1/322. Non-surgical treatment of meningioma: a case report and review.

    A woman initially presented with a right hemiparesis and subsequently underwent subtotal resection of a left parietal meningioma arising from the lateral wall of the sagittal sinus. She again presented 18 months after surgery with a hemiparesis and repeat MRI showed tumour spreading into the sagittal and the transverse sinuses. She had a therapeutic abortion at 8 weeks gestation 3 weeks prior to her representation. gestrinone, a synthetic steroid and an antiprogesterone was commenced. Two months later she stopped her medication and is asymptomatic at 16 months. A follow-up MRI revealed that the tumour had shrunk dramatically. This case is the first of its kind with tumour size reduction to less than 20%. We feel that the future of meningioma treatment will be multi-disciplinary and non-surgical options should be considered.

2/322. aneurysm occurring within a meningioma: case report.

    Aneurysms can be expected to be found in approximately 0.5% of patients with brain tumours; nevertheless, the real incidence is difficult to assess because angiography is now seldom performed for brain tumours. In the literature, 42 cases of meningioma associated with aneurysms are reported, but in none was the aneurysm intratumoural. We describe a case of intracranial meningioma with an intratumoural aneurysm in a 48-year-old woman.

3/322. Primary pulmonary meningioma presenting as lung metastasis.

    A benign primary pulmonary meningioma, an extremely rare tumour, was incidentally detected in a 57-year-old woman in association with a contralateral pulmonary adenocarcinoma. The meningioma was initially suspected to be a metastasis. Both tumours were excised, with excellent outcome. Anatomic features of primary pulmonary meningioma and differential diagnosis are discussed.

4/322. meningioma followed up for radiological findings before and after radiosurgery: case report.

    The present paper reports on serial image changes before and after radiosurgery regarding a falx meningioma incidentally discovered in a 79-year-old female. The tumor doubling time during the 3-year observation period before radiosurgery was 469 days. Following radiosurgery using a gamma knife, tumour volume temporarily increased, but then reduced to 55% in 22 months. No symptoms or signs are observable at present, when the patient is 84 years old. These findings suggest that in treating elderly patients with meningioma diagnosed by imaging, radiosurgery should be the first method considered. Also presented are changes in image findings over time.

5/322. Biologic heterogeneity of angiomatous meningiomas.

    Three angiomatous meningiomas, classified histologically as benign, were analyzed cytogenetically and examined for the expression of EGF/PDGF and their receptors by immunohistochemistry. An accumulation of p53 protein and the presence of mutations in exons 5-8 of the p53 gene in neoplastic cells were also determined. In one tumour, chromosome studies revealed near diploid karyotype with the loss of chromosome 22. Two other meningiomas revealed tetraploid karyotypes with the presence of telomeric associations and a wide spectrum of numerical, complex chromosome aberrations. Moderate EGF and EGFR immunoreactivity was found in three and one meningioma, respectively. All tumours exhibited diffuse PDGF and PDGFR-beta expression. No p53 gene mutations were found, but one tumour expressed strong and dispersed p53 immunopositivity. This findings reflect the biological heterogeneity of angiomatous meningiomas.

6/322. choroid plexus papilloma of cerebellopontine angle with extension to foramen magnum.

    A case of choroid plexus papilloma resembling meningioma of cerebellopontine (CP) angle with its extension to foramen magnum is presented. Occurrence of this tumour in CP angle is very rare. Its extension towards foramen magnum is further rare. It was a real diagnostic enigma preoperatively as the tumour was resembling meningioma upto some extent on radiological study. Retromastoid craniectomy with microsurgical excision of tumour and its extension was achieved in toto. Tumour was attached to few rootlets of lower cranial nerves which were preserved. Attachment of the tumour with lower cranial nerves again caused diagnostic confusion with neurofibroma intraoperatively.

7/322. Primary pulmonary malignant meningioma.

    Fewer than 20 cases of primary pulmonary meningioma have been reported. Most of these cases have been histologically and clinically benign. We report an unusual case of primary pulmonary malignant meningioma with atypical histologic features and malignant behavior. A computed tomography scan of the head did not show evidence of tumor. The right upper lobe mass was resected and showed features of an atypical meningioma with loss of architectural pattern, mild nuclear pleomorphism, increased mitotic counts (up to 15 mitotic figures per 10 high power fields), and focally prominent nucleoli. Focally, cells with rhabdoid features were identified. The tumor's immunohistochemical and ultrastructural profiles were consistent with a meningioma. The tumor stained negative for estrogen and focally positive for progesterone receptors and had a MIB-1 labeling index (marker of cell proliferation) of 9.2%. Approximately 5 months after the initial resection, the patient experienced a tumor recurrence with multiple lymph node metastases, spread to the middle and lower lobes of the right lung, and metastasis to the diaphragm. Rarely, primary pulmonary meningiomas may present as high-grade malignant lesions.

8/322. Presumed intraventricular meningioma treated by embolisation and the gamma knife.

    A 58-year-old woman with a presumed incidentally discovered meningioma in the left lateral ventricle was treated by superselective embolisation and gamma knife therapy. The diameter of the tumour was 40 mm, and its main feeding artery was the left lateral posterior choroidal artery. This vessel was embolised with microcoils. At 8 months following embolisation, the diameter of the tumour had decreased and was stable. The gamma knife was chosen as an adjuvant therapy for the further control 13 months after embolisation. Embolisation and gamma knife therapy may be an alternative treatment for meningiomas where surgical resection appears difficult.

9/322. thallium-201 SPECT of adjacent intracranial tumours: a contrast in thallium kinetics.

    We report a case of adjacent intracranial tumours: malignant fibrous histiocytoma (MFH) and meningioma. thallium-201 single-photon emission computed tomography demonstrated different thallium kinetics between the tumours (slow washout from the MFH and rapid clearance in the meningioma) and could be said to have been useful for preoperative histological estimation.

10/322. Unique radiological appearance of a microcystic meningioma.

    A 35-year-old female presented with partial complex seizures. Computed tomography (CT) showed a slightly high density mass over the right frontal convexity, with heterogeneous contrast enhancement. T1-weighted magnetic resonance (MR) imaging showed the tumour as a hypo-intense lesion, with faint reticular enhancement after intravenous injection of gadolinium-diethylenetriaminepenta-acetic acid. The tumour was totally removed. The specimen was extremely soft and moist. The histological diagnosis was microcystic meningioma. The tumour cells were composed of typical meningothelial cells and stellate cells. The degenerative character of the tumour may be reflected in the poor enhancement on CT and MR imaging. This faint enhancement effect may be a neuro-imaging characteristic indication of this rare microcystic variant of meningioma.
