1/6. Combined cutaneous hamartoma encompassing benign melanocytic naevus, vellus hair cyst and epidermoid cyst.

    We describe a combined cutaneous hamartoma in a 57-year-old man, which comprised a vellus hair cyst, an epidermoid cyst and an intradermal melanocytic naevus in a single facial tumour. The vellus hair cyst was filled with keratinous material with unusual slit-like lacunae. While numerous cases of epidermoid cyst have been reported in association with an intradermal melanocytic naevus, our case is a rare example of a lesion combining several hamartomatous elements.

2/6. Proliferating trichilemmal tumor with a malignant spindle cell component.

    BACKGROUND: Proliferating trichilemmal tumors (PTTs) are uncommon exuberant growths of cells derived from the external root sheath. They tend to occur in older women, with a predilection for the scalp. Resection is usually curative. Spindle cell components arising within PTTs are exceedingly rare, with only three known cases previously reported. Of these three cases, one patient died from widespread metastasis. methods: Histological and immunohistochemical examinations were performed on a tumor from a 54-year-old man which, after exhibiting slow growth for 3 years, increased rapidly in size. RESULTS: Microscopic examination revealed cystic lobules of squamous cells without a granular cell layer surrounding amorphous eosinophilic debris within the dermis. Also present was a poorly circumscribed focus of spindle cells with hyperchromasia, pleomorphism, giant cells, and abundant mitoses, including abnormal forms. immunohistochemistry showed the spindle cells to stain positively for vimentin, but negatively for keratin, epithelial membrane antigen and S-100 antibodies. CONCLUSION: A malignant spindle cell component arising within a PTT is a rare neoplasm which has been associated with death from widespread metastasis.

3/6. Pigmented terminal hair cysts within an intradermal melanocytic naevus.

    The combination of melanocytic naevi and cysts of adnexal or epithelial origin has been described previously. Herein we describe a case of intradermal melanocytic naevus containing pigmented terminal hair cysts, a rare variant of epithelial cysts. Our observation suggests that pigmented follicular cysts and combined cystic-melanocytic lesions may be attributed to follicular occlusion.

4/6. CD34-reactive trichodiscoma.

    BACKGROUND: Trichodiscomas are rare hamartomas of the dermal portion of the hair disc, a specialized component of the perifollicular mesenchyme. They are usually found as asymptomatic multiple skin-colored papules on the face and extremities and may have an autosomal dominant inheritance pattern. However, a solitary variant has been described. CASE REPORT: A 78-year-old woman presented with a single, non-pigmented, firm papule on the left tip of the nose, measuring 3.5 mm in diameter. RESULTS: The histological examination revealed the previously described features of a trichodiscoma. The immunohistochemical analysis showed strong immunoreactivity for CD34 in the spindle cell component. Spindle cells were negative for S-100, HMB-45, Melan-A, EMA, neurofilament, desmin, and Factor XIIIa by immunohistochemistry. CONCLUSIONS: We report strong reactivity for CD34 in the spindle cell component of a trichodiscoma. We suggest that this lesion be considered in the differential diagnosis of any CD34 dermal spindle cell proliferation, in which an adjacent epithelial component cannot be entirely excluded.

5/6. Lipomatous mixed tumor with follicular differentiation of the skin.

    A very rare case of cutaneous mixed tumor with numerous adipose tissue and hair follicular structures in a 67-year-old Japanese male was reported. A well-circumscribed tumor was in the subcutaneous tissue of the cheek and far from the parotid gland. Histologically, the tumor consisted of an admixture of the adipose tissue, fibromyxoid tissue with spindle cells, and branching tubular structures. Outer layers of ductal epithelial cells and single spindle cells were often in a transition, likely as typical mixed tumor. Branching ducts connected with keratinous cysts, strands of trichoblastic basophilic cells and clear cell nests. There was a gradual transition, between small-sized adipocytes and vacuolated spindle cells. No chondroid stroma was seen. To our knowledge, this tumor is the first case of a lipomatous mixed tumor with hair follicular differentiation. The case indicates an additional wide spectrum of histologic appearances of cutaneous mixed tumor.

6/6. Pilar neurocristic hamartoma: its relationship to blue nevus and equine melanotic disease.

    A unique pigmented lesion, judged to be a hamartoma of neural crest origin, occurring in a female patient, is compared with equine melanotic disease, The characteristic perifollicular arrangement of pigment-laden spindle cells is remarkably similar in both. Previously described patch- and plaque-like blue nevi in humans are also closely related. light and ultrastructural features showed differentiation toward both nevus cells and schwann cells, and it is proposed that the lesion be termed pilar neurocristic hamartoma.
