1/3. Urological findings in inguinal hernias: a case report and review of the literature.

    We report on a rare case of massive incarcerated inguinoscrotal bladder herniation in a direct hernia and present the review of the literature on urological findings in relation to the inguinal hernial sac. The English-based literature was searched using the words inguinal hernia, bladder, ureter, diverticule, and incarceration and discussed in relation to the present case. We found 190 cases of inguinal hernia associated with urological findings, such as herniation of the bladder, ureter, and diverticulum. We also found that 11.2% of these hernias were associated with urological malignancies and 23.5% of these were associated with a variety of complications. The high-risk patients, who are males, obese, older than 50 years and who have symptoms that indicate urological pathologies to a physician, are more likely to be in the high-risk group for bladder herniation.

2/3. Paratesticular well-differentiated, adipocytic type liposarcoma presenting as inguinal hernia.

    Paratesticular masses can pose difficult diagnostic and therapeutic problems to the physician. We report a rare case of paratesticular liposarcoma with the clinical symptomatology of an inguinal hernia. The treatment was surgical and included radical orchiectomy and wide excision of the tumor mass to the macroscopically healthy margins. The patient is well and with no evidence of recurrence 1 year after the operation. We review the literature, and discuss the role of radical orchiectomy, radiation, and chemotherapy in the treatment of paratesticular liposarcomas.

3/3. Unsuspected phaeochromocytoma presenting during surgery.

    A patient with an unsuspected phaeochromocytoma presented with transient hypertension during anaesthesia for an elective hernia repair, and developed severe post-operative hypotension. The management of this acute crisis, appropriate preoperative preparation and the subsequent successful removal of the tumour is described. awareness of this unusual presentation may help the attending physician to save such a patient.
