeth in a day for the maxilla and mandible: case report.

    BACKGROUND: A growing body of evidence indicates that successful osseointegration of dental implants can take place in the wake of immediate loading, providing that bone quality and quantity are adequate, and patients follow postsurgical instructions carefully. PURPOSE: The goal of this report is to demonstrate the efficient treatment protocol based on immediate loading for both the maxilla and mandible, including extraction site locations. MATERIALS AND methods: Following extraction of the remaining anterior mandibular teeth, 18 Branemark implants (Nobel Biocare AB, Gothenburg, sweden), including two zygoma and two pterygoid implants, were installed in both arches in accordance with the Teeth in a DayTM protocol developed by the authors 9 years ago. This protocol uses an acrylic screw-retained prosthesis, with steel prosthetic copings embedded, supported by full-size Branemark implants to prevent micromotion at the bone-to-implant interface. RESULTS: Only 1 of the 18 immediately loaded implants failed to osseointegrate. Three years after completion of treatment, the patient reported functioning well with no complications. CONCLUSIONS: When appropriate subjects are selected, the Teeth in a Day protocol offers patients a number of significant advantages, including condensed treatment time, reduced postsurgical discomfort, and almost instantaneous improvement in speech and masticatory function, esthetics, and patient self-image.

7/21. Immediate loading with complete implant-supported restorations in an edentulous heavy smoker: histologic and histomorphometric analyses.

    The clinical case presented is that of an edentulous female patient, a heavy smoker, who received implant-supported complete restorations in the maxilla and mandible using the immediate loading concept according to the Ankylos implant system. The patient received 12 commercially pure titanium (grade 2) Ankylos implants, 6 in the maxilla and 6 in the mandible. The implants were loaded immediately after surgery with temporary acrylic resin prostheses fabricated chairside using a prefabricated customized splint. The definitive ceramometal restorations were seated 4 months after surgery. Clinical and radiologic evaluation at 7 months after implant placement indicated functional bone anchorage of all implants, despite the patient being a smoker and having poor bone quality. The patient died 7 months after implant placement because of lung cancer; however, there was no known disease at the time of implant placement. After her death, the implants with the surrounding tissues were removed en bloc and examined histologically and histomorphometrically using undecalcified cut and ground sections. All implants were osseointegrated to some extent and surrounded by lamellar bone. However, around the upper, nonthreaded parts of the implants, much of the bone had been resorbed. In this region, fibrous connective tissue was in close contact with the titanium surface. Epithelial proliferation with pocket formation could not be observed in any of the implants. The histomorphometric evaluation of bone-implant contact in threads demonstrated a mean of approximately 51% of the available surface and a mean bone volume of approximately 52%, with a tendency toward greater contact and volume around the implants in the maxilla. If the nonthreaded cylindric portions of the implants were included, mean bone-implant contact was 46% and mean bone volume was 47%.

8/21. Report of a case receiving full-arch rehabilitation in both jaws using immediate implant loading protocols: a 1-year resonance frequency analysis follow-up.

    BACKGROUND: Immediate occlusal implant loading has been documented as a viable treatment option for various indications. However, documentations related to full-arch rehabilitation are usually limited to treatment of one jaw at a time, thereby leaving the opposing dentition unchanged. Furthermore, clinical documentation using traditional, well-accepted measuring techniques may not be adequate when it comes to short-term evaluation of the success or failure of implants subjected to immediate occlusal loading. PURPOSE: The purpose of this case report is to (1) present an implant stability follow-up of a patient receiving an immediate, implant-supported full-arch rehabilitation in both jaws and (2) evaluate the patient's acceptance of this rehabilitation. MATERIALS AND methods: A 68-year-old patient scheduled for implant treatment was selected for an immediate implant loading protocol in both jaws. During two surgical events 3 weeks apart, eight maxillary and four mandibular Branemark System Mk IV TiUnite fixtures (Nobel Biocare AB, Goteborg, sweden) were inserted and subsequently used to immediately support a cross-arch fixed prosthesis in the maxilla and a bar-retained overdenture in the mandible. Implant stability was recorded from the day of surgery periodically during a 1-year follow-up using resonance frequency analysis (RFA). RESULTS: At the 1-year follow-up, based on clinical, RFA, and radiographic evaluations, all implants and the reconstructions were classified as successful. All maxillary implants showed a decrease in the implant stability quotient (ISQ) value from the measurement at the time of surgery to the first follow-up, whereas two of four mandibular implants revealed an initial drop in stability. Irrespective of a specific ISQ level measured at implant surgery (ISQ range 53-74) and despite an initial decrease in stability, measurements recorded at the 12-month follow-up indicated similar stability levels for all maxillary implants (ISQ range 64-68) or the group of mandibular implants (ISQ range 72-75) but with a higher ISQ level for mandibular implants. Furthermore, the patient's acceptance of the immediate full-arch rehabilitation in both jaws was high. CONCLUSIONS: The present case report demonstrates that a slightly staged approach for full-arch rehabilitation in both jaws using immediate implant loading protocols is a realistic treatment option. Furthermore, RFA follow-up indicates that immediately occlusally loaded implants placed in reduced bone quality and quantity are more prone to loose stability in the early healing period compared with implants placed in dense bone quality.

9/21. tooth position in full-mouth implant restorations--a case report.

    Reconstructing the edentulous mouth with opposing, fixed, implant-supported metal-ceramic restorations demands great attention to detail. This article reviews the basic principles of tooth position in dentate patients and in complete denture prosthodontics and relates them to the edentulous patient undergoing implant restoration. Determining gnathologic points and recognizing anatomic landmarks found in the edentulous jaws aid in correct tooth positioning, resulting in proper comfort, function, speech, esthetics, and a prearranged occlusal scheme, all of which are critical factors behind restorative-driven implant dentistry.

10/21. Prosthodontic treatment during active osteonecrosis related to radiation and bisphosphonate therapy: a clinical report.

    osteonecrosis of the jaws following the use of bisphosphonate drugs has been reported in the literature. Presently, there is limited evidence to establish guidelines for the prosthodontic management of patients with active osteonecrosis, a history of osteonecrosis, or medical history of using these medications. This clinical report reviews the current literature regarding bisphosphonate use, and reports the prosthodontic treatment of an edentulous patient with active osteonecrosis who had a history of oral bisphosphonate use and jaw irradiation.
